I'm writing to you because Contract 8950\CARELON BEHAVIORAL HEALTH PROVIDER PARTICIPATION
AGREEMENT with Weld County Department of Human Services is ready for renewal. We no longer need
this agreement because Carelon is no longer the Administrative Service Organization for Northeast
Health Partners, so an agreement with Carelon is no longer necessary. Please accept this email as
confirmation that we are terminating the agreement.
