RE: APPROVE SERVICE CONTRACT PROPOSAL ACCEPTANCE FORM FOR
RECABLING VARIOUS COUNTY OFFICES AND AUTHORIZE CHAIR TO SIGN -
H AND H DATA SERVICES, INC.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Service Contract Proposal
Acceptance Form for Recabling Various County Offices between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Phone Services Department, and H and H Data Services, Inc., with terms and conditions being
as stated in said contract, and
WHEREAS, after review, the Board deems it advisable to approve said contract, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Service Contract Proposal Acceptance Form for Recabling
Various County Offices between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Phone Services Department,
and H and H Data Services, Inc., be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said contract.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 18th day of November, A.D., 2002.

Demo existing cables after cut-over

Materials: $ 25,947.56
Labor: $ 10,781.00
Total: $ 36,728.56
Payment Terms:
Invoices will be progressively invoiced as material is delivered and as work is completed.
All invoices will be paid Net 30.
Any alteration or deviation from this proposal involving extra costs will be executed only
upon receipt of written orders and will be an extra charge, over and above this proposal.
All agreements are contingent on strikes, accidents or delays beyond our control. Owner
to carry fire and tornado and other necessary insurance. All H&H Data Service personnel
are covered by Workmen's Compensation Insurance.
Authorization of Proposal:
All material is guaranteed to be as specified. All work to be completed in a substantial
workmanlike manner according to the specifications submitted in this proposal and is
guaranteed to be free of defects for one (1) year unless covered by an extended
manufactures warranty.
This pro osal may be withdrawn if not accepted within 90 days.
