payment including where and to whom should CHSDA send the funds to for your department to

On behalf of CHSDA,we are pleased to inform you that Weld has been selected as an award recipient
for a CHSDA mini-grant utilizing our Casey Family Programs funding!

The total amount we are able to award Weld is:$1,500.
This award is intended to support your county's efforts as outlined in your application (attached)to
support FYiT phone, housing application, and transportation expenses.
Because this is such a small amount and isn't the full amount you requested, please let us know if
you would prefer to decline the funding.We will require a simple reporting update on how funds are
spent in alignment with your proposed project.
If you would like to accept the funding, please send Emily and I details asap for where CHSDA
should send the check(payee name, mailing address, any other department information to
receive the funds).We have to have finance staff at Larimer County(our CHSDA treasurer) send out a
check tomorrow.
