RE: APPROVE FOUR NOTIFICATION OF FINANCIAL ASSISTANCE AWARDS FOR
VARIOUS PROGRAMS AND AUTHORIZE CHAIR TO SIGN - INNERCHOICE
COUNSELING CENTER
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with four Notification of Financial Assistance
Awards between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Social Services, and
Innerchoice Counseling Center, commencing June 1, 2002, and ending May 31, 2003, with
further terms and conditions being as stated in said awards for the following programs:
1) Lifeskills
2) Intensive Family Therapy
3) Option B
4) Alternative Programs to RTC Placements under FPP Option B Home
Based Services, and
WHEREAS, after review, the Board deems it advisable to approve said awards, copies
of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, ex-officio Board of Social Services, that the Notification of Financial
Assistance Awards for the above listed programs between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Department of Social Services, and Innerchoice Counseling Center, be, and hereby are,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said awards.

Enclosed for Board approval are the PY 2002-2003 Notifications of Financial Assistance
Awards (NOFAA) for Families, Youth, and Children Commission (FYC) Core Services
Funds, which are for the period of June 1, 2002, through May 31, 2003.
The Families, Youth and Children Commission (FYC) reviewed proposals under a
Request for Proposal process and are recommending approval of these bids.
Innerchoice Counseling Center
A. Lifeskills
This program is a Lifeskills mentoring program for youth and adults. Targeted
areas include (1) Lifeskills development, (2) substance abuse issues, (3) anger
management, and (4) mentoring. A total of 24 clients can be served at any one
time, average length of program is two to four months, and the average number of
clients to be served is ten. Approximately one-third of the families is estimated to
be South Weld County, and over half will have availability to Bilingual-Bicultural
services. Rate is $45/hour for 8 hours of service per week and$40/hour for 4
hours of service per week.
B. Intensive Family Therapy
Services are provided to clients utilizing a multi-systemic strength based model.
The implementation of these services may include parent-child interventions,
marital interventions, non-nuclear family interventions, school-based
interventions,peer interventions, substance abuse interventions, and linkages

families will receive Bilingual-bicultural services, and one-third of the families
served will be in South Weld County. The average length of stay is eight weeks.
Rate is $75/hr.
C. Option B
The program provides a maximum of 14 service hours per week for families. The
program is tiered so the treatment plan can be individualized to meet the specific
needs of the family. Crisis intervention is available 24 hours per day, 7 days per
week. Services include in-home family therapy, team leadership, behavior
management,family aids,peer and adult mentoring, couples therapy and co--
therapy, group therapy, drug-alcohol therapy, splash events, and restorative
justice events. Services are billed on the level system. Level "A"service, $2,300
(6-week average stay); Level C Service, $900,(three-month average stay). Hourly
rate per direct service is $48/hr.
D. Option B, Alternatives to Residential Treatment Center Placements
This program provides highly intensive in-home family therapy treatment and
intensive case management services to at-risk youth and their families. Services
are provided to families within their natural environments via homes, schools,
and neighborhood settings. Services are provided through a four-tiered step-down
program. Staff is available 24 hours a day, 7 days a week, and can generally meet
at the family's convenience. This program provides services to males and females,
ages 0-18, as well as treatment to younger siblings,for approximately three to
five months'duration. Services are billed on the level system. Hourly rate is
$48/hr.

The Families,Youth and Children Commission, an advisory commission to the Weld County Department of
Social Services, announces that applications will be accepted for approved vendors pursuant to the Board of
Weld County Commissioners' authority under the Statewide Family Preservation Program (C.R..S. 26-5.5-
101) and Emergency Assistance for Families with Children at Imminent Risk of Out-of-Home Placement
(C.R.S. 26-5.3-101). The Families, Youth and Children Commission wishes to approve services targeted to
run from June 1,2002, through May 31, 2003, at specific rates for different types of service, the county will
authorize approved vendors and rates for services only. The Life Skills Program must provide services that
focus on teaching life skills which are designed to improve household management competency,parental
competency, family conflict management and effectively accessing community resources. This program

I. Program Description-Life Skills/Mentoring Program for Weld County Youth andAdults
InnerChoice Life Skills Team has the availability to go into the client's home or meet individually
within an office setting to assess how we can best serve the client's needs and help them to become
more self-sufficient and healthier community participants. Once a thorough assessment has been
completed, a life skills counselor will be assigned under the direct supervision of a licensed clinical
social worker to help the identified client to create their own individualized treatment plan to
address these main targeted areas:
1)Life Skills Development
2)Substance Abuse Issues
3)Anger Management
4)Mentoring
Once a complete assessment has been made, a life skills counselor will be assigned who will be
under the direct supervision of a licensed clinical social worker. The life skills counselor will help
the client to create their own individualized treatment plan to address any or all of the above listed
areas.All treatment plans will be evaluated two limes per month with the treatment team( life-skills
counselor, case managers, substance abuse counselors and clinical supervisors).
Case notes and progress reports will be faxed weekly to case managers at Weld County Social
Services for quality assurance. Clients will work with a life skills counselor at least one hour
individually per week. Some of the life-skills services that can be accessed include:
• Job hunting skills -How to interview, application writing,establishing and identifying good
work habits, and identifying the importance of transportation assistance and planning.
• Housing - Assistance finding housing, what choices do we have, what questions to ask,
leases, role of the landlord,how to get utilities hooked up, deposits, and how to be a good
neighbor.
• Health - Education about sexually transmitted diseases, substance abuse groups, nutrition
issues, how to find a doctor and/or dentist, hygiene issues, pregnancy testing and
contraceptive usage.
• Transportation-How to ride the bus,how to buy a vehicle,drivers license testing assistance,
insurance issue and how to maintain the upkeep of a vehicle.
• Housekeeping - How to get your kids to do chores, how to run a household, pay bills and
budget allocations for expenditures.
• Leisure Activities-How to stay out of trouble,the laws and statutes,exploration of healthy
interests and hobbies, staying fit, staying sober, how to make pro-social peer contacts and
adult contacts, community support groups, and healthy community resource development.
• Education- Career planning,resource identification, GED Testing,career inventories, and
college tours.
• Empowerment- How to be assertive, family relationships, sexuality, spiritual issues, pro-
social support systems,problem solving, and social responsibility ethics discussions.
Life skills counselors are available to help youth find alternative activities within the community that
will lower their risk of becoming involved in gang-related activities, substance abuse or any other
unhealthy and/or illegal activity. The counselors are of all ages and are chosen for their ability to be
a positive role model and join with resistant clients.
The life skills counselors teach and empower the client to be more confident in his or her own
capabilities,and to participate and feel like they are a contributing member within their community.
One of the main missions of InnerChoice Life Skills Program is to help the client know how to
continue pro-social support for their goals once they have completed the life skills program.
A wide umbrella of groups has been created to meet the client's identified needs that sometimes can
best be met through a pro-social support group.InnerChoice staff are constantly increasing their level
of service options in this arena to include:

• Attitude Awareness Classes
• Substance Abuse Groups
• Rotating Life Skills Curriculum

17. Target/Eligibility Population
• Criteria for accepting a client: InnerChoice's staff is committed to providing services quickly
to clients that are appropriate for this service. The first criteria is that the client is "a child"
who is at risk for out-of-home placement or a "parent of a child"who is at risk of out of
home placement. The second criteria is that the client is committed to working on their
issues and are evaluated as "customers for change".
• The life-skills counselor will not carry a caseload greater than four individual clients. Six
life skills counselors will be available to provide a wide range of assistance to the clients.
Being served in this program..A total of 24 clients can be served at any given time. The age
range is 12 years old and up. The average number of clients to be served is ten.
Approximately one-third of those families are estimated to be in south Weld County and over
half will have availability to bilingual/bi-cultural services.
• The gender,ethnic,and diagnostic mental health needs of the client will have "top priority"
in matching the client with the most appropriate staff. For example, if a client would be
more open to a male counselor then every effort would be made to accommodate that
request. If a Hispanic client is referred, a Hispanic counselor will be accessed. Special
hiring measures have been taken so that Hispanic and bilingual staff are available to serve
Hispanic clients.
• Client strengths will also be prioritized in selecting counselors. InnerChoice prides itself in
having a large staff with diversified training, expertise, and experience in working with a
wide array of clients. For example, if a client is interested in being artistically creative,
they will be provided an art counselor or if they enjoy the outdoors and sports activities, an
experiential counselor will be utilized; if a client enjoys spending lime with animals,then an
equine counselor will be utilized.
• Average length of stay in the program is two to four months. InnerChoice staff work as
quickly and efficiently as possible to help the client increase their life skills and community
resource networking.The goal is to immediately start connecting the client with their natural
support systems within the community so that the length of stay in treatment cane be
shortened.
• Program availability is 24 hour per day on call crisis coverage for all clients. A licensed
clinical social worker or mental health professional is available for case management and
crisis intervention 24 hours per day, 7 days per week.
• The average length of stay in the program is 2- 4 months.
HI. Type of Services Provided
A. InnerChoice Life Skills Counselor
When a client is referred,the service director will meet with the referring case manager to determine
the needs of the client. The director will also meet with the client to assess which staff counselor
will best fit the needs of the client. The service director then meets with the counselor to design a
program that best fills the desired goals of the client and referring case manager. The counselor will
work directly with the client in the community, to coach, and direct the client in whatever skill is
needed. Bilingual staff are available to work with Spanish speaking clients. The client can be
placed in a life skills group depending on their needs and the referring agent's request. Life skills are
demonstrated to the client. Then the client is encouraged to tackle the task with the counselor
coaching. In the life skills group, the clients take a test in each major area listed. In class, they will
work on assignments that will help them directly in each specific area. After the class, during the
following week,the assigned counselor will further assist the client in successfully completing each
life skill area taught in class,main emphasis will be placed in each clients' area of specific need.
B. Substance Abuse Counseling and Education
InnerChoice substance abuse counselors are case workers with a CAC II or III,who are providing
drug and alcohol assessments, one-on-one counseling and group therapy to youth and adults with
substance abuse problems. The services will be provided within an office setting. The program is
designed to assess and target the needs of the individual. InnerChoice believes in focusing on
individual strengths and family strengths to help clients overcome their addictions.Assessments will
consist of self-disclosure reports, Substance Abuse Subtle Screening Inventories,Alcohol and Drug
Differential Assessments and the Beck Depression Inventory in an effort to determine risk factors.
C. Anger Management Counseling and Education
InnerChoice life skills counselors are trained to provide individual and group psycho-educational
services for anger management clients. Role-playing,joumaling, and experiential activities which
address anger management issues will be covered during these sessions. Family of origin issues and
corrective thinking errors will also be addressed.
D. On-call services
On-call services are available to life skills clients 24 hours a day, seven days per week by the on-call
clinician. Clients are encouraged to use this service for crisis mediation.
Group Therapy -Life Skills Education/Drug alcohol/Anger Management Group is an eight week
group designed to be started at anytime. It incorporates substance abuse education with life skills,
and anger management. This unique group is designed to be educational, fun and beneficial.

Week 1-We discuss leisure activities that are not connected to substance use. We will discuss why
clients have used drugs and/or alcohol, what kind of trouble they have been in as a result of the
substance abuse, and what their other options might be. (Set goals for the week)
Week 2-We will discuss personal care. How does substance use and abuse effect your health. We
will begin to discuss the effects of drugs and alcohol, and play a game that teaches drug facts.
(Discuss goals for the week)
Week 3- We talk about money issues and how much money we have spent on drugs and alcohol.
What kind of legal trouble has your substance abuse created,and how much did it cost you?We will
plan our budgets and talk about job hunting.(Set goals for the week)
Week 4-We will discuss drugs,tattoos,body piercings, and sex, and how and why these are high-
risk activities. We will offer HIV testing in group, these are the mouth swab tests. We will discuss
what happens if you are positive. (We will discuss safe sex goal)
Week 5-How do you handle an emergency? We will discuss how to protect yourself on the street,
street drugs such as Ecstacy, Meth, Heroine, LSD, and how you can die or become ill from these
drugs. We will watch a video on Ecstacy.
Week 6-We will talk about finding your own home.How to lease or rent your own place,what you
need to know about roommates and how not to set yourself up with high risk roommates. Set up
housing goals.
Week 7 - What's wrong with gangs? Video and open discussion on gangs, how to find healthy
solutions. Long-term and short-term life goals.
Week S-. We will do a life-plan worksheet wich covers friends, family, work, education, material
goals,and spiritual goals. Discuss how to stay committed to your goals and get pro-social support
to help you achieve them. Create a life contract with yourself.
Mentoring Program - When a client is referred to the life skills program the counselor will talk
with the client and the client's parents to assess which mentor will best suit the client. The next step
is for the mentor and the client to meet. This meeting is facilitated by the counselor with the client's
parents in attendance. At this time, the family gets an overview of what the program is about and
what the goals and expectations of the client are. They are asked to sign a release of information so
communication can be exchanged with the caseworker.A Right to Transport form is then signed so
that the mentor can transport the client. We ask for a commitment from the client to keep their
appointments with their mentor. Should the client need to cancel,we ask that it is done in a timely
manner.
The mentoring program specifically provides a young adult role model to spend one-on-one time
with an at-risk youth,of any age.Mentors are not counselors and are there to support and listen. Any
specific goals set for the client are supported and facilitated by the mentor. A mentor can help with
specific life skills,tutoring,help with family chores and training. We have mentors who specialize
in grief and loss who can help facilitate the grieving process in an informal setting, as a friend. We
also have mentors who specialize in substance abuse issues who can work with the youth to help
them find other alternatives to drugs. Other common areas our mentors can help with are abuse in
the family, gang involvement, and anger management.
Levels of care:
Level I Service
Four hours of service per week consisting of any combination of mentoring, life skills education,
substance abuse counseling and any psycho-educational or therapy group - $40.00 per hour.
Level II Service
Eight hours of service per week consisting of any combination of mentoring, life skills education,
substance abuse counseling and any psycho-educational or therapy group - $45.00 per hour
Quantitative measures will be approximately 80 percent therapy services(direct care)to include the
administration of psycho-educational assistance in the development of life-skills such as stress
reduction,problem solving,budgeting,hands-on parenting,support groups,mentoring, Ten percent
will be case management and the final percent will be an additional 10 percent directed towards
collateral contact training and referral based services. Clinicians will make sure that services are not
duplicated by contacting and collaborating with other service providers on a weekly basis. Capitated
services will be utilized whenever possible to lower the costs to the county and when formally
approved by the county case worker.
II.Measurable Outcomes-At the beginning of services,a written assessment of the client's needs
will be created and an eight week follow-up plan. Every two weeks, the counselor will staff the
progress of the client with the team members. Strengths will be assessed and continually used by
the counselor to empower the client to continue with on with his or her goals. If a client is not
progressing or responding,other methods will be approached or other counselors will be brought in
to help. If a client is completely non-compliant, the referring agent will be notified as soon as
possible to discuss other approaches or termination of services.
As long as a client continues to work with his or her counselor and attend groups and one-on-ones,
and meets goals and passes the required assignments they will continue in the program. The life
skills counselor will be in direct contact with the Weld county case manager on a weekly basis and
case notes will be faxed bi-monthly to report the progress of the client. Life skills progress will be
measured by continued assignments and testing where can be applied. Pre-treatment scores and
termination scores will be monitored for treatment outcomes.

• 90% of the clients that complete the life skills program will have a working knowledge of
the skills they were taught. Clients will have received direct coaching on all skills that have
been designated as part of their life skills goals. For example, if a client was trained on
parenting skills,they,after training,will be observed while demonstrating what they have
learned. Until they adequately demonstrate the skill,the life skills counselor will continue
to work with them in that area
• 90%of adolescents referred will complete the mentoring program. Clients mentoring will
receive hands-on experience with many of the leisure activities available in their community.
They will have the opportunity to meet new peers and will explore new interests in a effort
to get them interested in healthy activities. All the clients who complete the program will
learn new options to replace their old behavior.
• 85% of the clients who complete the substance abuse program will receive a measurable
education about most of the drugs they have used.They will receive the life skills needed to
make better choices about their substance use and lifestyle choices. All clients in group and
with the help of their counselor will complete long-term and short-term goals.The will work
out an in-depth plan that will be written out step-by-step to help them achieve their goals.
• Clients involved an any aspect of the life skills and mentoring will improve their life skills
and knowledge through varied and creative therapeutic styles. Clients will reduce their
negative resistance through the mentors' and counselors' abilities to uncovering their unique
needs and strengths by joining with their clients.
V. Service Objectives
A. 90% of the clients served in this program will develop their life-skills and will be able to
incorporate them and replace habits of self-medicating and self-defeating behaviors. With
the resources we have available,InnerChoice,along with the suggestions of the case manager
will implement a plan that best suits each client with a variety of services available such as
assessments,substance abuse counseling,mentoring,life-skills education and support groups
for substance abuse. A treatment plan can be made to accommodate the needs of the
population being served.
13. Life skills counselors,substance abuse counselors and group counselors will work together
throughout the program to adapt a support plan for each client as they progress in treatment.
Through individual counseling and group therapy,the goal is to support the client as they
work through issues that keep them stuck in the cycle of substance abuse. Additional
support will be added as needed with life-skills counselors and mentors. Stress reduction,
problem-solving education, self-care and leisure time will continue to be assessed by the
individual counselor, so that stress factors can successfully be reduced in the client's life.
C. Ninety-five percent of the clients will develop alliances through natural, familial, and
community supports such as extended family,AA Services,Al-A-teen Groups,NA Groups,
Cornerstone, The Boys and Girls Club,their local churches,health care professionals, and
neighborhood groups and a variety of community organizations.
D. A follow-up program can be implemented for clients who need additional support after they
complete the initial eight week program offered though InnerChoice. Ongoing support
groups and counseling will be available to each client on a"sliding fee scale" in an effort to
provide low cost strength based services.
VI. Workload Standards
A. The typical staffing pattern for individuals inthis program would be a treatment team
consisting of the referring case manager, a life skills counselor, a CAC II or CAC III, a
mentor and a clinical supervisor.
B.
Individual appointments and groups will be scheduled in the afternoons, evenings, or
weekends as needed to fit into each client's schedule. Clients will receive a minimum of four
hours of service per week. Twenty-four hour on-call crisis management is available by a
licensed clinician.
C. Life skills counselors will serve no more than four individual clients per week. Group
counselors will facilitate no more than three groups per week.
Twenty-four staff members are employed through InnerChoice Counseling with a wide array
of skills and styles to help facilitate the success of each client.
All counselors,mentors and group leaders are clinically supervised by an LCSW and a CAC
III. Supervision occurs once a week and are mandatory for each staff member.
D. An average of eight substance abuse clients can be served per week in one eight week group.
A maximum of three groups will be running at any given time. Five CAC counselors are
available to see up to five clients per week individually.
E. Motivational interviewing and multi-systemic therapy will be utilized as the treatment
modality by certified substance abuse therapists.
F. Certified addiction counselors will be available for appointments Monday through Saturday
8 a.m. through 8 p.m. for individual counseling sessions and groups. On-call support for
crisis situations is available 24-hours a day and seven days per week.
G. Six CAC counselors are available to provide these services. Ten mentors are available and
six life-skills counselors.
H. The clinical supervisor will supervise no more than four CAC counselors. An LCSW will
supervise the mentors and the life skills counselors.
I. InnerChoice counselors are required to have their own personal professional liability policy
and the agency is covered through American Professional Agency in New York,New York.
VII. Staff Qualifications
Denise E. Klein-LCSW
Denise serves as the Program Director for InnerChoice Counseling Center. She will coordinate
inter-agency services and will manage the Inner Choice teams as they provide services to each
family. Denise earned a Bachelor's Degree in Social Work from Minot State University and a
Master's Degree in Social Work from the University of North Dakota. She was trained in multi-
systemic therapy at the graduate level and began her first paid position as an in-home therapist. She
has 12 years of experience in working with youth and families. She has developed and implemented
two separate Intensive In-home Family Preservation Programs in North Dakota and Minnesota.
Denise also worked at the Grand Forks Air Force Base with families with multi-cultural backgrounds
providing service to youth and their families reported for child and spouse abuse. Denise has also
served as a guardian ad litem,court appointed visitor,supervised visitation monitor and foster parent.
Linda Griego-MS, CAC III
Linda will serve as the Clinical Director of the InnerChoice Substance Abuse Program overseeing
the direct care initiatives and requirements of each client. She has 12 years of substance abuse
counseling experience working at the Hope Center, Beaucaire Youth Services, Drug Court in
Larimer County,and the State Probation Department in New Mexico.She specializes in serving high
risk oppositional youth facilitating individual and group sessions.
Rose Moon-BA,CAC II
Rose will serve as the Life-Skills and Mentoring Program Manager assisting in referral
distribution,case management, assessments individual and group counseling. She will work with
case managers to determine the needs of each client. She will then assign counselors to the client
and monitor all services. Rose will communicate weekly with each agency representative to assist
in the success of each client.Rose has worked with youth and adults for the past ten years. She has
worked in personnel management,risk management, and has managed mentoring programs for the
past three years. She has a degree in art and has worked as a professional artist for the last 12
years. Rose has been able to use this experience in creating strategies for clients with artistic and
creative interests. She has developed InnerChoice's Life Skills Program and facilitates the art
therapy groups for all ages. Rose is presently working on completing her CAC III and will
continue to work directly with clients with addictions and will run the substance abuse,life skills and
mentoring program for InnerChoice Counseling.
CAC I,II,III(5)-this position requires the state ADAD certifications,or the CAC I must be in the
process of completing the class work for the certification. All substance abuse counselors will be
supervised by a CAC III.
Life-Skills Counselors (6)- must have a high school diploma and a Bachelors degree from any
accredited college. Our counselors have a variety sr skills and interest to add diversity to our life-
skills program.
Mentors(10)-Must have a minimum of a high school diploma and have the qualities to be a good
role model to be involved in the program.
All Employees-Must have a clear Central Registry Background Check,be drug free and not abuse
alcohol. All employees must consent to random drug testing if we feel it is necessary. Any employee
who has abused drugs or alcohol in the past, have to have five years of sobriety. At the present time
we have 24 staff available for this program. We have a waiting list of ten to fifteen people who wish
to work for us at any given time.All staff must have a clean driving record and carry current insurance
and a license. All of our mentors and life skills counselors must complete a training course before
they can begin. Training consists of driving safety, high risk situations, safety in the home and
community, and first aid training.

Innerchoice is designed to meet a wide continuum of level of care based on the minimum and
maximum number of clients to be served. We have many part-time staff who are flexible with their

a decrease in referrals in on one or more other programs. Overheads costs are kept low to ensure the

The Placement Alternatives Commission, an advisory commission to the Weld County Department of Social
Services, announces that competing applications will be accepted for approved vendors pursuant to the Board
of Weld County Commissioners authority under the Statewide Family Preservation Program(C.R.S. 26-5.5-
101)and Emergency Assistance for Families with Children at Imminent Risk of Out-of-Home Placement
(C.R.S. 26-5.3-101). The Placement Alternatives Commission wishes to approve services targeted to run
from June 1,2002,through May 31, 2003,at specific rates for different types of service, the County will
authorize approved vendors and rates for services only. The Intensive Family Therapy Program must provide
for therapeutic intervention through one or more qualified family therapists, typically with all family
members,to improve family communication, function, and relationships. This program announcement

L Program Description
InnerChoice Counseling Center provides "Intensive Family Therapy" to children and
families. The services are provided to clients utilizing a multi-systemic strength based
model. InnerChoice therapists empower families by using identified strengths to develop
natural support systems and view the parents as the solution, as opposed to the problem.
The implementation of these services may include:
• Parent-Child Interventions
• Marital Interventions
• Non-nuclear Family Interventions
• School-based Interventions
• Peer Interventions
• Substance Abuse Interventions
• Linkages with Community Support
Mission Statement:
InnerChoice is committed to providing services professionally and expediently to families
who are dealing with family conflict and seeking a strength based and solution based model
to empower and strengthen the family unit.
• By providing a safe environment for families to explore issues that affect the stability
of the home and help family members to problem solve together so that the family
unit can remain intact.
• By providing an individualized family treatment plan which identifies the needs of
each family member so that services can be tailored to meet the "unique needs" of
the family.
• By teaching family members to come from a strengths perspective and empower them
to advocate for their themselves and their family members so that they learn to view
one another as natural supports.
• By empowering family members to view their unique strengths as a powerful tool to
work through family crisis and create a more cohesive family unit.
• By helping family members to feel hopeful about the future in regard to healthy
family functioning and movement away from dependency on the system towards self-
sufficiency.
II. Target/Eligibility Population
Families to be served
• Criteria for Accepting a Family: InnerChoice's staff is committed to providing services
quickly to families that are appropriate for services. The first criteria is that the family is
committed to staying in tact, are "customers for change", and participate actively in the
therapeutic process.
• The family therapist will not carry a caseload greater than six families within this program.
Six family therapists will be available, providing a wide range of unique intervention
specializations, so that the appropriate fit between therapist and family can be made. A total
of 36 families can be served at any given time. Youth can range in age from 0-18. The
average number of families served is eight.
• The gender, ethnic, and diagnostic mental health needs of the family will have TOP
PRIORITY in matching the family with the most appropriate staff. For example, if a family
would be more open to a male therapist, every effort would be made to accommodate that
request. If a Hispanic family is referred, a Hispanic family therapist will be accessed
whenever possible. Special hiring measures have been taken so that Hispanic and bilingual
staff are available to serve Hispanic families. If a family member is diagnosed with a
mental health issue that a particular therapist has more expertise in treating, then that
therapist will be matched with that particular individual client. At lease one-third of the
families served will receive bi-cultural/bilingual services. At lease one third of the families
served will be in South Weld County.
• Family strengths will also be prioritized in selecting therapists. InnerChoice prides itself in
having a large staff with diversified training, expertise, and experience in working with a
wide array of families. For example, if a family is interested in being artistically creative,
they will be provided an art therapist; or if they enjoy the outdoors and sports activities, an
experiential therapist will be utilized; if a family member enjoys spending time with animals,
then an equine therapist will be utilized. If necessary co-therapy can be conducted when
the need arises for this service.
• Average length of stay in this program is eight weeks.InnerChoice staff work as quickly and
efficiently as possible to help the family to create the change required to meet their treatment
goals..
• The average number of direct service hours per week is 2-4 per family. Innerchoice places
a great deal of emphasis on providing excellent communication with collateral contacts,
documentation, multi-disciplinary meetings and clinical reviews so that the flow of
communication remains consistent and current.
• Program availability is 24 hour per day on call crisis coverage for ALL FAMILIES. A
licensed clinical social worker or mental health professional is available for crisis
intervention 24 hours per day, 7 days per week.

III. Types of Services Provided
Program Positions and Services
CLINICAL MULTI-SYSTEMIC THERAPIST 2 to 4 hours per week
A licensed clinical social worker, psychologist or licensed marriage and family counselor
trained to assess, diagnose and treat families,will provide individual and family therapy to
family members referred to this program. However,the family therapist will also assess for
"natural supports" and start connecting family members with their family supports and
community supports by at least the second week of enrollment in the program. InnerChoice
will also assess for services that are already in place in an effort to make sure that services
are not duplicated and utilize other financial resources whenever possible to decrease the
cost to the county for this program such as Senate Bill 94 Funding, Victim's Assistance and
outside insurance providers.
The family therapist will place special emphasis on maintaining good communication with
the referring case manager and all other collateral contacts. They will schedule multi-
disciplinary meetings to review treatment goals. These meetings ensure quality management
and expedient service delivery. They provide a consistent time frame to measure and
document the effectiveness of treatment goals relative to client and family functioning. The
multi-disciplinary meetings also ensure that the treatment goals are on target and continue
to meet the family's changing needs and can be modified as the issues change. The family
therapist will also provide crisis management to families in acute crisis and will access
specialists when the diagnosis is outside of the provider's realm of training.
Co-therapy is also widely utilized with InnerChoice families. InnerChoice therapist's are
trained to work together as a team when working with challenging client's and families
multiple diagnosis and perhaps more than one child at risk out of home placement. The
multi-systemic model that InnerChoice therapists utilize is especially designed for children
who are at risk of being taken out of the home and have serious oppositional and conduct
disorder diagnosis.
InnerChoice's success rates with these difficult families is high because of the focus on the
families strength's,joining, solution focused treatment,team building, creative self-esteem
building and the over all multi-systemic approach. Seventy-five percent of families referred
will make progress in treatment and the child-well being scale will be utilized to measure this
success.
B. Community Collaboration Efforts

testing.

5. Any other agencies providing services to the family.
InnerChoice staff will get signed releases and coordinate the efforts of all other
service providers involved with the family when deemed necessary by the Weld
County and train client's access these services on their own.

D. Parental Caretaker Involvement
1. Parents will be encouraged to participate and be involved in the therapy. All activities
will be implemented in order to assess,treat, and define family issues so that when
the family has completed the program,they will know how to utilize family strengths
and problem solve from this perspective.
E. Assessment and Plan
1. After the initial referral is made to InnerChoice, the family therapist will initiate a
contact with the family and assess appropriateness of service. Once it is established
that the family can benefit from services, a contract for level of care will be
established. The level of care will be determined by the unique needs of the family
and number of outside service providers involved. Once the level of care has been
determined, the family therapist through InnerChoice will make contact with the
Weld County Case Manager to make recommendations for the formal authorization
for the level of care.
Levels of Care
Twenty Direct Service Hours - Level One
(Client can utilize anytime in six month time frame-$70.00 per hour) $1,400
Forty Direct Service Hours -Level Two
(Client can utilize anytime in one year time frame- $65 per hour) $2,600
Or provider can access individual hourly rate of$75 per hour for
office counseling if NO using the level system.
F. Pro-Active Planning
1. The reintegration plan will begin to be developed as early as the first weeks
enrollment in the program. The family therapist will be looking at natural support
systems via kin and the community so that the family does not become dependent on
InnerChoice staff.
2. Initially, InnerChoice will provide micro-management in regard to accessing and
utilizing these natural supports, however, once the family members have been
provided the necessary skills in accessing the support they will then be encouraged
to develop and strengthen these strategies with little outside intervention.
3. Continuity of care with other agencies providing services to the family will be
prioritized throughout the length of stay. InnerChoice staff will contact collateral
contacts on a weekly basis to ensure the quality of inter-agency communications and
coordination of treatment so that treatment goals can be targeted as a team.
4. Inner-agency staffmgs will occur frequently to review treatment goals and ensure that
the therapists are helping the families to work towards their goals in an efficient and
expedient fashion.
5. Diagnostic and medical evaluations will be given top priority so that medical and
mental health concerns can be addressed as quickly as possible. It is important that
any impairment from medical/mental health complications can be minimized and
treated so that clients can accelerate their progress towards their therapeutic goals.
6. After-care groups and splash events will be provided on a sliding fee scale for family
members so that they can continue to access support and maintain their treatment
goals through InnerChoice.
7. Clinicians will utilize the assessment tool to gather initial assessment scores, three
month assessment scores and discharge scores. A 12- month follow-up assessment
will be done to determine success and recidivism rates.
8. Program availability is 24 hour per day on call crisis coverage for all families. A
licensed clinical social worker or mental health professional carries a pager and is
available for case management and crisis intervention 24 hours per day, 7 days per
week.A critical incident report form will be filled out and sent to the county any time
a crisis contact is made with the on-call therapist. The on-call therapist will also be
trained to identify when detoxification services will be utilized and coordinated
through Island Grove. On-call therapists are available to make on-sight visits to
Island Grove Detoxification Center,North Range,the police station and Psych-Care
for crisis consult and case management.
9. Quantitative measures will be approximately 80 percent therapy services(direct care)
to include the administration of psycho-educational assistance in the development
of life-skills such as stress reduction, problem solving and hands-on parenting. Ten
percent will be crisis management and the final percent will be an additional 10
percent directed towards collateral contact training and referral based services.
Clinicians will make sure that services are not duplicated by contacting and
collaborating with other service providers on a weekly basis. Capitated services will
be utilized whenever possible to lower the costs to the county and when formally
approved by the county case worker.
IV. Measurable Outcomes
A. 80 % of children receiving IFT services will not go into placement.
B. 80% of families will successfully resolve their conflict through IFT Services.
C. 80% of the children reunified will remain within the home for at least one year
following the terminations of services.
D. 95%of the families will improve their parental competency ratings prior to closure
of treatment and 90%of cases that receive IFT services will register low on the risk
assessment devise at service closure.
E. The cost of this program is significantly lower than the cost of out of home placement
which is generally $4,000 per month verses $2,600 for IFT services through
InnerChoice.
Project Monitoring and Evaluation
Pre-tests and post-tests(Child Well-Being Scale and Family Risk Scale)will be administered to the
families immediately before and following the implementation of services. A six-month and twelve-
month follow-up survey will be done to assess for success and recidivism.
InnerChoice will monitor daily,weekly and monthly hard and soft services provided for the family.
This information will be provided to the county on a monthly basis for evaluation and program
management review.
The program will also fill out quarterly client progress reports as requested by the County.
A case is terminated as a treatment success when: The parent(s) have the motivation and skills
necessary for handling subsequent problems,the youth is involved with pro-social peers and actively
involved in a commnunity activity that is deemed healthy and constructive, and the family has been
able to function reasonably well with minimal support from InnerChoice for a period for at least four
weeks.
Treatment is prematurely terminated when: the clients are"not customers for change", the services
required are beyond the out-patient level of care and/or the safety of the child or community is not
assured.
Y. Service Objectives
A. 90%of the families will improve their communication skills and problem solving skills
through varied and creative therapeutic styles and interventions such as structural family
therapy,multi-systemic therapy,psychotherapy,psycho-drama,guided imagery,equine
therapy, art therapy, music therapy, mediation services for extended family, CAC
services for addiction issues and cognitive behavioral therapy for behavior management
issues. Gender appropriate and culturally appropriate therapeutic matches will be made
with families to increase success outcomes. Families will reduce their negative
resistance through the therapists' specialized training in uncovering the unique needs
and strengths of the families and their joining skills..
B. Group parenting classes will be conducted on an after care sliding fee scale for clients.
This will allow parents a cost effective way to work together to find solutions and vent
their frustration in regard to ongoing parenting concerns and issues.
C. 95% of the families will develop alliances through natural familial and community
supports such as extended family, church, health care professionals, neighborhood
groups and a variety of supportive organizations. Therapists will provide assistance in
establishing resources and accessing services through other state programs such as
Housing for Humanity, CHIP, LEAP and Social Security for families enrolled in this
program.
Vt Workload Standards
A. A typical staffing pattern for the provision of IFT services for one family is one
master's level therapist chosen based on the families unique needs and culturally
sensitive requirements.Appointments are available to families seven days per week
throughout the year so that optimal utilization and implementation of services can occur.
Many of the appointments will be scheduled after school and in the evening and
weekend hours. Clients can receive a minimum of one service hour to a maximum of
four service hours per week. Twenty-four hour per day, 7 days per week on call crisis
management is available by a licensed clinician.
B. InnerChoice has eight staff available for this project to provide a wide array of
therapeautic services. Therapists are required to be licensed when possible, signed up
with the grievance board and carry their own personal professional policy. InnerChoice
Agency also has a professional policy through American Professional Agency in New
York.
C. Family therapists will carry a caseload of no more than six families at any given time
each month in Weld County. The average length of stay in the IFT program is two
months.

and accessibility for all families being served through InnerChoice Counseling
Center*
D. Structural and multi-systemic family therapy training and booster training sessions will
be provided eight hours per month to all staff engaged in the treatment of families.
Interventions will be clinically supervised and monitored and assessed for effectiveness
on a weekly basis. Staff providing multi-systemic interventions will be requested to also
access training outside of the agency to compliment the already existing service training
format. InnerChoice will gear a great deal of the training and interventions to treat
serious oppositional defiance and anti-social behaviors in youth. Staff members will be
trained in utilizing a strength's model and interventions and requested to be creative
and focus on joining techniques when engaging with a new client. Strong emphasis will
be requested in regard to connecting the families with their next-of-kin, familial friends,
neighborhood resources and community networks.
E. Service hours usually range from two to four hours per week for one family on the IFT
caseload.
F. InnerChoice has eight staff members presently employed forte IFT program.
G. Clinical supervisors carry caseloads no greater than six. An outside clinician will be
utilized to supervise the InnerChoice Clinical Director at least one time per month to
make sure that the staff and supervisor are keeping up with state of the art modalities of
treatment.
H. InnerChoice requires that all staff members transporting clients have automobile
insurance coverage and that their policies are reviewed and photocopied for their files
every six months to ensure continuity. .
I. InnerChoice is located at 415 Cleveland Avenue in Loveland, CO and 710 11th Avenue
in Greeley, CO. The future plan is to secure a house in Greeley, Co to expand the
availability of group therapy services.
Plans are also presently being activated for a bed to be secured at a licensed Child
Placement Agency so that when family members need a time out from one another and
parents are ready to throw in the towel we are able to avoid placing the youth in a
residential treatment facility. The youth could be immediately placed at the CPA so that
treatment can continue through InnerChoice, the family can take their time out and
permanent out of home placement is avoided. This CPA placement would offer 24 hour
care and the on call therapist could continue to serve the other families who may be in
crisis or need other services.
VII. ALL InnerChoice Staff Qualifications

Denise E Klein-LCSW Family Service Program Director/Clinical Director
Denise Klein serves as the Program Director for InnerChoice Counseling Center. She will
coordinate inter-agency services and will manage the InnerChoice teams as they provide
services to each family. Denise earned a Bachelor's Degree in Social Work from Minot State
University and a Master's Degree in Social Work from the University of North Dakota. She was
trained in multi-systemic therapy at the graduate level and began her first paid position as an
in-home therapist. She has 12 years of experience in working with youth and families. She has
developed and implemented two separate Intensive In-home Family Preservation Programs in
North Dakota and Minnesota. Denise also worked at the Grand Forks Air Force Base with
families with multi-cultural backgrounds providing service to youth and their families reported
for child and spouse abuse. Denise has also served as a guardian ad litem, court appointed
visitor, supervised visitation monitor and foster parent.
Clinical Supervisor(2)-This position requires a high degree of training in the MST model and
a master's degree in counseling or social work.
Family Therapist(4) -This position requires a minimum of a master's degree in social work
or a master's degree in marriage and family counseling(Focus on MST Model)
Behavioral Specialist/Connseling position (4) - This position requires a minimum of a
bachelor's degree in social work, counseling,psychology or divinity.(MST Supervision)
CAC I,II,III (4)-This position requires the necessary supervision hours and course work to
be certified as a certified addictions counselor. Follow-up supervision through InnerChoice will
be provided by a CAC III trained clinician.(MST Supervision)
A. Staff including supervisors will meet the training and minimum qualifications described
in the Staff manual Volume VII, section 7.303.17 and Section 7.006,Q, Colorado
Department of Human Services.
B. Eight staff are available for this project and two will serve as part-time master's level
clinical supervisors.
C. All staff members go through an initial four hour training period and receive eight
hours of going booster training sessions per month. Therapist's will be trained in multi-
systemic strength based family system's theory in order to treat families with serious
oppositional defiant and conduct disorder youth. This model also addresses the issues
of the younger children within the home by Meting prevention issues and the younger
siblings onset of behavioral problems.
D. All staff will be trained in risk assessment and have a minimum sixteen hours of
continuing education units per year that will be documented for the county..
E. EFT therapist's will have at least one hour of MST supervision per week
F. The clinical supervisors will go to yearly trainings on the MST model and will have at
least 16 hours of continuing CEU's in other state of the art modalities.

Addendum to Minimum and Maximum Number of Clients Served
Innerchoice is designed to meet a wide continuum of level of care based on the minimum
and maximum number of clients to be served.We have many part-time staff who are
flexible with their hours to either increase service or decrease service hours as needs
increase and decrease with the families and with county referrals.We have cross over
programs that help to counter the effects of a decrease in referrals in a program.
Overheads costs are kept low to ensure the maximum amount of services for a lower
cost to the county.

VIII. COMPUTATION OF DIRECT SERVICE RATE
This form is to be used to provide detailed explanation of the hourly rate your
organization will charge the Core Services Program for the services offered in
this Request for Proposal. This rate may only be used to bill the Weld County
Department of Social Services for direct, face-to-face services provided to
clients referred for these services by the Department. Requests for payment based
on units of service such as telephone calls, no shows, travel time, mileage
reimbursement, preparation, documentation, and other costs not involving direct
face-to-face services will not be honored. Likewise, billings must be for hours
of direct service to the client, regardless of, the number of staff involved in
providing those services. Therefore, it is imperative that this rate be
sufficient to cover all costs associated with this client, regardless of the

on our vendor list. The FYC Commission attached the following
recommendations and/or conditions regarding your REP bid(s).

I. Program Description
InnerChoice Counseling Center provides Home Based Family Therapy(Option B) services
combined with numerous wrap around services to target at-risk youth and their families.
The primary goals of the InnerChoice Family Preservation Program are to enhance family
functioning, create cohesiveness, and help families to become empowered and independent
from the system. The services are provided to families within their natural environments via
homes, schools, and neighborhood settings. The program is designed to prevent costly out-
of-home placements using a multi-systemic strength's based model.
The goal of the MST model approach is to provide an integrative, cost effective family
based treatment that results in positive outcomes for children who display serious
oppositional and conduct disorders. MST focuses on improving psycho-social functioning
for youth and their families so that the need for out- of- home child placements is reduced
or eliminated.
The program is specifically tailored to meet the unique needs of the family through a two
tiered step-down program. The family can enter at either level and move up or down in the
amount of service hours provided based on the Weld county case manager's approval. The
maximum length of stay in the program is four months. The program is goal directed so that
family members have definite time lines in which to accomplish their goals.
InnerChoice has over twenty staff members with diverse interests, cultural backgrounds and
educational levels so that the unique needs, interests and strengths of the family can be
addressed by staff with similar interests and cultural sensitivity. InnerChoice programming
is designed to provide the families with the optimal ability to be successful and remain
intact. InnerChoice therapists empower families by using identified strengths to develop
natural support systems and view the parents as the solution, as opposed to the problem.
InnerChoice also provides treatment to the younger siblings of the identified client to
prevent further involvement within the system.They currently serve both males and females,
between the ages of 0-18, and their families.
Staff are available 24 hours per day, 7 days per week and can usually meet at the families'
convenience, resulting in many evening and weekend appointments. An on-call therapist
carries a pager to be accessible for immediate emergencies and crisis interventions.
InnerChoice employs a team of six multi-systemic therapists who serve as team leaders and
have no more than four families on their caseload at any given time. Caseloads are kept
small to prevent staff burn-out and to keep creativity and accessibility high for the families
they serve. The multi-systemic family therapists clinically supervise and coordinate the
implementation of wrap-around services. A large umbrella of wrap-around services has
been created to address the vast needs of the family as expediently and efficiently as
possible. Special emphasis is placed on family strengths, creativity, and joining and
respecting all family members on their own terms, helping families to build on their
strengths in ways that enhance self-respect and offer families specific ways to improve.
The components that are compelling and make the InnerChoice Program unique are weekly
multi-disciplinary meetings of all family treatment plans,the utilization of cost effective and
well-trained family-aids, behavior management specialists, and mentors. An intense
emphasis is placed on helping family members to become more connected with their natural
resources so that they can become less dependent on the system.
Mission Statement:
The mission of InnerChoice is to prevent out-of-home placements for children and help
families to become self-sufficient and independent from the system.
• By providing a safe environment for families to explore issues that affect the child's
ability to remain in the home and help family members to problem solve together so
that the family unit can remain intact.
• By providing an individualized family treatment plan which identifies the needs of
each family member so that wrap-around services can be tailored to meet the"unique
needs" of the family and the culturally sensitive requirements of the family.
• By teaching family members to come from a strengths perspective and empower them
to advocate for their themselves and their family members so that they learn to view
one another as natural supports.
• By empowering family members to view their unique strengths as a powerful tool to
work through family crisis and create a more cohesive family unit.
• By helping family members to feel hopeful about the future in regard to healthy
family functioning and movement away from dependency on the system towards self-
sufficiency.
II. Target/Eligibility Population
Families to be served
• Criteria for Accepting a Family: InnerChoice's staff is committed to providing services
quickly to families that are appropriate for home based services. The first criteria is that the
family includes a child who is at risk for some type of out-of-home placement. The second
criteria is that the family is committed to staying in tact, are "customers for change", and
participate actively in the family preservation work. The third criteria is that other less
intense services have proven to be insufficient.
• The home based family therapist/team leader will not carry a caseload greater than four
families. Three family therapists will be available, providing a wide range of unique
intervention specializations, so that the appropriate fit between therapist and family can be
made. A total of twelve families can be served at any given time. Youth can range in age
from 0-18. The average number of families served in this program is six families.
Approximately half of those families are estimated to be in south Weld County.
• The gender, ethnic, and diagnostic mental health needs of the family will have TOP
PRIORITY in matching the family with the most appropriate staff. For example, if a family
would be more open to a male therapist, every effort would be made to accommodate that
request. If a Hispanic family is referred, a Hispanic family therapist or Hispanic family aid
will be provided to the family. Special hiring measures have been taken so that Hispanic and
bilingual staff are available to serve Hispanic families. Every Hispanic family referred to
InnerChoice will have a team member that is able to speak the language and provide
culturally sensitive interventions. If a family member is diagnosed with a mental health
issue that a particular therapist has more expertise in treating, then that therapist will be
matched with that particular individual client. At least one-third of the families served will
receive bi-cultural/bilingual services. At least one third of the families served will live in
South Weld county.
• Family strengths will also be prioritized in selecting therapists, behavioral management
specialists, family-aids, and mentors. InnerChoice prides itself in having a large staff with
diversified training, expertise,and experience in working with a wide array of families. For
example, if a family is interested in being artistically creative,they will be provided an art
therapist;or if they enjoy the outdoors and sports activities, an experiential therapist will be
utilized;if a family member enjoys spending time with animals,then an equine therapist will
be utilized.
• Average length of stay in the program is three months. InnerChoice staff work as quickly and
efficiently as possible to help the family to create the change required to keep the child within
the home. The goal is to immediately start connecting the family with their natural support
systems within the community so that the length of stay in treatment cane be shortened.
• The average number of direct service hours per week is six to fourteen hours per family.
InnerChoice places a great deal of emphasis on providing excellent communication with
collateral contacts, documentation,multi-disciplinary meetings and clinical reviews so that
the flow of communication remains consistent and current.
• Program availability is 24 hour per day on call crisis coverage for all families. A licensed
clinical social worker or mental health professional is available for case management and
crisis intervention 24 hours per day, 7 days per week.
• It is on very rare occasions that InnerChoice would recommend that the family be involved
in family preservation for more than five months. Because we expedite the natural
movement towards outside support groups and service providers along with encouragement
towards natural familial supports the length of stay is shortened. We do a great deal of work
with Senate Bill 94 through their mentoring program so the child is generally followed by
another funding source as soon as possible to deter from dependency on InnerChoice
therapists. We are in the process of securing a contract with Weld County Senate Bill 94 at
this point and time.
III. Types of Services Provided
The InnerChoice Home Based(Option B)Family Therapy Program provides a maximum of fourteen
service hours per week for families. The program is tiered so that the treatment plan can be
individualized to meet the specific needs of the family.
Once a referral is accepted,the family therapist will make contact with the family within 48 hours.
An intake will be scheduled at the families earliest convenience. Contact will be made with all
family members including extended family to assess the level of support within the family unit. If
familial friends are available for consult and support , InnerChoice will make contact with them
when deemed appropriate by agency supervisors and referring Weld County Case Managers.Update
reports will be provided to Weld County Case Managers on a weekly basis in an effort to keep the
flow of communication clear and consistent.
Crisis intervention will be available 24 hours per day, 7 days per week. An average of one hour
direct care (face-to-face) response time to mediate crisis situations will be available through
InnerChoice. Telephone crisis consults will be dealt with immediately. If the client is in imminent
danger,the Greeley 911 emergency contact will be called to intervene. Detoxification,when deemed
appropriate,will be addressed through Island Grove Treatment Center. InnerChoice will make every
effort to establish good working relationships with all other professionals involved with the case so
that coordination of care is efficient and cost effective to the County.
A. Program Positions and Services
IN-HOME FAMILY THERAPIST/TEAM LEADER
A licensed clinical social worker, psychologist or licensed marriage and family counselor
trained to assess,diagnose and treat families,will provide multi-systemic family therapy and
also serve as a team leader in the delegation, coordination and budgeting of wrap-around
services. Wrap-around services begin as in-house services through InnerChoice to include:
individual therapy, family-aid service, recreational programming, group therapy, and
mentoring. However,the staff will assess for"natural supports"and start connecting family
members with their community supports by at least the second week of enrollment in the
program.
The family therapist/team leader will place special emphasis on maintaining good
communication with the referring case manager, extended family members and all other
collateral contacts. They will schedule multi-disciplinary meetings weekly to review
treatment goals and supervise their team of staff providing the wrap-around services to the
family. These meetings ensure quality management and expedient service delivery. They
provide a consistent time frame to measure and document the effectiveness of treatment goals
relative to family functioning. The multi-disciplinary meetings also ensure that the treatment
goals are on target and continue to meet the family's changing needs and can be modified as
issues change.
The family therapist will also provide crisis management and intensive in-home family
therapy to immediate family members and extended family members either in their own
homes or if necessary in an office environment. When necessary re-parenting and guided
imagery sessions will be conducted in the office when private sessions are warranted.
Support groups will also be held within an office setting as opposed to being conducted
within the client's home. The level and type of services provided will be determined by the
unique needs of the family and the referring case manager. Twenty-four hour on-
call crisis management is always available to family members in this program. An on-
call number is provided at the time of intake to all family members.
BEHAVIOR MANAGEMENT SPECIALIST
The clinically trained Behavior Management Specialist will be available to provide
individual therapy and group therapy sessions for clients requiring specialized individual
treatment such as a mental health diagnosis,cognitive thinking errors, life-skills curriculum,
communication and assertiveness classes, parenting classes, self-esteem enrichment and
behavioral or addiction issues. Behavior Management Specialists will be required to have
at least a Bachelor's Degree in Social Work, a CAC I, II, or III, or a counseling degree.
Referrals to outside agencies may occur if the diagnosis requires specialized expertise and
treatment that is unavailable through InnerChoice staff.
FAMILY-AIDS
Family-aids are a cost effective way of complimenting therapeutic services and teaching
client to access other agencies and community resources.. Family- aids can visit with the
parents and other family members in their home on a daily basis initially as a support to the
family in following the therapist's treatment recommendations. As the family progresses
in treatment, the family-aid can reduce their level of contact. Family-Aids are trained to
assist families with parenting skills, stress reduction,homemaking and housekeeping skills,
budgeting and money management issues, net-working with other agencies, structuring
mealtime and family recreational activities, problem solving, self care, leisure time,
communication skills,medication management, self-advocacy, consumer skills, life skills,
and behavior modification techniques. A Family-Aid will be utilized to support the
therapists' recommendations and the treatment plan and will help to implement consistency
within the family and ultimately be connected with after-care services.
MENTORS
Peer mentors and adult mentors are available to serve youth and will be trained and
clinically supervised to spend one-on-one time with youth participating in leisure activities,
tutoring,tracking,and role modeling healthy life skills. Mentors will be available to provide
respite support for the family as needed. Peer mentors and adult mentors' job is to teach
clients creative and healthy ways to have fun without using drugs and alcohol.
COUPLES THERAPY AND CO-THERAPY
Couples counseling will be available to individuals having issues with blended families,
extended families, marital problems, united front issues, non nuclear family interventions
and mediation issues. Co-therapy is also available when deemed necessary for movement in
therapy.
GROUP THERAPY
Ongoing groups are available on a weekly basis for anger management, life skills,
empowerment, parenting skills, a Dad's support group and a Mom's support group. Other
groups will be developed as needs arise with the families. These groups will also be provided
as an after-care program on a sliding fee scale for families referred through Weld County
Social Services
DRUG ALCOHOL THERAPY
Substance abuse treatment provided by a Certified Addictions Counselor will include drug
and alcohol assessments,administering UA's and breathalyzers, drug and alcohol treatment
both individual and family, after care support, and referrals and transportation to outside
support systems such as AA,Al-A-Teen and Al-Anon. This service will be available to all
family members with addiction issues.
SPLASH EVENTS
Splash Events are events organized by the mentor manager that provide a healthy and
creative recreational outlet for youth and their families. Transportation will be provided for
all family members.
RESTORATIVE JUSTICE EVENTS
Restorative Justice Events are organized by the mentor manager and clinical supervisor in
an effort to help clients learn to give back to their communities, explore their consciences
and help create victim awareness and empathy through organized events which clients will
be requested to volunteer their time at.

E. Assessment and Plan
1. After the initial referral is made to InnerChoice, the family therapist will initiate a
contact with the family and assess appropriateness of service. Once it is established
that the family can benefit from services, a contract for level of care will be
established. The level of care will be determined by the unique needs of the family
and number of outside service providers involved. Once the level of care has been
determined,the family therapist will make contact with Weld County Case Manager
to formally authorize the level of care.
Two Levels of Care and Length of Stay
Level "A"Service -minimum of 12 hours of direct service per week
( Six week average length of stay at this level) $ 2,300.00
Level "C"Service - minimum of 3 hours of direct service per week

Level "A" Service is the intensive version of Home Based family services. Level A
includes a family therapist who does an assessment and makes a recommendation to the case
manager in regard to what wrap around services are deemed necessary. The family therapist
will provide the family therapy and staff directives to the InnerChoice mentors, family-aids
and individual therapists with approval of the Weld County case manager.
Level "C"Service is for families who are not in acute crisis and is a less intensive mode
of treatment.. These families may require psycho-educational services,case management
services, and therapeutic interventions necessary to keep the child stabilized within the
home. The Level C service includes a family therapist who provides therapy and assesses
and monitors the level of wrap around services necessary to keep the family stable and
networked independently within the community.
F. Pro-Active Planning
1. The reintegration plan will begin to be developed as early as the first weeks
enrollment in the program. The family therapist will be looking at natural support
systems via kin and the community so that the family does not become dependent on
InnerChoice staff.
2. Initially, InnerChoice will provide micro-management in regard to accessing and
utilizing these natural supports, however, once the family members have been
provided the necessary skills in accessing the support they will then be encouraged
to develop and strengthen these strategies with little outside intervention.
3. Continuity of care with other agencies providing services to the family will be
prioritized throughout the length of stay. InnerChoice staff will contact collateral
contacts on a weekly basis to ensure the quality of inter-agency communications and
coordination of treatment so that treatment goals can be targeted as a team.
4. Inner-agency staf ings will occur on a weekly basis to review treatment goals and
ensure that the therapists are helping the families to work towards their goals in an
efficient and expedient fashion.
5. Diagnostic and medical evaluations will be given top priority so that medical and
mental health concerns can be addressed as quickly as possible. It is important that
any impairment from medical/mental health complications can be minimized and
treated so that clients can accelerate their progress towards their therapeutic goals.
6. After-care groups and splash events will be provided on a sliding fee scale for family
members so that they can continue to access support and maintain their treatment
goals through InnerChoice.
7. Clinicians will utilize the assessment tool to gather initial assessment scores, three
month assessment scores and discharge scores. A 12-month follow-up assessment
will be done to determine success and recidivism rates.
8. Program availability is 24 hour per day on call crisis coverage for all families. A
licensed clinical social worker or mental health professional carries a pager and is
available for case management and crisis intervention 24 hours per day, 7 days per
week.A critical incident report form will be filled out and sent to the county any time
a crisis contact is made with the on-call therapist. The on-call therapist will also be
trained to identify when detoxification services will be utilized and coordinated
through Island Grove. On- call therapists are available to make on-sight visits to
Island Grove Detoxification Center,North Range Behavioral Health,Psych-Care,the
hospital and the police station to coordinate care plans.

9. Quantitative measures will be approximately 70 percent therapy services(direct care)
to include the administration of psycho-educational assistance in the development
of life-skills such as stress reduction, problem solving and hands-on parenting.
Fifteen percent will be crisis management and the final percent will be an additional
15 percent directed towards collateral contact training and referral based services.
Clinicians will make sure that services are not duplicated by contacting and
collaborating with other service providers on a weekly basis. Capitated services will
be utilized whenever possible to lower the costs to the county and when formally
approved by the county case worker.
IV. Measurable Outcomes
A. 80%of children who are provided services will remain within their home at time of closure.
B. 95% of the families will improve their parental competency ratings prior to closure of
treatment.
C. 80% of children who are currently in their home will remain in their home twelve months
following case closure.
D. 80% of children currently in long term placement facilities provided reunification Home
Based Intensive Family Intervention Services will return to their home and not render out-of-
home placement twelve months after completion of Homes Based Family Intervention.
E. 90%of the families will not have a subsequent report of neglect or abuse.
F. 90% of cases that receive family preservation services will register low on the risk
assessment devise at service closure.
Project Monitoring and Evaluation
Pre-tests and post-tests(Child Well-Being Scale and Family Risk Scale)will be administered to the
families immediately before and following the implementation of services. A six month and twelve
month follow-up survey will be done to assess for success and recidivism.
InnerChoice will monitor daily,weekly and monthly hard and soft services provided for the family.
This information will be provided to the county on a monthly basis for evaluation and program
management review.
The program will also fill out quarterly client progress reports as requested by the County.
A case is terminated as a treatment success when: The parent(s) have the motivation and skills
necessary for handling subsequent problems,the youth is involved with pro-social peers and actively
involved in a community activity that is deemed healthy and constructive, and the family has been
able to function reasonably well with minimal support from InnerChoice for a period for at least four
weeks.
Treatment is prematurely terminated when: the clients are not customers for change, the services
required are beyond the out-patient level of care and/or the safety of the child or community is not
assured.
V. Service Objectives
A. 90% of the families will improve their communication skills and problem solving skills
through varied and creative therapeutic styles and interventions such as structural family
therapy, multi-systemic therapy, psychotherapy, psycho-drama, guided imagery, equine
therapy, art therapy, music therapy,mediation services for extended family, CAC services
for addiction issues and cognitive behavioral therapy for behavior management issues.
Gender appropriate and culturally appropriate therapeutic matches will be made with families
to increas- success outcomes. Families will reduce their negative resistance through the
therapists' specialized training in uncovering the unique needs and strengths of the families
and their joining skills..
B. Group parenting classes will be conducted. InnerChoice will provide transportation for the
mother and/or father to attend parenting and empowerment class . This will allow parents
to work together to find solutions and vent their frustration in regard to ongoing parenting
concerns and issues.Individual treatment plans will be addressed by the family therapist and
the family-aids will support the recommendations that the therapists make to the families on
a weekly basis. Hands on skills will be reviewed and monitored by family-aids to assess for
progress in treatment on a weekly basis.
C. Family-aids will work towards increasing parental competency in parenting skills,
homemaking and housekeeping skills, accessing resources, budgeting and money
management issues, structuring mealtime and family activities, stress reduction, problem
solving, self care, leisure time, communication skills, medication management, consumer
skills, life skills, and behavior modification techniques.
D. 95 %of the families will develop alliances through natural familial and community supports
such as extended family, church, health care professionals, neighborhood groups and a
variety of supportive organizations through the implementation of mentors and family-aids
who will provide transportation and support to these meetings and activities until family
members access their own natural supports. Family- aids and mentors will also provide
assistance in establishing resources and accessing services through other state programs such
as Housing for Humanity, CHIP, LEAP and Social Security.
* Family-aids and behavior specialists will keep monthly survey records to identify and
substantiate progress in parenting skills and deficits. These progress review surveys will be
compiled so that statistics can be accumulated in regard to what"hands on"parenting and
life skills were provided to the parents and to what degree the interventions and education
were successful.
VI. Workload Standards
A. A typical staffing pattern for the provision of home based services for one family is a
treatment team consisting of one Master's Level Therapist, one Behavior Management
Specialist and one mentor. In cases where there are parental education needs and/or neglect,
a family-aid will be brought in to assist the parents with these concerns or a parenting group
will be encouraged.Minimal number of hours is three per week and maximum number of
service hours is 14 per week.
Appointments are available to families seven days per week throughout the year so that
optimal utilization and implementation of services can occur. Many of the appointments will
be scheduled after school and in the evening and weekend hours. Twenty-four hour per day,
7 days per week on call crisis management is available by a licensed clinician.
B. InnerChoice has over 20 staff available to provide a wide array of services and work as team
members.
C. Family therapists/team leaders will serve no more than four families at any given time.
Behavioral management specialists will serve no more than six individual clients at any
given time.Family-aids will serve no more than four families at any given time.Mentors will
have no more than five clients at any given time.. The average length of stay in this program
is two to four months.
*Case loads will remain small to avoid staff burn-out, improve quality management and
accessibility for all families being served through Innerchoice Counseling Center*
D. Multi-systemic and structural family therapy training and booster training sessions will be
provided 8 hours per month to all staff engaged in the treatment of families. Interventions
will be clinically supervised and monitored and assessed for effectiveness on a weekly basis.
Staff providing multi-systemic interventions will be requested to also access training outside
of the agency to compliment the already existing service training format. InnerChoice will
gear a great deal of the training and interventions to treat serious oppositional defiance and
anti-social behaviors in youth. Staff members will be trained in utilizing a strength's model
and interventions and requested to be creative and focus on joining techniques when
engaging with a new client. Strong emphasis will be requested in regard to connecting the
families with their next-of-kin, familial friends, neighborhood resources and community
networks.
E. Service hours usually range from 3 hours per week to 14 hours per week for one family on
the caseload.
F. InnerChoice has 24 staff members presently employed.
G. Clinical supervisors carry caseloads no greater than six. The clinical director will be
clinically supervised by an outside clinician to ensure that current modalities of treatment are
maintained and kept updated.
H. InnerChoice requires that all staff members transporting clients have automobile insurance
coverage and that their policies are reviewed and photocopied for their files every six
months to ensure continuity. The agency itself has professional liability coverage through
American Professional Agency in New York,New York.
I. InnerChoice is located at 415 Cleveland Avenue in Loveland, CO and 710 11th Avenue in
Greeley,CO.The future plan is to secure a house in Greeley,CO to increase space for splash
events,psychotherapy and psycho-educational groups.
Plans are also presently being activated for a bed to be secured at a licensed Child Placement
Agency so that when family members need a time out from one another and parents are
ready to throw in the towel we are able to avoid placing the youth in a residential treatment
facility. The youth could be immediately placed at the CPA so that treatment can continue
through InnerChoice, the family can take their time out and permanent out of home
placement is avoided. This CPA placement would offer 24 hour care and the on call therapist
could continue to serve the other families who may be in crisis or need other services.

Denise E. Klein - LCSW Family Service Program Director
Denise will serve as the Program Director for InnerChoice Counseling Center. She will coordinate
inter-agency services and will manage the InnerChoice teams as they provide services to each family.
Denise earned a Bachelor's Degree in Social Work from Minot State University and a Master's
Degree in Social Work from the University of North Dakota. She was trained in multi-systemic
therapy at the graduate level and began her first paid position as an in-home therapist. She has 12
years of experience in working with youth and families. She has developed and implemented two
separate Intensive In-home Family Preservation Programs in North Dakota and Minnesota. Denise
also worked at the Grand Forks Air Force Base with families with multi-cultural backgrounds
providing service to youth and their families reported for child and spouse abuse. Denise has also
served as a guardian ad litem,court appointed visitor,supervised visitation monitor and foster parent.
Clinical Supervisor(2)-This position requires a high degree of training in the MST model and a
master's degree in counseling or social work.
Family Therapist/ Team leader Position (6) - This position requires a minimum of a master's
degree in social work or a master's degree in marriage and family counseling.
Behavioral Specialist/Counseling position (4)-This position requires a minimum of a bachelor's
degree in social work, counseling,psychology or divinity.
Family Aid (4)- This position requires training and supervision in parent education, money
management skills, life skills, assertiveness training and behavior modification. The family aid will
be trained and supervised by a master's level clinician.
Mentor (10) - This position requires that the individual role-model healthy life skills and a large
capacity to join with a client in a positive and productive way. The mentors will be trained and
clinically supervised by a masters level clinician.
CAC I, II, In (5)- This position requires the necessary supervision hours and course work to be
certified as a certified addictions counselor. Follow-up supervision through InnerChoice will be
provided by a CAC III trained clinician.
A. Staff including supervisors will meet the training and minimum qualifications described in
the Staff manual Volume VII, section 7.303.17 and Section 7.006,Q, Colorado Department
of Human Services.
B. 24 staff are available for this program, three of whom are in supervisory positions.
C. All staff members go through an initial four hour training period and receive eight hours of
going booster training sessions per month and mandated new caseworker training.
D. All staff will be trained in risk assessment at the earliest possible date.
E. The staff will complete the State Home Based Family Intensive Family Services Training
Component and the mandated new caseworker training at the earliest possible date.
F. InnerChoice has hired bilingual and Hispanic staff to educate other staff members in regard
to culturally sensitive interventions and treatment. The Hispanic staff provide consultation
to our staff at least two hours per week and also work directly with the families in their
homes and communities.

RFP Bidding process outcome letter shall be shared with Innerchoice staff members
and incorporated into our program protocols.

The purpose of this letter is to outline the results of the RFP Bid process for PY 2002-
2003 and to request written information or confirmation from you by Wednesday, April
17, 2002.
A. Results of the RFP Bid Process for PY 2002-2003
Through the 2002-2003 Core Services bid evaluation process,the Families, Youth
and Children(FYC)Commission approved the RFP(s)listed above for inclusion
on our vendor list. The FYC Commission attached the following
recommendations and/or conditions regarding your RFP bid(s).
The FYC Commission approved the following recommendation for all programs
on the vendor list for 2002-2003. The recommendation reads as follows:

The Families, Youth and Children Commission, an advisory commission to the Weld County Department of
Social Services, announces that competing applications will be accepted for approved vendors pursuant to the
Board of Weld County Commissioners' authority under the Statewide Family Preservation Program (C.R.S.
26-5.5-101) and Emergency Assistance for Families with Children at Imminent Risk of Out-of-Home
Placement(C.R.S. 26-5.3-101). The Families, Youth and Children Commission wishes to approve services
targeted to run from June 1, 2002, through May 31, 2003, at specific rates for different types of service. The

I. Program Description
InnerChoice Counseling Center provides highly intensive in-home family therapy treatment
and intensive case management services to at-risk youth and their families. The services are
provided to families within their natural environments via homes, schools, and
neighborhood settings. The program is designed to prevent high out-of-home placement
costs and to help families stay intact.
The goal of the multi-systemic strength based model is to provide an integrative, cost
effective family based treatment that results in positive outcomes for children who display
serious oppositional and conduct disorders. MST focuses on improving psycho-social
functioning for youth and their families so that the need for out- of- home child placements
is reduced or eliminated.
The program is specifically tailored to meet the unique needs of the family through a four
tiered step-down program. The family can enter at any given tier, based on the level of need
and care required. InnerChoice has over twenty staff members with diverse interests,
cultural backgrounds and educational levels so that the unique needs, interests and strengths
of the family can be addressed by staff with similar interests and cultural sensitivity.
InnerChoice programming is designed to provide the families with every opportunity to
succeed at remaining intact.
Staff are available 24 hours per day, 7 days per week and can generally meet at the families'
convenience,resulting in many evening and weekend appointments. InnerChoice therapists
empower families by using identified strengths to develop natural support systems and view
the parents as the solution, as opposed to the problem. InnerChoice also provides treatment
to the younger siblings of the identified client to prevent further involvement within the
system. They currently serve both males and females, between the ages of 0-18, and their
families. Services are provided for three to five months in duration.
InnerChoice employs a team of six multi-systemic structural family therapists who serve as
team leaders and have no more than four families on their caseload at any given time.
Caseloads are kept small to prevent staff burn-out and to keep creativity and accessibility
high for the families they serve. The structural family therapists clinically supervise team
members and coordinate the implementation of wrap-around services. A large umbrella of
wrap-around services has been created to address the vast needs of the family as expediently
and efficiently as possible. Special emphasis is placed on family strengths, creativity, and
joining and respecting all family members on their own terms, helping families to build on
their strengths in ways that enhance self-respect and offer families specific ways to improve.
The primary goals of the InnerChoice Family Preservation Program are to enhance family
functioning, create cohesiveness, and help families to become empowered and independent
from the system. The components that are compelling and make the InnerChoice Program
unique are weekly multi-disciplinary meetings of all family therapy treatment plans, the
utilization of family-aids, behavior management specialists, and mentors. An intense
emphasis is placed on helping family members to become more connected with their natural
resources so that they can become less dependent on the system.
Mission Statement:
The mission of InnerChoice is to prevent out-of-home placements for children and help
families to become self-sufficient and independent from the system.
• By providing a safe environment for families to explore issues that affect the child's
ability to remain in the home and help family members to problem solve together so
that the family unit can remain intact.
• By providing an individualized family treatment plan which identifies the needs of
each family member so that wrap-around services can be tailored to meet the"unique
needs" of the family.
• By teaching family members to come from a strengths perspective and empower them
to advocate for their themselves and their family members so that they learn to view
one another as natural supports.
• By empowering family members to view their unique strengths as a powerful tool to
work through family crisis and create a more cohesive family unit.
• By helping family members to feel hopeful about the future in regard to healthy
family functioning and movement away from dependency on the system towards self-
sufficiency.
II. Target/Eligibility Population
Families to be served
• Criteria for accepting a family: InnerChoice's staff is committed to providing services
quickly to families that are appropriate for in-home services. The first criteria is that the
family includes a child who is at risk for out-of-home placement. The second criteria is that
the family is committed to staying in tact,are customers for change, and participate actively
in the family preservation work. The third criteria is that other less intense services have
proven to be insufficient.
• The in-home family therapist/team leader will not carry a caseload greater than four families.
Six family therapists will be available, providing a wide range of unique intervention
specializations, so that the appropriate fit between therapist and family can be made. A total
of 24 families can be served at any given time. Youth can range in age from 0-18. The
average number of families served is 17 families. Approximately half of those families are
estimated to be in south Weld County and have availability to bilinguaUbi-cultural services..
• The gender, ethnic, and diagnostic mental health needs of the family will have TOP
PRIORITY in matching the family with the most appropriate staff. For example, if a family
would be more open to a male therapist, every effort would be made to accommodate that
request. If a Hispanic family is referred,a Hispanic family therapist or Hispanic family aid
will be provided to the family. Special hiring measures have been taken so that Hispanic and
bilingual staff are available to serve Hispanic families. Every Hispanic family referred to
InnerChoice will have a team member that is able to speak the language and provide
culturally sensitive interventions. If a family member is diagnosed with a mental health
issue that a particular therapist has more expertise in treating, then that therapist will be
matched with that particular individual client.
• Family strengths will also be prioritized in selecting therapists, behavioral management
specialists, family-aids, and mentors. InnerChoice prides itself in having a large staff with
diversified training,expertise,and experience in working with a wide array of families. For
example,if a family is interested in being artistically creative,they will be provided an art
therapist; or if they enjoy the outdoors and sports activities,an experiential therapist will be
utilized;if a family member enjoys spending time with animals,then an equine therapist will
be utilized.
• Average length of stay in the program is three to five months. InnerChoice staff work as
quickly and efficiently as possible to help the family to create the change required to keep
the child within the home. The goal is to immediately start connecting the family with their
natural support systems within the community so that the length of stay in treatment cane be
shortened.
• The average number of direct/indirect service hours per week is 10-12 per family.
InnerChoice places a great deal of emphasis on providing excellent communication with
collateral contacts,documentation,multi-disciplinary meetings and clinical reviews so that
the flow of communication remains consistent and current.
• Program availability is 24 hour per day on call crisis coverage for all families. A licensed
clinical social worker or mental health professional is available for case management and
crisis intervention 24 hours per day, 7 days per week.
• The average length of stay in the program is 3-5 months. Generally, by the fifth month of
service the families are down to the lowest level care on the continuum and the lowest cost
level. Generally, the family would be receiving only mentoring and case management
services at this time and would be networked into their natural familial alliances and
community service providers. If the service goes beyond six months of care, it is at a low
level of service so as not to nurture dependency on the service providers or the county.
• InnerChoice is networked with Senate Bill 94, AA, Al-A-Non,Al-A-Teen,
The Community Center,The Boy's and Girl's Club, Local Churches, Cornerstone,
Athletic Clubs and numerous other community support systems who support families within
Weld County.
***Innerchoice staff have reviewed and researched the common modalities of service being
offered to youthand their families in the state of Colorado and have chosen the modality of
the M ST MODEL that at this given time is best proven to be effective in reducing out of
home placements for youth and reunifying children with their permanent families.***
III. Types of Services Provided
The InnerChoice Intensive In-Home Family Therapy Program provides a maximum of 25 service
hours per week for families. The program is tiered so that the treatment plan can be individualized
to meet the specific needs of the family. There are four levels of service available and four team
staffing positions which address the family needs.
Once a referral is accepted,the family therapist will make contact with the family within 48 hours.
An intake will be scheduled at the families earliest convenience. Contact will be made with all
family members including extended family to assess the level of support within the family unit. If
familial friends are available for consult and support , InnerChoice will make contact with them
when deemed appropriate by agency supervisors and referring Weld County Case Managers. Update
reports will be provided to Weld County Case Managers on a weekly basis in an effort to keep the
flow of communication clear and consistent.
•
Crisis intervention will be available 24 hours per day, 7 days per week. An average of one hour
direct care (face-to-face) response time to mediate crisis situations will be available through
InnerChoice. Telephone crisis consults will be dealt with immediately. If the client is in imminent
danger,the Greeley 911 emergency contact will be called to intervene. Detoxification,when deemed
appropriate,will be addressed through Island Grove Treatment Center. InnerChoice will make every
effort to establish good working relationships with all other professionals involved with the case so
that coordination of care is efficient and cost effective to the County.

IN-HOME FAMILY THERAPIST/TEAM LEADER 4 to 12 hours per week*

A licensed clinical social worker, psychologist or licensed marriage and family counselor
trained to assess, diagnose and treat families and couples will provide family therapy and
couple's therapy and also serve as a team leader in the delegation, coordination and
budgeting of wrap-around services. Wrap- around services begin as in-house services
through InnerChoice to include: individual therapy, family-aid service, recreational
programming, group therapy, and mentoring. However, the staff will assess for "natural
supports"and start connecting family members with their family supports and community
supports by at least the second week of enrollment in the program.
The family therapist/team leader will place special emphasis on maintaining good
communication with the referring case manager and all other collateral contacts. They will
schedule multi-disciplinary meetings weekly to review treatment goals and supervise their
team of staff providing the wrap-around services to the family. These meetings ensure
quality management and expedient service delivery. They provide a consistent time frame
to measure and document the effectiveness of treatment goals relative to family functioning.
The multi-disciplinary meetings also ensure that the treatment goals are on target and
continue to meet the family's changing needs and can be modified as issues change.
The family therapist will also provide crisis management and intensive in-home family
therapy. The level of services provided will be determined by the unique needs of the family
and the referring case manager.
BEHAVIOR MANAGEMENT SPECIALIST 2 to 8 hours per week*

The Behavior Management Specialist will provide individual therapy for clients requiring
specialized individual treatment such as a mental health diagnosis,cognitive thinking errors,
and behavioral or addiction issues. Behavior Management Specialists will be required to
have at least a Bachelor's Degree in Social Work,a CAC I,II,or III, or a counseling degree.
Referrals to outside agencies may occur if the diagnosis requires specialized expertise and
treatment that is unavailable through InnerChoice staff.
FAMILY-AIDS 2 to 8 hours per week*

Family-aids are a cost effective way of complimenting therapeutic services. Family-aids
can visit with the parents and other family members in their home on a daily basis initially
as a support to the family in following the therapist's treatment recommendations. As the
family progresses in treatment,the family-aid can reduce their level of contact. Family-Aids
are trained to assist families with parenting skills, homemaking and housekeeping skills,
budgeting and money management issues, structuring mealtime and family activities, stress
reduction, problem solving, self care, leisure time, communication skills, medication
management,consumer skills,life skills,and behavior modification techniques. A Family-
Aid will be utilized to support the therapists' recommendations and the treatment plan and
will help to implement consistency within the family.
MENTORS 2 to 8 hours per week*
* combination of direct/indirect service hours per family based on level system.
Peer mentors and adult mentors are available to serve each youth and will be trained and
clinically supervised to spend 2 to 8 hours per week, one-on-one, with youth participating
in leisure activities,tutoring,tracking, and role modeling healthy life skills. Mentors will
also be available to provide respite support for the family as needed . Peer mentors and adult
mentors' main job is to teach clients creative and healthy ways in which to have fun without
using drugs and alcohol.
COUPLES THERAPY as needed basis
Couples counseling with one or two therapists will be available to individuals having issues
with blended families,marital problems,united front issues,non nuclear family interventions
and mediation issues.

GROUP THERAPY as needed basis
Ongoing groups are available on a weekly basis for anger management, life skills,
empowerment issues,parenting skills, a Dad's support group and a Mom's support group.
Other groups will be developed as needs arise with the families. These groups will also be
provided as an after-care program on a sliding fee scale for families referred through Weld
County Social Services
DRUG ALCOHOL THERAPY as needed basis
Substance abuse treatment provided by a Certified Addictions Counselor will include drug
and alcohol assessments,administering UA's and breathalyzers, drug and alcohol treatment
both individual and family, after care support, and referrals and transportation to outside
support systems such as AA , Al-ATeen and Al-Anon. This service will be available to
family members with addiction issues.
SPLASH EVENTS one time per month
Splash Events are events organized by the mentor manager that provide a healthy and
creative recreational outlet for youth and their families. Transportation will be provided for
all family members.
RESTORATIVE JUSTICE EVENTS one time per month
Restorative Justice Events are organized by the mentor manager in an effort to help clients
learn to give back to their communities, explore their consciences and help create victim
empathy through organized events which clients will be requested to volunteer at.

E. Assessment and Plan
1. After the initial referral is made to InnerChoice, the family therapist will initiate a
contact with the family and assess appropriateness of service. Once it is established
that the family can benefit from services, a contract for level,of care will be
established. The level of care will be determined by the unique needs of the family
and number of outside service providers involved. Once the level of care has been
detuuuined,the family therapist will make contact with Weld County Case Manager
to formally authorize the level of care.
Levels of Care
A+Level of Care-20 +hours of direct and indirect service per week;
A Level of Care- 12 to 20 hours of direct and indirect service per week;
B Level of Care-8 to 12 hours of direct and indirect service per week;
C Level of Care-3 to 7 hours of direct and indirect service per week.
Option A+ is the most intensive version of in-home family services. It is designed to
stabilize families in acute crisis. Option A+ includes a family therapist who provides 10
to 12 hours of direct and indirect services per week. The family therapist will provide a
minimum of six hours of direct care and six hours of case management and supervise the
other InnerChoice staff working with the family providing wrap-around services..

Option A is the second most intensive version of in-home family services. Option A
includes a family therapist who provides 6 to 10 hours of direct and indirect services per
week. The family therapist will provide a minimum of four hours of direct care and two
hours of case management and supervision to other InnerChoice staff working with the
family.

Option B is for families who are not in acute crisis,however, require psycho-educational
services,case management services,and therapeutic interventions necessary to keep the child
in the home. Option B includes a family therapist who provides 3 to 6 hours of direct and
indirect services per week. The family therapist will provide a minimum of three hours of
direct care and two hours of case management and supervision to other InnerChoice staff
working with the family.

Option C is for families transitioning out of the InnerChoice Family Preservation Program.
These families have effectively met their treatment goals and are accessing transitional
therapeutic and case management services in order to stabilize their treatment goals. After

Direct Care
Direct Care is face-to-face contact or telephone contact made with individual family members or the
family as a unit. It can occur in therapy sessions,multi-disciplinary staffings,recreational activities,
group activities, mentoring services and assistance provided to family members while accessing
community supports and case management services.
Indirect Care
Indirect Care does not require the families' presence, however, indirect care is being provided for
the welfare of the family, i.e. telephone contacts to collateral contacts, written reports, court
evaluations, court appearances, accessing hard services for the family, travel time, meetings, and
clinical staffings and supervision for the benefit of the family.
F. Pro-Active Planning
1. The reintegration plan will begin to be developed as early as the second weeks
enrollment in the program. The family therapist will be looking at natural support
systems via kin and the community so that the family does not become dependent on
InnerChoice staff.
2. Initially, InnerChoice will provide micro-management in regard to accessing and
utilizing these natural supports, however, once the family members have been
provided the necessary skills in accessing the support they will then be encouraged
to develop and strengthen these strategies with little outside intervention.
3. Continuity of care with other agencies providing services to the family will be
prioritized throughout the length of stay. ImierChoice staff will teach client's to
access these services and maintain contact with these collateral contacts. Contacts
will be made on a weekly basis to ensure the quality of inter-agency communications
and coordination of treatment so that treatment goals can be targeted as a team.
Client's will be encouraged to maintain contact with these agencies after services are
discontinued by the county.
4. Inner-agency staffings will occur on a weekly basis to review treatment goals and
ensure that the therapists are helping the families to work towards their goals in an
efficient and expedient fashion.
5. Diagnostic and medical evaluations will be given top priority so that medical and
mental health concerns can be addressed as quickly as possible. It is important that
any impairment from medical/mental health complications can be minimized and
treated so that clients can accelerate their progress towards their therapeutic goals.
6. After-care groups and splash events will be provided on a sliding fee scale for family
members so that they can continue to access support and maintain their treatment
goals through InnerChoice. These services will be provided on a sliding fee scale as
part of the families after-care programming.
8. Program availability is 24 hour per day on call crisis coverage for all families. A
licensed clinical social worker or mental health professional carries a pager and is
available for case management and crisis intervention 24 hours per day, 7 days per
week.A critical incident report form will be filled out and sent to the county any time
a crisis contact is made with the on-call therapist. The on-call therapist will also be
trained to identify when detoxification services will be utilized and coordinated
through Island Grove. On-call therapists are available to make on-sight visits to
Island Grove Detoxification Center,North Range,the police station and Psych-Care
for crisis consult and case management.
9. Quantitative measures will be approximately 70 percent therapy services(direct care)
to include the administration of psycho-educational assistance in the development
of life-skills such as stress reduction, problem solving and hands-on parenting.
Fifteen percent will be crisis management and the final percent will be an additional
15 percent directed towards collateral contact training and referral based services.
Clinicians will make sure that services are not duplicated by contacting and
collaborating with other service providers on a weekly basis. Capitated services will
be utilized whenever possible to lower the costs to the county and when formally
approved by the county case worker.
IV. Measurable Outcomes
A. 80%of children who are provided services will remain within their home at time of
closure.
B. 95%of the families will improve their parental competency ratings prior to closure
of treatment.
C. 80% of children who are currently in their home will remain in their home twelve
months following case closure.
D. 80% of children currently in long term placement facilities provided reunification
Home Based Intensive Family Intervention Services will return to their home and not
render out-of-home placement twelve months after completion of Homes Based
Family Intervention.
E. 90%of the families will not have a subsequent report of neglect or abuse.
F. 90% of cases that receive family preservation services will register low on the risk
assessment devise at service closure.
Project Monitoring and Evaluation
Pre-tests and post-tests(Child Well-Being Scale and Family Risk Scale)will be administered to the
families immediately before and following the implementation of services. A six month and twelve
month follow-up survey will be done to assess for success and recidivism.
InneiChoice will monitor daily,weekly and monthly hard and soft services provided for the family.
This information will be provided to the county on a monthly basis for evaluation and program
management review.
The program will also fill out quarterly client progress reports as requested by the County.
A case is terminated as a treatment success when: The parent(s) have the motivation and skills
necessary for handling subsequent problems,the youth is involved with pro-social peers and actively
involved in a community activity that is deemed healthy and constructive, and the family has been
able to function reasonably well with minimal support from InnerChoice for a period for at least four
weeks.
Treatment is prematurely terminated when: the clients are not customers for change, the services
required are beyond the out-patient level of care and/or the safety of the child or community is not
assured.
V. Service Objectives
A. 90% of the families will improve their communication skills and problem solving
skills through varied and creative therapeutic styles and interventions such as
structural family therapy, multi-systemic therapy, psychotherapy, psycho-drama,
guided imagery, equine therapy, art therapy, music therapy, mediation services for
extended family, CAC services for addiction issues and cognitive behavioral therapy
for behavior management issues. Gender appropriate and culturally appropriate
therapeutic matches will be made with families to increase success outcomes.Families
will reduce their negative resistance through the therapists' specialized training in
uncovering the unique needs and strengths of the families and their joining skills..
B. Group parenting classes will be conducted. InnerChoice will provide transportation
for the mother and/or father to attend parenting and empowerment class . This will
allow parents to work together to find solutions and vent their frustration in regard to
ongoing parenting concerns and issues. Individual treatment plans will be addressed
by the family therapist and the family-aids will support the recommendations that the
therapists make to the families on a weekly basis. Hands on skills will be reviewed
and monitored by family-aids to assess for progress in treatment on a weekly basis.
C. Family-aids will work towards increasing parental competency in parenting skills,
homemaking and housekeeping skills, budgeting and money management issues,
structuring mealtime and family activities,stress reduction,problem solving,self care,
leisure time, communication skills, medication management, consumer skills, life
skills, and behavior modification techniques.
D. 95 %of the families will develop alliances through natural familial and community
supports such as extended family, church, health care professionals, neighborhood
groups and a variety of supportive organizations through the implementation of
mentors and family-aids who will provide transportation and support to these meetings
and activities until family members access their own natural supports. Family-aids
and mentors will also provide assistance in establishing resources and accessing
services through other state programs such as Housing for Humanity, CHIP, LEAP
and Social Security.
* Family-aids and behavior specialists will keep monthly survey records to
identify and substantiate progress in parenting skills and deficits. These
progress review surveys will be compiled so that statistics can be accumulated in
regard to what"hands on"parenting and life skills were provided to the parents
and to what degree the interventions and education were successful.
VI. Workload Standards
A. A typical staffing pattern for the provision of intensive in-home services for one
family is a treatment team consisting of one Master's Level Therapist, one Behavior
Management Specialist and one or two Mentors. In cases where there are parental
education needs and/or neglect, a family-aid will be brought in to assist the parents
with these concerns.
Appointments are available to families seven days per week throughout the year so
that optimal utilization and implementation of services can occur. Many of the
appointments will be scheduled after school and in the evening and weekend hours.
Clients can receive a minimum of 3 service hours to a maximum of 25 service hours
per week. Twenty-four hour per day, 7 days per week on call crisis management is
available by a licensed clinician.
B. InnerChoice has over 20 staff available to provide a wide array of services and work
as team members.
C. Family therapists/team leaders will serve no more than four families at any given time.
Behavioral management specialists will serve no more than six individual clients at
any given time. Family-aids will serve no more than four families at any given time.
Mentors will have no more than five clients at any given time. An average of 17
families can be served each month in Weld County. The average length of stay in the
program is three to five months.
*Case loads will remain small to avoid staff burn-out, improve quality management
and accessibility for all families being served through InnerChoice Counseling
Center*
D. Structural family therapy training and booster training sessions will be provided 8
hours per month to all staff engaged in the treatment of families. Interventions will
be clinically supervised and monitored and assessed for effectiveness on a weekly
basis. Staff providing multi-systemic interventions will be requested to also access
training outside of the agency to compliment the already existing service training
format. Innerchoice will gear a great deal of the training and interventions to treat
serious oppositional defiance and anti-social behaviors in youth. Staff members will
be trained in utilizing a strength's model and interventions and requested to be
creative and focus on joining techniques when engaging with a new client. Strong
emphasis will be requested in regard to connecting the families with their next-of-kin,
familial friends, neighborhood resources and community networks.
E. Service hours usually range from 6 hours per week to 25 hours per week for one
family on the caseload.
F. InnerChoice has 24 staff members presently employed.
G. Clinical supervisors carry caseloads no greater than eight.
H. InnerChoice requires that all staff members transporting clients have automobile
insurance coverage and that their policies are reviewed and photocopied for their files
every six months to ensure continuity. The agency itself has liability coverage through
American Professional Agency in New York,New York.
I. InnerChoice is located at 415 Cleveland Avenue in Loveland, CO and 710 I11'
Avenue In Greeley, CO. The future plan is to secure a house in Greeley in order to
offer more groups and life-skills services.
Plans are also presently being activated for a bed to be secured at a licensed Child
Placement Agency so that when family members need a time out from one another
and parents are ready to throw in the towel we are able to avoid placing the youth in
a residential treatment facility. The youth could be immediately placed at the CPA so
that treatment can continue through InnerChoice, the family can take their time out
and permanent out of home placement is avoided. This CPA placement would offer
24 hour care and the on call therapist could continue to serve the other families who
may be in crisis or need other services.
VII. Staff Qualifications
Denise E Klein-LCSW Family Service Program Director
Denise will serve as the Program Director for InnerChoice Counseling Center. She will
coordinate inter-agency services and will manage the InnerChoice teams as they provide
services to each family. Denise earned a Bachelor's Degree in Social Work from Minot State
University and a Master's Degree in Social Work from the University of North Dakota. She
was trained in multi-systemic therapy at the graduate level and began her first paid position
as an in-home therapist. She has 12 years of experience in working with youth and families.
She has developed and implemented two separate Intensive In-home Family Preservation
Programs in North Dakota and Minnesota. Denise also worked at the Grand Forks Air Force
Base with families with multi-cultural backgrounds providing service to youth and their
families reported for child and spouse abuse. Denise has also served as a guardian ad litem,
court appointed visitor, supervised visitation monitor and foster parent.
Family Therapist/ Team leader Position (6) - This position requires a minimum of a
master's degree in social work or a master's degree in marriage and family counseling.
Behavioral Specialist/ Counseling position (4) - This position requires a minimum of a
bachelor's degree in social work, counseling,psychology or divinity.
Family Aid(4)-This position requires training and supervision in parent education,money
management skills, life skills, assertiveness training and behavior modification. The family
aid will be trained and supervised by a master's level clinician.
Mentor(10) - This position requires that the individual role-model healthy life skills and a
large capacity to join with a client in a positive and productive way. The mentors will be
trained and clinically supervised by a masters level clinician.
CAC I, II, III (5)-This position requires the necessary supervision hours and course work
to be certified as a certified addictions counselor. Follow-up supervision through InnerChoice
will be provided by a CAC III trained clinician.
A. Staff including supervisors will meet the training and minimum qualifications
described in the Staff manual Volume VII, section 7.303.17 and Section 7.006,Q,
Colorado Department of Human Services.
B. 24 staff are employed through InnerChoice Counseling Center. Three clinical
supervisors are available to staff
C. All staff members go through an initial four hour training period and receive eight
hours of going booster training sessions per month and the mandated new caseworker
training..
D. All staff will be trained in risk assessment.
E. The staff will complete the State Home Based Family Intensive Family Services
Training Component at the earliest possible date.
F. InnerChoice has hired bilingual and Hispanic staff to educate other staff members in
regard to culturally sensitive interventions and treatment. The Hispanic staff provide
consultation to our staff at least two hours per week and also work directly with the
families in their homes and communities. A Hispanic clinical supervisor will be on
staff to make sure culturally sensitive interventions are being utilized.
***Therapist's will be trained in multi systemic strength based family system's
theory in order to treat families with serious oppositional defiant and conduct
disorder youth. This model also addresses the issues of the younger children within
the home by targeting prevention issues and the younger siblings onset of
behavioral problems.***

Innerchoice is designed to meet a wide continuum of level of care based on the minimum

flexible with their hours to either increase service or decrease service hours as needs
increase and decrease with the families and with county referrals.We have cross over
programs that help to counter the effects of a decrease in referrals in a program.
Overheads costs are kept low to ensure the maximum amount of services for a lower
cost to the county.

I am writing this letter in follow-up to the recent correspondence that I received
regarding the RFP Proposals 02005,02008 and 02110-Option B and Alternatives to
RTC Placements. Per your request,I am noting the number of hours our program
will serve each client per week for each initial referraL

Intensive Family Therapy: Any Number requested

All levels of care shall be determined by the referring case manager through Weld
County Social Services. The outcome of the program will be monitored through the
Child Well Being pre-tests and posts tests. All other recommendations noted in the
RFP Bidding process outcome letter shall be shared with Innerchoice staff members
and incorporated into our program protocols.

The purpose of this letter is to outline the results of the RFP Bid process for PY 2002-
2003 and to request written information or confirmation from you by Wednesday, April
17,2002.
A. Results of the RFP Bid Process for PY 2002-2003
Through the 2002-2003 Core Services bid evaluation process,the Families, Youth
and Children(FYC)Commission approved the RFP(s)listed above for inclusion
on our vendor list. The FYC Commission attached the following
recommendations and/or conditions regarding your RFP bid(s).
The FYC Commission approved the following recommendation for all programs
on the vendor list for 2002-2003. The recommendation reads as follows:
Recommendation:Providers will report outcomes specific to their programs.
The FYC Commission approved the following condition for all Option B and
Intensive Family therapy programs. The condition reads as follows:
Condition:State the number of hours your program will serve each client per
initial referral.

• Innerchoice, IFT, will report outcomes specific to their program. (See
above recommendation for all programs.)
FYC Conditions
• State the maximum number of hours your program will serve each client

• Innerchoice will not bill the Department of Social Services for staffing
hours.
• Innerchoice will,idt set up psychological evaluations for referred clients.
All psychological evaluations will be referred by the Department of Social
Services caseworker.
• Innerchoice will not perform case management services for referred
clients. All case management shall be completed by the Department of
Social Services caseworker. Innerchoice will perform the therapy
component only.
• All levels of care will be determined by the Department of Social Services
caseworker and the caseworker's perception of the referred family s
needs.
• All instances of abuse and neglect will be reported immediately to the
Department of Social Services Intake unit caseworker.
• All grievances will be reported to the caseworker. The caseworker will
initiate a grievance process if merited
• No additional services will be initiated without the approval of the
Department of Social Services caseworker.

Recommendations and Conditions
The Weld County Department of Social Services is requesting your written
response to the FYC Commission's recommendations. Please respond in writing
to
Gloria Romansik,Weld County Department of Social Services,P.O. Box A,
Greeley, CO, 80632,by Wednesday,April 17,2002, close of business, as
follows:
FYC Commission Recommendations:
You are requested to review the FYC Commission recommendations and
to:
a. accept the.recommendation(s) as written by the FYC Commission;
or
a. request alternatives to the FYC Commission's recommendation(s);
or
b. not accept the recommendation(s) of the FYC Commission.
Please provide in writing how you will incorporate the recommendation(s)
into your bid. If you do not accept the recommendation, please provide
written reasons why. All approved recommendations under the NOFAA
will be monitored and evaluated by the FYC Commission.
2. FYC Commission Conditions:
All conditions will be incorporated as part of your RFP Bid and
Notification of Financial Assistance Award(NOFAA). If you do not
accept the condition(s),you will not be authorized as a vendor unless your
mitigating circumstances are accepted by the FYC Commission and the
Weld County Department of Social Services. If you do not accept the
condition, you must provide in writing reasons why. A meeting will be
arranged to discuss your response. Your response to the above conditions
will be incorporated in the RFP Bid and Notification of Financial
Assistance Award.

one will be arranged prior to April 17, 2002.
