HomeMy WebLinkAbout20022396.tiff RESOLUTION RE: APPROVE TASK ORDER FOR MATERNAL AND CHILD HEALTH SERVICES AND AUTHORIZE CHAIR TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with a Task Order for Maternal and Child Health Services between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Department of Public Health and Environment, and the Colorado Department of Public Health and Environment, commencing October 1, 2002, and ending September 30, 2003, with further terms and conditions being as stated in said task order, and WHEREAS, after review, the Board deems it advisable to approve said task order, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Task Order for Maternal and Child Health Services between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Department of Public Health and Environment, and the Colorado Department of Public Health and Environment be, and hereby is, approved. Wallace, MD, MPH-Director aoo;t-,2 R b ATTACHMENT 1 Weld County Health Department MCH Plan October 1, 2002 -September 30, 2003 WELD COUNTY DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT PERINATAL PLAN FOR FEDERAL FISCAL YEAR 2003 INTRODUCTION Significant changes in community problem solving have emerged due to frustration with the same problems coming up again. The "Kids Count"report shows the same low percentage of women receiving prenatal care in the first trimester of pregnancy and high fertility rate for 15 to 17 year olds. Weld County acknowledges that this is a community problem and not just a Health Department problem. Agencies and clinics are revaluating what they have been doing and are talking about changing the status quo. What it comes down to is when do you stop doing the same things over and over and expecting different results?Many agencies and community members have been working hard on these issues and are tired of not seeing improvement in the indicators. However,this frustration and a hard look at the system in this County stimulated the community to take new actions. Agencies that typically have not worked together are starting to join forces to solve these problems. The status report shows how these community entities formed and what they are working on. It is encouraging to see barriers begin to break down and collaboration happen. The Promises for Children and WECAN teen pregnancy prevention project show a new unity in the community. The priority needs have not changed from the previous plan. As indicated in the report and introduction, the community and WCDPHE are committed to seeing these issues through. The objectives and activities for the 2002 plan have not yet reached the time lines indicated. In addition, these issues cannot be turned around quickly. The Strategic Plan for WCDPHE is in alignment with these priority issues as well. Our Strategic Plan was developed by all staff and is supported by our Board of County Commissioners. The capacity of the Sunrise Community Health Center to serve uninsured low-income women is still presenting pressure on the medical system and calls for fortification of the prenatal care services to that population. In summary, there is support to continue these efforts from staff, community agencies, and families to continue working on the priorities identified. Priority Need: Increase the number of women in Weld County who receive prenatal care in their first trimester of pregnancy. (We see our role in assisting the county to reach this goal as well.) The reason that 78% of the MCH funds are going to prenatal care is that we contract that amount to Cottonwood Pointe Clinic. This delivery of prenatal care is for 100 to 200 women per year. As you can guess this does not cover all the expenses for that care, but this is the most cost effective approach that has been found so far. Sunrise Community Health Center serves around 800 pregnant women per year and struggles with a small facility to meet that need. There are no other providers in the Greeley region that will serve low income, undocumented women. The undocumented women are not able to qualify for the public health funded coverage for all their prenatal needs. There are over 2100 babies delivered at North Colorado Medical Center every year. About half of the deliveries are to Sunrise and Cottonwood Pointe women combined. Our support of prenatal care services helps provide access to prenatal care for undocumented women. Without this support there would be even more women unable to obtain inadequate prenatal care. Program objective: WCDPHE will Implement two strategies from best practices to increase early entry into prenatal care to 70%by December 31, 2003. Activities: 1. One best practice is the maintenance of Presumptive Eligibility(PE) Sites to assist women with enrollment into Medicaid. We will provide onsite PE during department business hours and direct prenatal referral services to 450 women by September 30, 2003. 2. Contract at least $89,562.00 of the MCH funds to Cottonwood Pointe for Prenatal Care to serve 200 women by September 31, 2003. Program objective: Obtain the results from the Prams over sample in Weld County by October 1, 2002. Activities: 1. One new strategy or system change will be identified to decrease prenatal care barriers by September 31, 2003. 2. A report with the over sample results and recommendations will be presented to each member of the NCA by September 31, 2003. Types of Activities: Enabling and Infrastructure Building Monitoring: 1. Number of women who receive PE/referral services 2. Number of women who receive prenatal care at Cottonwood Pointe 3. Data results from the Prams over sample will be reported to the NCA. Evaluation: 1. Women who receive PE/referral services will be at least 450. 2. Women who receive prenatal care will be at least 200. 3. One strategy or system change will be implemented by the NCA Priority Need: The teen fertility rate of 15-17 year olds in Weld County will be reduced to 41.9 births per 1,000 females by December 31, 2005. Program objective: By September 30, 2003, implement the Abstinence Education Clinical Intervention in at least one nursing clinic for adolescent clients at the WCDPHE. Activities: 1. Plan and coordinate pilot project in one nursing clinic by December 31, 2002. 2. Train two nurses in the Abstinence Education Clinical Intervention by March 31,2003. These nurses will provide the Medical Model for Sexual Cessation with teenagers that visit the Health Department for STD/contraceptive care. 2. One clinic nurse will be trained by December 31, 2002 in the male sexual responsibility curriculum called "Wise Guys". Clinic staff will receive information/training from this nurse to include this perspective to teenage male clients who seek services for STDs. 3. Two parent education programs will be conducted regarding"Walking to Your Child/Adolescent About Sex"by September 30, 2003. Monitoring: 1. There will be 100 clients participating in the Abstinence education medical Intervention at WCDPHE clinics. This will be tracked through a log kept by the participating staff. 2. There will be 75 adolescent males who utilize STD clinic services receiving education on male sexual responsibility. This will be tracked through a log kept by participating staff. 3. The will be 100 parent education programs on adolescent sexuality communication. This will be monitored through registration lists. Evaluation: 1. Collect quantitative and qualitative feedback from adolescent clients on Short-term and intermediate outcome achievement(Measurement tools to be developed). 2. Compare the teen fertility rate for 15 to 17 year olds in Weld County from 1999 to December 31, 2005. This will be obtained from CDPHE MCH Data Sets. 3. Chlamydia rates for 15 to 17 year olds in Weld County will be compared from 1999 to December 31, 2005. Child Health Local Plan FFY 2003 The Department of transportation reports 59 million children under the age of 15 in the year 2000. Twenty-one percent of this population was in the 0— 14 age group. During 2000, 8,145 passenger vehicle occupants under 15 years old were involved in fatal crashes. 56% of the children who were fatally injured were unrestrained and were of the following age groups: 27 %were ages 0—4, 36% were ages 5 —9,and 44 %were ages 10— 14. Research on the effectiveness of child safety seats has found them to reduce fatal injury by 71 % for infants less than 1 year old, and by 54 % for toddlers 1 — 4 years old in passenger cars.' In 2000 there were 529 passenger vehicle occupant fatalities among children under 5 years of age. An estimated 251 of these fatalities, or 47%, were totally unrestrained. From 1975 to 2000, an estimated 4,816 lives were saved by the use of child restraints (child safety seats or adult belts). In 2000, an estimated 316 children were saved as a result of child restraint use. Unfortunately Weld County Drive Smart Coalition reports 90%of child safety seats are installed incorrectly Failure to read the child safety seat instructions, in addition to vehicle owner manual instructions regarding safety belts, could result in serious injury or death. This is a result of a failure of the child safety seat to be securely and/or properly restrained2 In 1998- 2000 State of Colorado MCH data measured the rate of deaths per 100,000 children ages 0—14 caused by motor vehicle crashes. Weld County measured 7.5 compared to Colorado's measure of 4.4.3 Locally, in Greeley, there are 2 fife stations who have car seat safety technicians on staff available to the public to inspect child safety seats. Citizens can make an appointment at the fire stations, but the appointment may be cancelled due to the fire station's work load. Drive Smart of Weld County Coalition reports 2000 births in Weld County last year, with only 280 car seats checked by the coalition due to limited resources. Weld County and the state of Colorado are limited in the quantity of certified car seat safety technicians and inspectors. Currently in our division of Public Health Education and Nursing we have two certified technicians and eight inspectors. The division also coordinates the monthly Drive Smart of Weld County Coalition meetings. Car seat safety is included in the overall WCDPHE strategic plan under injury prevention for the citizens of Weld County. In order to reduce pediatric death and injuries related to car crashes, we will work to educate parents about proper car seat usage, safety and installation. Addressing this need will be one of our FFY 2003 MCH child health objectives. The child health team is represented on the planning committee and participates in the annual Children's Festival of Weld County. The popular"Fishing for Health Booth"attracted 700 children this year . Weld County is fortunate to have many coalitions and partnerships serving children in the community. Medical providers report high volume of appointments being utilized for acute illnesses limiting well child examinations. The loss of educational opportunities at a child's physical exam due to time constraints is truly a misfortune for parents. Anticipatory guidance, injury prevention, and developmental guidelines all are important pieces quickly fading from the regular childhood check-up. Medicaid recognizes the importance of parental education by increasing the reimbursement when anticipatory guidance and. safety precautions are included (EPSDT Physical Screening form). As public health nurses we are concerned about the loss of educational opportunities for parents since we no longer offer well child clinics. WCDPHE families accessing immunization services are a prime target audience to provide educational opportunities. Immunization clinic appointments and walk-in services are offered four times a week , serving up to 400 clients a month.. We plan on developing written materials for parents with messages of injury prevention, developmental stages, and age appropriate guidance . A child health nurse will utililize the materials to provide one on one education to parents in the immunization clinic. Our initial target group will be parents with children ages 0— 5 years old. This particular age group has been selected in order to tie our two objectives ,injury prevention and car seat education, together for our MCH FFY 2003 child health plan. Plan—Objectives,Activities, Monitoring, and Evaluating Child Health program Need: Improve parents' knowledge of proper car seat usage and installation for the reduction of pediatric deaths and injuries related to car crashes in Weld County. Objective: By September 30th, 2003 WCDPHE child health nurses trained in car seat safety will participate in six Weld County car seat safety check points. Build infrastructure with Weld County ambulance to expand check point access. Activities: 1. Continuing education for car seat safety technicians to meet required certification by September 30, 2003. 2. Car seat safety inspectors will receive one training update by safety technician by May 315f, 2003. 3. One WCDPHE car seat safety tecnician will conduct the Weld County Drive Smart monthly coalition meetings. 4. Schedule of car seat safety inspection check points and car seat safety information will be available to clients accessing services at WCDPHE. 5. Meet with Weld County paramedics to plan check point to be located at their facility by September 30, 2003. Type of Activity: Population Monitoring: 1. Number of car seat safety check points and seats checked. 2. Number of staff attending car seat training. 3. Minutes from Drive Smart coalition. 4. This renewal letter will provide continuation funding for the Prenatal and Child Health programs at WCDPHE. The funding will be used to provide prenatal and postpartum care, including education and counseling,to increase early entry into prenatal care, and to assist women with enrollment in Medicaid by maintaining the Health Department's status as a Presumptive Eligibility Site. Funding for the Child Health Program will be used to provide continuing education for car seat safety, car seat safety inspection check points. written materials for parents with messages of injury prevention, developmental stages, and age appropriate guidance, and one on one education to parents in immunization clinics regarding these concerns for children.
