RE: APPROVE RENEWAL AGREEMENT FOR SATELLITE ELIGIBILITY
DETERMINATION SITE AND AUTHORIZE CHAIR TO SIGN - CHILD HEALTH
ADVOCATES/POLICY STUDIES, INC
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Renewal Agreement for a Satellite
Determination Site for Child Health Plan Plus between the County of Weld, State of Colorado,
by and through the Board of County Commissioners of Weld County, on behalf of the
Department of Public Health and Environment, and the Child Health Advocates, operated by
Policy Studies, Inc., commencing July 1, 2002, and ending December 31, 2002, with further
terms and conditions being as stated in said renewal agreement, and
WHEREAS, after review, the Board deems it advisable to approve said renewal
agreement, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Renewal Agreement for a Satellite Determination Site for Child
Health Plan Plus between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Department of Public Health and
Environment, and the Child Health Advocates, operated by Policy Studies, Inc., be, and hereby
is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said agreement.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 1st day of July, A.D., 2002.

SUBJ: Renewal Agreement
A. In accordance with Paragraph V. A of Memorandum of Understanding(MOU),between the
Child Health Advocates,operated by Policy Studies Inc. (CHARS!)and
Weld County Department of Health&Environment
covering the period beginning upon signature date through June 30,2002 the parties hereby
renew the contract for an additional six month's performance period,effective July 1,2002
to December 31,2002.
B. The Parties acknowledge and agree that all terms and conditions set forth in MOU
regarding SED obligations and reimbursement,shall continue and apply for the renewal
period.The SED Site shall meet and maintain the required efficiency benchmarks as
established in the MOU.
C. The Parties acknowledge and agree that the obligations of Child Health Advocates operated
by Policy Studies Inc. (CHA/PSI)shall continue and be limited to as established in the MOU.

SUBJECT: Addendum to SED Site Memorandum of
Understanding
Enclosed for Board review and approval is an addendum to the current SED (Satellite Eligibility
Determination Site) for the Child Health Plan Plus (CHP+). This plan provides for outpatient
and inpatient medical services to children under age nineteen in medically indigent families. As
a Satellite Eligibility Determination Site, WCDPHE has been 1) assisting clients in completing
their applications for this insurance plan; 2) has determined eligibility for applicants; and 3) has
submitted each application with required documentation to the Child Health Advocates.
For these services, WCDPHE has been paid $10.00 for each completed application submitted
and an additional $20.00 per application for eligibility determination for each correctly
determined application submitted. This addendum to the MOU will continue this agreement for
the time period July 1, 2002 through December 31, 2002.
