RE: APPROVE TASK ORDER RENEWAL LETTER#2 FOR TUBERCULOSIS CONTROL
AND OUTREACH PROGRAM AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Task Order Renewal Letter#2 for the
Tuberculosis Control and Outreach Program between the County of Weld, State of Colorado,
by and through the Board of County Commissioners of Weld County, on behalf of the Weld
County Department of Public Health and Environment, and the Colorado Department of Public
Health and Environment, commencing July 1, 2002, and ending June 30, 2003, with further
terms and conditions being as stated in Task Order Renewal Letter#2, and
WHEREAS, after review, the Board deems it advisable to approve Task Order Renewal
Letter#2, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that Task Order Renewal Letter#2 for the Tuberculosis Control and
Outreach Program between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Weld County Department of Public
Health and Environment, and the Colorado Department of Public Health and Environment be,
and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign Task Order Renewal Letter#2.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 22nd day of May, A.D., 2002.

State Fiscal Year 2002 -2003, Task Order Renewal Letter Number 02, Contract Routing Number 03-00036
Pursuant to Part F.5.of the Master Contract with contract routing number 00-00008 and paragraph E,2 of the Task Order with
contract routing number 01-00158 and contract encumbrance number PO FHA EPI0100158,(as amended by Task Order
Renewal Letter number 01 contract routing number 02-00036;Task Order Change Order Letter number 01,contract routing
number 02-00156) hereinafter referred to as the "Original Task Order" (a copy of which is attached hereto and by this
reference incorporated herein and made a part hereof)between the State of Colorado,Department of Public Health and
Environment and the Board Of County Commissioners Of Weld County,for the use and benefit of, the Weld County
Department of Public Health and Environment the parties agree that the maximum amount payable by the State for the
eligible services referenced in paragraph B of the Original Task Order for the renewal term from July 1,2002,through June
30,2003,is increased by NINTY ONE THOUSAND FOUR HUNDRED THIRTY FIVE DOLLARS, ($91,435.00); and,
that the Blanket Encumbrance amount in Section C,2.of the Original Task Order is decreased to TWENTY TWO
THOUSAND EIGHT HUNDRED FIFTY DOLLARS,1$22,850.00)for Statewide Tuberculosis Direct Observed
Therapy for a new total financial obligation of the State of ONE HUNDRED NINTY ONE THOUSAND TWO
HUNDRED THIRTY DOLLARS,($191,230.00)in accordance with the budget outlined below. The first sentence in
paragraph C,1,of the Original Task Order is hereby modified accordingly. All other terms and conditions of the Original Task
Order are hereby reaffirmed. In accordance with section 24-30-202(1),C.R.S., as amended,the effective date of this
amendment is the date the State Controller approves this amendment.

SUBJECT: Task Order Renewal Letter for the
Tuberculosis Program
Enclosed for Board review and approval is Task Order Renewal Letter#2 between the Colorado
Department of Public Health and Environment (CDPHE) and Weld County for the Tuberculosis
Control and Outreach Program.
Under the provisions of this renewal letter, WCDPHE will provide TB treatment, TB preventive
treatment, provide for and interpret chest x-rays and other medical evaluation services, and
conduct contact investigations and TB skin test screening programs. WCDPHE will also follow
up with active patients to ensure they receive appropriate information and education and they
adhere to the completion of their TB therapy.
For these services, WCDPHE will receive an amount not to exceed $91,435 for TB control and
outreach activities, chest x-rays, and medical consultation plus $12.50 per visit for directly
observed therapy for the time period Julyl, 2002 through June 30, 2003.
I recommend your approval of this task order renewal letter.
