RE: APPROVE TASK ORDER CHANGE ORDER LETTER#1 FOR HEALTH CARE
PROGRAM FOR CHILDREN WITH SPECIAL NEEDS AND AUTHORIZE CHAIR TO
SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Task Order Change Order Letter#1
for Health Care Program for Children with Special Needs between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Weld County Department of Public Health and Environment, and the Colorado Department of
Public Health and Environment, commencing October 1, 2001, and ending September 30,
2002, with further terms and conditions being as stated in said task order change order letter,
and
WHEREAS, after review, the Board deems it advisable to approve said task order
change order letter, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Task Order Change Order Letter#1 for Health Care Program
for Children with Special Needs between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Weld County
Department of Public Health and Environment, and the Colorado Department of Public Health
and Environment be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said task order change order letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 31st day of July, A.D., 2002, nunc pro tunc October 1, 2001.

Task Order Change Order Letter Number 1 Contract Routing Number O2-F**-G1066
State Fiscal Year 2001-2002, Health Care Program for Children with Special Needs(HCP)
This Task Order Change Order Letter is between the Colorado Department of Public Health and Environment and the
Board of County Commissioners of Weld County. This Task Order Change Order Letter is issued in accordance with
paragraph F.3.of the Master Contract with contract routing number 00 FAA 00008 and paragraph E.16.of the Task Order with
contract routing number 01 FAA 00797 and contract encumbrance number PO FAA HCP0100797. That Task Order has been
amended by Task Order Renewal Letter t,contract routing number 02 FAA 00497. The Task Order,as amended is hereinafter
referred to as the"Original Task Order". A copy of the Original Task Order is incorporated herein by this reference,made a part
hereof,and attached hereto unless a Letter of Compliance has been submitted with this Task Order Change Order Letter. The parties
hereto agree that for the term from October I,2001 through September 30,2002,the maximum amount payable by the
State for the work referenced in part B.of the Original Task Order is increased by Five Hundred Dollars,($500.00)for an
amended total financial obligation of the State to the Contractor of Three Hundred Two Thousand Nine Hundred
Seventy-Seven Dollars,($302,977.00). The revised Budget is incorporated herein by this reference,made a part hereof and
attached hereto as"Attachment 1". The first sentence in part C.,of the Original Task Order is hereby modified accordingly.
All other terms and conditions of the Original Task Order are hereby reaffirmed. The proposed effective date of this Task
Order Change Order Letter is August 15,2002. However,in accordance with section 24-30-202(1),C.R.S., as amended,the
effective date of this Task Order Letter is the date it is approved by the State Controller. In no event shall this Task Order
Change Order Letter be deemed valid until it shall have been approved by the State Controller or such assistants as he may
designate.
Please sign,date,and return all Four(4)originals of this Task Order Change Order Letter by July 31,2002,to the attention of:Peggy

SUBJECT: Task Order Change Order Letter for the
Health Care Program for Children with
Special Needs
Enclosed for Board review and approval is a change order letter to the task order between
Colorado Department of Public Health and Environment and Weld County Department of Public
Health and Environment for the Health Care Program for Children with Special Needs.
This letter provides additional funding in the amount of$500 to provide speech therapy to
children who are eligible for services. Eligible children are those who have or are at risk for
a chronic physical, developmental, behavioral, or emotional condition. With the addition
of the $500, WCDPHE will be paid an amount not to exceed $145,769 for the time period
October 1, 2001 through September 30, 2002.
I recommend your approval of this change order letter.
