RE: APPROVE TASK ORDER CHANGE ORDER LETTER#1 FOR FAMILY PLANNING
PROGRAM AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Task Order Change Order Letter#1 for
the Family Planning Program between the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Weld County Department
of Public Health and Environment, and the Colorado Department of Public Health and
Environment, commencing July 1, 2001, and ending June 30, 2002, with further terms and
conditions being as stated in said task order, and
WHEREAS, after review, the Board deems it advisable to approve said task order, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that Task Order Change Order Letter#1 for the Family Planning
Program between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the Colorado Department of Public Health and Environment be, and hereby
is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said task order.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 11th day of February, A.D., 2002, nunc pro tunc
July 1, 2001.

SUBJECT: Task Order Change Order Letter for Family
Planning Program
Enclosed for Board review and approval is Change Order Letter Number One to the Task Order
for the Family Planning Program. If approved, this change order letter will become part of the
Master Contract between the Colorado Department of Public Health and Environment and Weld
County Board of Commissioners.
This change order letter will increase funding for the Family Planning Program by $20,250 for
the provision of eligible services to approximately 1,370 target program clients plus 250
alternative site clients. With this additional funding, the total financial obligation of the State for
the time period July 1, 2001 through June 30, 2002 will be $117,763.
I recommend your approval of this change order letter.

Pursuant to paragraph C3 of the Task Order with contract routing number FLA 02-00026 and contract encumbrance number PO
FLA FPP02-00026,hereinafter referred to as the"Original Task Order"(a copy of which is attached hereto and by this reference
incorporated herein and made a part hereof)between the State of Colorado, Department of Public Health and Environment and
BOARD OF COUNTY COMMISIONERS OF WELD COUNTY,(a political subdivision of the state of Colorado), for the use
and benefit of WELD COUNTY PUBLIC HEALTH DEPARTMENT,for the term from 7-1-01,through 630-02, the parties agree
that the maximum amount payable by the State for the eligible services referenced in paragraph Cl of the Original Task Order is
increased by TWENTY THOUSAND TWO HUNDRED FIFTY DOLLARS, ($20,250.00)for a new total financial obligation of
the State of ONE HUNDRED SEVENTEEN THOUSAND SEVEN HUNDRED SIXTY THREE DOLLARS,($117,763.00).
This additional funding is state funding which must be spent by 6-30-02 to provide services by the Contractor for a total of
approximately 1370 target program clients+250 state alternative site clients.
The first sentence in paragraph 1 of the Original Task Order is hereby modified accordingly. All other terms and conditions of the
Original Task Order are hereby reaffirmed. The proposed effective date is February 7,2002. In accordance with section 24-30-
202(l),C.R.S., as amended,the effective date of this amendment to the original Task Order is the date the State Controller approves this
contract. In no event shall this amendment be deemed valid until it shall have been approved by the State Controller or such assistant as
he may designate.
Please sign,date,and return all 4 originals of this Task Order Change Order Letter as soon as possible to: Laura Montoya,Financial
Specialist, Women's Health Section, Colorado Department of Public Health and Environment,4300 Cherry Creek Drive South, 5th
Floor, Denver, Colorado 80246-1530. One original of this Task Order Change Order Letter will be retumed to you when fully
approved.

Re: Family Planning Contracts for Expansion/Alternative Site Projects
Enclosed is the contract or task order change letter for your federal expansion or state
alternative site project, which has been signed by all parties. Please include this in your
file. When the original has been directed to the Administrative Director, a copy has been
sent to the Family Planning Coordinator. Family Planning Coordinators will need to
notify the appropriate financial staff at their agencies when they receive the original
signed contract.

date for this contract. (� 7
Reimbursement for these expansion projects may be requested monthly and must be
clearly marked for the expansion or alternative site project. You may indicate the amount
billed for the expansion or alternative site project as a separate line in your regular
monthly family planning billing or send us a separate statement as you prefer. The state
alternative site project funding ends June 30, 2002 and therefore we will utilize this
funding first if possible when we pay your reimbursements.
