RE: APPROVE FOUR REVISED NOTIFICATION OF FINANCIAL ASSISTANCE AWARDS
FOR FOSTER PARENT CONSULTATION WITH VARIOUS VENDORS AND
AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with four revised Notification of Financial
Assistance Awards for Foster Parent Consultation between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Department of Social Services, and the following vendors, commencing November 1, 2002, and
ending May 31, 2003, with further terms and conditions being as stated in said revised awards,
and
1) Ackerman and Associates, P.C.
2) Lutheran Family Services
3) Lori Kochevar
4) North Range Behavioral Health, and
WHEREAS, after review, the Board deems it advisable to approve said revised awards,
copies of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, ex-officio Board of Social Services, that the four revised Notification of
Financial Assistance Awards for the above listed programs between the County of Weld, State
of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of
the Department of Social Services, and the above listed vendors, be, and hereby are,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said awards.

RE: Revision of Notification of Financial Assistance Awards under
Core Services with Various Vendors
Enclosed for Board approval is a revision of Notification of Financial Assistance Awards
(NOFAAs) under Core Services for Foster Parent Consultation Programs. The four
vendors affected are: Ackerman and Associates, P.C., Lutheran Family Services, Lori
Kochevar, and North Range Behavioral Health.The Foster Parent Consultation Program
provides support and consultation for County Certified Foster Parents as well as
certification training. The revised NOFAAs were discussed and reviewed at the Board's
Work Session of November 18, 2002.
The revised NOFAA decreases the number of one-on-one consultation sessions provided
by the vendor unless specifically approved by the Department and increases the use of
foster parent group sessions with a maximum group rate of$300 for a minimum of 8
foster parents.
The revised NOFAA would become effective November 1, 2002, and for the remaining
period of the NOFAA ending May 31, 2003. All other aspects of the NOFAA remain the
same.
