HomeMy WebLinkAbout20023260 RESOLUTION RE: APPROVE FUNDING AGREEMENT FOR AREA AGENCY ON AGING COMMUNITY FAMILY SUPPORT AND AUTHORIZE CHAIR TO SIGN - UNITED WAY OF WELD COUNTY, ON BEHALF OF PROMISES FOR CHILDREN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with a Funding Agreement for Community Family Support between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, Area Agency on Aging, on behalf of Weld County Grandparents Raising Grandchildren Task Force, and United Way of Weld County, on behalf of Promises for Children, commencing upon full execution of said funding agreement, and ending October 30, 2003, with further terms and conditions being as stated in said funding agreement, and WHEREAS, after review, the Board deems it advisable to approve said funding agreement, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Funding Agreement for Community Family Support between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, Area Agency on Aging, on behalf of Weld County Grandparents Raising Grandchildren Task Force, and United Way of Weld County, on behalf of Promises for Children, be, and hereby is, approved. Masden Date of signature: 1/7/03 2002-3260 HR0073 PROMISES FOR CHILDREN COMMUNITY FAMILY SUPPORT FUNDING AGREEMENT An agreement between United Way of Weld County on behalf of Promises for Children hereinafter referred to as Promises and Weld County Area on Aging on behalf of Weld County Grandparenting Raising Grandchildren hereinafter referred to as the Agency. 1. Agency is and shall remain a charitable 501(C)3 non-profit organization or government entity. 2. Agency agrees to be accountable for the funds provided and that such funds will be used for the purpose stated in the grant award letter. The Agency agrees to provide a progress report by June 30, 2003 and a final report providing a full accounting of how the funds were used and the number of children and families served at the end of the project or when funds have been totally expended but no later than October 30, 2003. 3. Agency agrees to abide by the Community Family Support Funding Guidelines (attached). 4. Agency agrees to work with the Community Family Support Committee to where possible to replicate family support programs to other communities in Weld County. 5. Agency agrees to give recognition to Promises for Children for this financial support. 6. Promises agrees to work with Agency to publicize the funded program. 7. Promises agrees to be available to Agency for advice and consultation regarding the funded service support. 8. Promises agrees to work with the Agency to strengthen Family Support in Weld County. Box 1805 Greeley, CO 80632 Dear Dorothy: On behalf of Promises for Children Community Family Support Committee, we are pleased to inform you that the Coordinating Council of Promises has approved a grant of$300 to Area Agency on Aging on behalf of Weld County Grandparenting Raising Grandchildren Task Force to provide child care for your legal clinics and community resource fair at the Rodarte Center and in Fort Lupton. Your proposal was one of four programs receiving funding through our recent Request for Proposal process. Promises for Children feels your program met our criteria and exemplifies the kind of services we hope not only to support but expand for Weld County families. Enclosed please find a one page agreement for this funding. Please sign and return one copy to United Way of Weld County, keep the other for your records. As soon as we have the returned agreement, we will send you a check for the full amount of your award. Promises for Children looks forward to working with you this year. We will be inviting you to a Community Family Support Meeting during the second quarter of 2003. We will want to hear about your progress, expansion plans and how we can better work together. Programs funded must have basic policies and procedures in place to provide adequate accountability and assure efficient and effective use of funds provided by Promises for Children. 2. Programs funded must be community based with strong volunteer involvement. 3. Funds must be used to increase service not as replacement funding for existing services. 4. If program is faith-based the following principles apply: a. Faith-based providers are eligible and encouraged to provide family support. b. Promises will respect the religious character of the faith-based provider c. Faith-based providers may not discriminate against an individual on the basis of religion, nor require religious affiliation or belief as a requirement of receiving services provided with Promises funding. d. Promises funds can only be used to provide the community family support services and not religious activities such as worship, sectarian instruction and proselytizing. 5. Programs will be operating under sound family support practices, such as the following: a. Programs affirm and strengthen families' cultural, racial and linguistic identities and enhance their ability to function in a multicultural society. b. Programs are involved in their communities and contribute to the community-building process. c. Programs work with families to mobilize formal and informal resources to support family development. d.
