HomeMy WebLinkAbout20022391.tiff RESOLUTION RE: APPROVE THREE REVISED NOTIFICATION OF FINANCIAL ASSISTANCE AWARDS FOR VARIOUS PROGRAMS AND AUTHORIZE CHAIR TO SIGN - UNDERWOOD, LLC, DBA UNDERWOOD FAMILY SERVICES WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with three revised Notification of Financial Assistance Awards between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Social Services, and Underwood, LLC, dba Underwood Family Services, commencing June 1, 2002, and ending May 31, 2003, with further terms and conditions being as stated in said revised awards for the following programs, and 1) Option B - Home Based Intensive (Child Protection) 2) Option B - Home Based Intensive (Adolescent) 3) Lifeskills WHEREAS, after review, the Board deems it advisable to approve said revised awards, copies of which are attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, ex-officio Board of Social Services, that the three revised Notification of Financial Assistance Awards for the above listed programs between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Social Services, and Underwood, LLC, dba Family Services, be, and hereby is, approved. Griego, Director, Social Services 4ca((i (Q x( ; 1 J RE: Revision of Notification of Financial Assistance Awards (NOFAAs) Between Weld County Department of Social Services and Underwood Family Services Enclosed for Board approval is a Revision of the Notification of Financial Assistance Awards (NOFAAs) between the Weld County Department of Social Services and Underwood Family Services. The revisions were reviewed at the Board's Work Session of August 26, 2002. The revisions are a result of a 15.75%rate reduction requested by Underwood Family Services beginning September 1, 2002, as follows: A. Option B-Home Based Intensive (Child Protection) from $121.25 per hour to $102.15 per hour. B. Option B-Home Based Intensive (Adolescent) from $121.25 per hour to $102.15 per hour. C. Lifeskills Program from $118.20 per hour to $99.58 per hour. The terms and other provisions of the original NOFAAs remain the same. The terms of the NOFAAs are June 1, 2002 through May 31, 2003.
