RE: APPROVE APPLICATION FOR PLATTE VALLEY MEDICAL CENTER COMMUNITY
HEALTH INVESTMENT PROGRAM - PLATTE VALLEY MEDICAL CENTER
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Application for Platte Valley Medical
Center Community Health Investment Program between the County of Weld, State of Colorado,
by and through the Board of County Commissioners of Weld County, on behalf of the
Department of Human Services, Area Agency on Aging, and the Platte Valley Medical Center,
Community Health Investment Program Committee, with further terms and conditions being as
stated in said application, and
WHEREAS, after review, the Board deems it advisable to approve said application, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Application for Platte Valley Medical Center Community Health
Investment Program between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Human Services,
Area Agency on Aging, and the Platte Valley Medical Center, Community Health Investment
Program Committee, be, and hereby is, approved.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 24th day of June, A.D., 2002.

SUBJECT: Weld County Area Agency on Aging Application
for Dental, Vision and Hearing Program Funds.
Enclosed for Board approval is the Area Agency on Aging request for additional funding
of $10,000.00 for the Dental, Vision and Hearing Aid Programs from Platte Valley
Medical Center - Community Health Investment Program.
The funding will be used to continue dental, vision and hearing aid services to seniors
60 years of age and older who live in South County.

Hopefully, we will continue to receive funding through the State Grant.
This application, with at least one additional proposal page, and a copy of the following must be submitted
to the CHIP Committee:
1) Names and addresses of applicant officers and members of its Board of Directors or governing body.
2) Articles of Incorporation and By-laws or other documents which define the applicant's purposes, membership,
management and operation.
3) Statute, regulation or the applicant's Internal Revenue Service letter which shows the applicant is exempt from
Federal income taxes, is not a private foundation and that donors may deduct contributions to the applicant as
provided in Section 170 of the Internal Revenue Code.
4)A financial audit or other financial information for the most recent three fiscal years.
5)A detailed budget for the proposal.
All applications must be submitted to the CHIP Committee no later than the month ending previous
to scheduled CHIP Committee meetings (The Committee meets the second Wednesday every other
month as follows: January, March, May, July, September and November).
PROPOSAL
Weld County Human Services houses the Employment Services of Weld County, Family
Education Network, Transportation Program,Area Agency on Aging and the Commodity
Supplemental Food Program. Our Area Agency on Aging Program provides Weld County
residents 60 years of age and older, supportive services of Adult Care, Carelink, Home Delivered
Meals, Legal Assistance, Peer Counseling, Rural Senior Centers, Senior Nutrition Education and
Hispanic Outreach. These services and contacts with community agencies help our Weld County
Seniors maintain their individual independence.
For the past two (2) years Weld County Area Agency on Aging has received funding from
House Bill 1072 and 1079 as well as the Long Bills. Weld County has provided several new
services to our seniors with these funds. The services we now can offer our seniors are Personal
Home Care, Extended Legal Services, Outreach such as Loneliness and Grandparents Raising
Grandchildren, Senior Center Operations, Extended Meal Sites, and our Dental, Optical and
Hearing Services.
Weld County's Area Agency on Aging is finding that there is an increasing medical need for
Seniors of South Weld County. We currently are observing that the expenditures for these
services are increasing as well as the demand. During the increase of expenditures and demands
House Bill 1209 and Long Bill have been decreased significantly. We are requesting community
support to help provide these services to the seniors in need and continue our goal of providing
these seniors with the ability to maintain their individual independence.
DOCUMENTED NEED:
The Weld County Area Agency on Aging implemented a Dental, Vision and Hearing Aid
Program with funding received from a special allocation from the State of Colorado to the
Colorado Area Agencies on Aging. The special funding was set aside to assist seniors in our
communities with services they were not receiving from any other agencies. The Weld County
Area Agency on Aging decided that funding dental, vision and hearing aid programs in our
community would be beneficial to seniors. Our agency was not aware of the tremendous impact
it would make. Without any official advertising of our dental, vision and hearing programs, we
assisted a total of 105 seniors in the two year period (this does not include the approximate 25
that have been put on a waiting list because of lack of additional funding). The majority of the
dollars spent went to the dental program with sixty-five (65) seniors provided with assistance in
North Weld County and seventeen (17) provided with assistance in South Weld County.
Seventeen (17) seniors received vision assistance, with thirteen(13) provided assistance in North
Weld County and five (5)provided assistance in South Weld County. Four(4) seniors were
provided with hearing aid assistance in North Weld County and one (1) was provided with
assistance in South Weld County. The majority of the seniors assisted in South Weld County
were from the Fort Lupton area. The seniors that received assistance had been turned down by
other programs for various reasons.
EXPECTED RESULTS OF THE PROJECT:
With two (2)years of service already provided, we have seen the results that the programs can
bring to our communities. The seniors that have received assistance had no other resources
available to them for these services. With continued funding we believe we will be able to assist
more seniors who are in need of dental, vision and hearing services Weld County.
SPECIFIC OBJECTIVES, TIMETABLES AND PROCESS UTILIZED TO RESEARCH
PROJECT GOALS:
Because the funding is being provided by the State of Colorado, we had to keep records of the
seniors we served and the total cost expended for the program. We set up an application process
similar to the one required for our federal nutrition program. The application compares the
seniors income with their monthly expenses. The data collected from the application and the
authorization for service form are all entered into a program that allows for tracking of the
services provided. The State funding so far has been on a yearly basis, so our timetable has been
to spend the funding in the current program year. The goals for the program have been to assist
as many of the seniors applying for assistance as possible. The seniors that are currently on our
waiting list are there because we did not receive enough funding to satisfy the demand. We had
no idea the program would grow so fast so soon. This growth has happened without any outside
advertising of the program. We have been working with Sunrise Health Clinic in Greeley as well
as the Salud Health Clinics in Fort Lupton and Frederick. Many of the referrals came from these
clinics.
HOW WILL RESULTS BE MEASURED?
The results will be measured by the number of seniors who have been assisted and the
satisfaction of those that have received assistance. We have encouraged the seniors that have
been assisted to visit their dentists on a regular basis to avoid any potential problems in the
future.
The Program Years of 2000-2001 & 2001-2002, have shown a definite need in Weld County for
Dental, Vision, and Hearing aid assistance. We are requesting funding from the Community
Health Investment Program to assist those seniors in South County. This request has been made
necessary because funding from the state has been decreased because of the current state budget
problems. The requested funding will be used only for those eligible seniors in South County.
The funding, if approved, would continue a service in South County that has not been present
before. We will still coordinate with the Old Age Pension Dental Program, Friends of Man and
the A.V. Hunter Trust. Funding received from the State Grant would then be used to satisfy the
needs of the remaining eligible seniors in Weld County. In the two years the program has been in
operation we have tried to work with dentists that have shown an interest in seniors. After two
years we have a good understanding of where the best deal will be offered and the savings it will
bring to the programs. By utilizing these savings we have been able to assist even more seniors.
