RE: APPROVE TEN PURCHASE OF SERVICES AGREEMENTS WITH VARIOUS SENIOR
CENTERS FOR OUTREACH ACTIVITIES AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with ten Purchase of Services Agreements
for outreach activities between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Department of Human
Services, Area Agency on Aging, and the following senior centers, with further terms and
conditions being as stated in said agreements:
1) Erie Senior Citizens
2) Evans Senior Center
3) Fort Lupton Community Center
4) Grover Senior Citizens
5) Hill-N-Park Senior Citizens
6) Johnstown Community Center
7) Kersey Senior Center
8) Nunn Senior Citizens
9) Pierce Senior Center
10) Tri-Town Senior Center
WHEREAS, after review, the Board deems it advisable to approve said agreements,
copies of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the ten Purchase of Services Agreements for outreach activities
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services, Area Agency
on Aging, and the above listed senior centers be, and hereby are, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said agreements.

THIS AGREEMENT, made and entered into this lstday of November, 2002 by and between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Division
of Human Services'Area Agency on Aging,hereinafter referred to as "AAA" and the Tri-Town Senior Center hereinafter
referred to as "Tri-Town."
WITNESSETH:
WHEREAS, "AAA" develops and administers the Area Plan for a comprehensive and coordinated system of services,
and
WHEREAS, "AAA" considers senior centers a vital link in the county's system of services to older adults, and
WHEREAS, "Tri-Town"is a senior center providing recreational,nutritional, and educational opportunities for senior
citizens; and
WHEREAS, "Tri-Town"proposes to implement a "The Tri-Town Loneliness"program to reach out to area seniors;
NOW THEREFORE, it is mutually agreed:
1. The "AAA"hereby agrees to award grant monies to"Tri-Town" in the sum of 5350.00.
2. "Tri-Town" agrees to develop a volunteer committee that will reach out to homebound and isolated seniors and
encourage senior center involvement. Volunteers will also send cards, letter and flyers to isolated seniors.
3. "Tri-Town"agrees to document contacts made. "Tri-Town"understands that the$350.00 may be used to pay for
the cost of the outreach program and to advance the Tri-Town Senior Program.
4. "Tri-Town"understands this is a one time only award and the terms of this agreement are valid through June 30,
2003.

WHEREAS,"Erie"is a senior program providing recreational, nutritional, and educational opportunities for senior
citizens; and
WHEREAS, "Erie"proposes to implement a "Connect to the Community"program to reach out to seniors in their
community to involve them in the social activities at the Erie Senior Program;
NOW THEREFORE, it is mutually agreed:
1. The"AAA"hereby agrees to award grant monies to the "Erie"in the sum of$350.00.
2. "Erie" agrees to continue working with community service groups to recruit volunteers who will visit lonely seniors
and initiate social activities. "Erie"will also send out birthday cards and other appropriate cards to seniors and
provide lunch coupons to seniors to encourage participation in the congregate meal program. "Erie" will participate
in video taping special events that will be broadcast on local cable television in an effort to reach isolated seniors.
3. "Erie" agrees to document contacts made. Erie Senior Citizens understand that the grant of$350.00 may be used to
implement the "Connect to the Community"and to advance the overall Erie senior recreational program.
4. "Erie" understands this is a one time only award and the terms of this agreement are valid through June 30, 2003.

WHEREAS, "Evans" is a senior center providing recreational,nutritional, and educational opportunities for senior

WHEREAS, "Evans"proposes to implement a "Project RX"to reach out to area seniors;
NOW THEREFORE, it is mutually agreed:
1. The "AAA"hereby agrees to award grant monies to "Evans" in the sum of$350.00
2. "Evans"agrees to provide information regarding affordable pharmaceutical prescriptions options available to senior
through one on one contact. "Evans"will use the pharmaceutical outreach program as an opportunity to actively
encourage isolated and lonely seniors to become involvement at the senior center.
3. "Evans" agrees to document the results of this outreach effort on a monthly bases. "Evans"understands that the
grant of$350.00 may be used to pay for the cost of the outreach program.

Center hereinafter referred to as "Fort Lupton."
WITNESSETH:
WHEREAS, "AAA"develops and administers the Area Plan for a comprehensive and coordinated system of services,
and
WHEREAS, "AAA"considers senior centers a vital link in the county's system of services to older adults, and
WHEREAS, "Fort Lupton" is a senior center providing recreational, nutritional, and educational opportunities for senior
citizens; and
WHEREAS, "Fort Lupton"proposes to implement a "Operation Connect" to reach out to area seniors;
NOW THEREFORE, it is mutually agreed:
1. The "AAA"hereby agrees to award grant monies to "Fort Lupton" in the sum of$350.00.
2. "Fort Lupton"agrees to mail birthday and holiday greetings to approximately 1000 local seniors. Birthday
greetings and special holiday mailing will be sent to seniors to encourage participation in the meal program and
other senior activities at the recreation center.
3. "Ft. Lupton"agrees to document contacts made. "Ft. Lupton" understand that the grant of$350.00 may be used to
implement the "Operation Connect"and to advance the overall senior recreational program.
4. "Fort Lupton"understands this is a one time only award and the terms of the agreement are valid through June 30,
2003.

hereinafter referred to as "Grover"
WITNESSETH:
WHEREAS, "AAA" develops and administers the Area Plan for a comprehensive and coordinated system of services,
and
WHEREAS, "AAA" considers senior centers a vital link to seniors who are lonely and isolated, and
WHEREAS, "Grover" is a program providing recreational, nutritional, and educational opportunities for senior citizens,
and
WHEREAS, "Grover"proposes to implement a "The Gift of Giving all Year Long"program to reach out to seniors in the
Grover area to involve them in the social activities at the Grover Senior Center;
NOW THEREFORE, it is mutually agreed:
1. The "AAA"hereby agrees to award grant monies to the "Grover" in the sum of$350.00.
2. "Grover" agrees to recruit volunteers who will contact homebound seniors and new seniors to the area and invite
them to join the Grover Senior Center. Monthly birthday parties and other events will be provided to draw in new
members. "Grover"will design a monthly new letter that will be sent to all Grover seniors to provide information
regarding center happenings and services.
3. "Grover" agrees to document contacts made. "Grover"understands that the grant of$350.00 may be used to
implement the "The Gift of Giving all Year Long"and to advance the overall senior recreational program.
4. "Grover" understands this is a one time only incentive award and the terms of this agreement are valid through June
30, 2003.

2. "Hill N Park" agrees to implement "S.O.S", an outreach project that would involve contacting seniors in the Hill N
Park community and encouraging involvement at the center. Guest meal passes will be offered and information
flyers advertizing senior center events will be mailed to community seniors.
3. "Hill N Park"agrees to document the results of this outreach effort. "Hill N Park"understands that the grant of
$350.00 may be used to pay for the cost of the outreach program and to advance the overall "Hill N Park" senior
citizens program.
4. "Hill N Park" understands this is a one time only award and the terms of this agreement are valid through June 30,
2003.

Johnstown area to involve them in the social activities at the Johnstown Community Center;

2. "Johnstown"agrees to implement"Johnstown Senior Outreach",an outreach project that would involve the
development of two volunteer committees that will reach out to isolated or homebound seniors. Special
programs such as ice cream socials and other social outreach events will be implemented. Local media options
will be used to advertise events and provide service information.
3. "Johnstown"agrees to document the results of this outreach effort on a monthly bases. "Johnstown"
understands that the grant of$350.00 may be used to pay for the cost of the outreach program and to advance
the overall"Johnstown"senior program.
4. "Johnstown"understands this is a one time only award and the terms of this agreement are valid through June
30, 2003.

hereinafter referred to as "Kersey."

WHEREAS, "AAA" considers senior centers a vital link in the county's system of services to older adults, and
WHEREAS, "Kersey"is a senior center providing recreational, nutritional, and educational opportunities for senior
citizens; and
WHEREAS, "Kersey"proposes to implement a "Reach Out"program to find homebound seniors in their community and
make a difference in their lives;
NOW THEREFORE, it is mutually agreed:
1. The "AAA"hereby agrees to award grant monies to "Kersey" in the sum of$350.00.
2. "Kersey"agrees to identify homebound seniors in the Kersey area and make home visits that will include a welcome
gifts and a coupon to attend the congregate meal program. The goal is to reach forty (40) seniors during the contract
period.
3. "Kersey" agrees to document contacts made. "Kersey" understands that the $350.00 may be used to pay for the
cost of the outreach program and to advance the overall senior program.
4. "Kersey"understands this is a one time only award and the terms of the agreement are valid through June 30, 2003.

WHEREAS, "Nunn"proposes to implement a "Building Bridges of Friendship Program"to reach out to seniors in the
Nunn and Can-areas to involve them in the social activities at the Nunn Senior Center;
NOW THEREFORE, it is mutually agreed:
1. The"AAA" hereby agrees to award grant monies to the"Nunn" in the sum of$350.00.
2. "Nunn" agrees to recruit volunteers who will contact homebound seniors,new seniors to the area, to increase phone
calls and mailings, and to help with shopping and transportation appointments for seniors.
3. "Nunn"agrees to document activities and results of this outreach effort. Nunn Senior Citizens understand that the
grant of$350.00 may be used to pay for the costs of the outreach program and to advance the overall Nunn Senior
Citizens Program.
4. "Nunn"understands this is a one time only incentive award and the terms of this agreement are valid through June
30, 2003.

2. "Pierce" agrees to recruit volunteers who will contact homebound seniors and providing home delivered meals and
chore services when needed. "Pierce" will also reach out to new community members and encourage involvement at
the center.
3. "Pierce" agrees to document activities and results of this outreach effort. "Pierce"understands that the grant of
S350.00 may be used to pay for the costs of the outreach program and to advance the overall Pierce Senior Center
Program.
4. "Pierce" understands this is a one time only award and the terms of this agreement are valid through June 30, 2003.

COLORADO SUBJECT: Purchase of Services Agreements with Rural
Weld County Senior Centers
Enclosed for Board approval are ten (10) Purchase of Service agreements with Weld County
Senior Centers to purchase outreach activities. These grants are in response to needs data which
shows that a high percentage of Weld seniors are lonely and isolated from community life. The
senior centers listed below have agreed to redouble their efforts to reach out to these seniors by
implementing telephone reassurance calls, one on one visitations, and buddy programs to attract
new seniors to the centers and a variety of other outreach activities. Grants will be given tothe
following
