RE: DONATION TO AMBULANCE SERVICE.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant
to Colorado Statute and the Weld County Home Rule Charter, is vested with the
authority of administering the affairs of Weld County, Colorado, and
WHEREAS, it is the policy of the Board of County Commissioners, in the event
that donations are received, that they be received formally by the Board of County
Commissioners and the designated purpose indicated by the Board, and
WHEREAS, the Weld County Ambulance Service has received a donation of ONE
HUNDRED FIFTEEN DOLLARS ($115.00) from Bess Fryzek, her friends and relatives in
memory of her husband, Frank Fryzek, and
WHEREAS, the donation to the Weld County Ambulance Service was to be used
for the benefit of the employees, and
WHEREAS, the Director of the Ambulance Service has indicated that the funds
could be used to enhance the Ft. Lupton station by the purchase of a new
mattress.
NOW, THEREFORE, BE IT RESOLVED by the Board of Commissioners of Weld County,
Colorado, that the above donation is received and that it can be used for the
recommended purpose by the Director of Ambulance Service, which is to acquire a new
mattress in the Ft. Lupton Station, and
BE IT FURTHER RESOLVED that a copy of this resolution and a letter of thanks
be forwarded to Mrs. Bess Fryzek.
The above and foregoing resolution was, on motion duly made and seconded,
adopted by the following vote on the 20th day of September, A.D. , 1982.

Dear Mrs. Fryzek:
The Board of Weld County would like to gratefully
acknowledge your generous donation to the Weld County
Ambulance Service. As you can see from the enclosed
resolution which formally accepts your donation, the
money will be used for the purchase of a new mattress
to be used in the employees ' sleeping quarters.

enjoy the mattress purchase made possible by your
donation.
