RE: APPROVE CONTRACT CHANGE ORDER LETTER#1 FOR STATE PER CAPITA
CONTRACT AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Change Order Letter#1 for the State
Per Capita Contract between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Weld County Department of Public
Health and Environment, and the Colorado Department of Public Health and Environment,
commencing July 1, 2001, and ending June 30, 2002, with further terms and conditions being
as stated in said change order letter, and
WHEREAS, after review, the Board deems it advisable to approve said change order
letter, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that Change Order Letter#1 for the State Per Capita Contract between
the County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Weld County Department of Public Health and Environment, and
the Colorado Department of Public Health and Environment be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said change order letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 6th day of May, A.D., 2002.

Enclosed for Board review and approval is change order letter #1 to the Per Capita Program
contract between Weld County and the Colorado Department of Public Health and Environment.
The Department of Public Health and Environment is directed to reduce its General Fund
spending during this fiscal year and the next fiscal year due to an unexpected shortfall of state
revenues. This requires a reduction in General Fund distributions to local health departments,
public health nursing services,and environmental health services.
For this fund reduction, Weld County Department of Public Health and Environment will receive
a decrease of$4,052.11 for an amended total of$249,204.89 for the time period July 1, 2001
through June 30,2002. I recommend your approval of this change order letter.

Contract Change Order Letter Number I, Contract Routing Number 02 FAA 00937
State Fiscal Year 2001 -2002. Per Capita Program
This Contract Change Order Letter is issued pursuant to paragraph 9 of the Contract with contract routing number 98 FAA 07021 and
contract encumbrance number PO FAA OLL9807021. This contract change order letter is between the DEPARTMENT OF PUBLIC
HEALTH AND ENVIRONMENT and the BOARD OF COUNTY COMMISSIONERS OF WELD COUNTY,for the use and benefit
of the Weld County Department of Public Health and Environment. The Contract has been amended by Contract Renewal Letter 1,
contract routing number 99 FAA 00578,Contract Renewal Letter 2,contract routing number 00 FAA 00730,Contract Renewal Letter 3,
contract routing number 01 FAA 00748,and Contract Change Order Letter 1,contract routing number 02 FAA 00199. The Contract,as
amended,if applicable,is referred to as the"Original Contract." A copy of the Original Contract is incorporated herein by this reference,
made a part hereof,and attached hereto unless a Letter of Compliance has been submitted with this Contract Change Order Letter. This
Contract Change Order Letter is for the current term of July 1,2001 through June 30.2002. The maximum amount payable by the State
for the work to be performed by the Contractor during this current term is decreased by$4,052.11 for an amended total financial obligation
of the State of$249.204.89. The revised Scope of Work and the revised Budget are incorporated herein by this reference,made a part
hereo`," a" ' The first sentence in paragraph 7 of the Original Contract is modified
accordingly. All other terms and conditions of the Original Contract are reaffirmed. The proposed effective date of this Contract Change
Order Letter is May 13,2002. However,in accordance with section 24-30-202,C.R.S.,as amended,in no event shall this Contract Change
Order Letter be deemed valid until it shall have been approved by the State Controller or such assistant as he may designate.
Please sign,date,and return all 4 originals of this Contract Change Order Letter by April 30th,2002,to the attention of: Stephen
Holloway,Office of Local Liaison,A-3,Colorado Department of Public Health and Environment,4300 Cherry Creek Drive South,
Denver,Colorado 80246-1530. One original of this Contract Change Order Letter will be returned to you when fully approved.
