RE: APPROVE TASK ORDER RENEWAL LETTER #1 FOR FAMILY PLANNING
PROGRAM AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Task Order Renewal Letter#1 for the
Family Planning Program between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Weld County Department of
Public Health and Environment, and the Colorado Department of Public Health and
Environment, commencing July 1, 2002, and ending June 30, 2003, with further terms and
conditions being as stated in said task order, and
WHEREAS, after review, the Board deems it advisable to approve said task order, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that Task Order Renewanl Letter#1 for the Family Planning Program
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the Colorado Department of Public Health and Environment be, and hereby
is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said task order.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 22nd day of May, A.D., 2002.

This Task Order Renewal Letter is between the COLORADO DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT and the BOARD
OF COUNTY COMMISIONERS OF WELD COUNTY(a political subdivision of the state of Colorado),for the use and benefit of Weld
County Department of Public Health and Environment. This Task Order Renewal Letter is issued in accordance with paragraph F.4.Of the Master
Contract with contract routing number FAA 00-00008 and,paragraph C4 of the Task Order with contract routing number FLA 02-00026 and
contract encumbrance number PO FLA FPP02-00026. That Task Order has been amended by Task Order Change Letter#1,routing number FLA 02-
00262.The Task Order,as amended,is hereinafter referred to as the"Original Task Order". A copy of the Original Contract is incorporated herein by
this reference,made a part hereof and attached hereto unless a Letter of Compliance has been submitted with this Task Order Renewal Letter.
The Parties hereto agree that the Original Task Order is renewed for the term from July 01,2002 through June 30,2003. The maximum amount
payable by the State for the continued performance of the work referenced in Part B.of the Original Task Order for this renewed term is
increased by ONE HUNDRED TWENTY EIGHT THOUSAND SEVEN HUNDRED TWENTY FOUR DOLLARS,($128,724.00).The
amended total financial obligation of the State to the Contractor is TWO HUNDRED FORTY SIX THOUSAND FOUR HUNDRED
EIGHTY SEVEN DOLLARS, ($246,487.00).
Title X federal funding from the United States government provides $79,442.00 for routine family planning(to be billed at$13.240.33 a month,
July through December). Additionally,a funding source from the State of Colorado provides$49,282.00 for routine family planning(to be billed
at$8,213.67a month,January through June). Family planning services will be provided by the Contractor for approximately 1,585 target clients.
The first sentence in paragraph C.1,of the Original Task Order is hereby modified accordingly. All other terms and conditions of the Original Task
Order are hereby reaffirmed. The proposed effective date of this Task Order Renewal Letter is July 01,2002. However,in accordance with section
24-30-202(1),C.R.S.,as amended,the effective date of this Task Order Renewal Letter is the date it is approved by the State Controller.In no event
shall this Task Order Renewal Letter be deemed valid until it shall have been approved by the State Controller or such assistant as he may designate.

SUBJECT: Task Order Renewal Letter for the Family
Planning Program
Enclosed for Board review and approval is Task Order Renewal Letter#1 between the Colorado
Department of Public Health and Environment (CDPHE) and Weld County for the Family
Planning Program.
Under the provisions of this renewal letter, WCDPHE will be reimbursed an amount not to
exceed $128,724.00 for the time period July 1, 2002 through June 30, 2003 to provide
contraceptive services to both men and women in their reproductive years. These services will
include education and counseling, comprehensive health and social histories. physical
examinations, supplies, follow-up and/or referral services.
