RE: AMBULANCE BILLING SERVICE CONTRACT
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado Statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, Weld County has a need for an automated accounts receivable
billing system for the Weld County Ambulance Service and
WHEREAS, a review of the services available to Weld County by the
Director of Finance and Administration and Director of Weld County Ambulance
Service indicates that Data Control Systems is a sole provider in the service
area for an ambulance accounts receivable billing system and
WHEREAS, the rates charged for the accounts receivable billing system are
less expensive than Weld County can do it in its own computer center,
therefore the most cost effective alternative and
WHEREAS, a service contract, attached hereto and incorporate herein by
reference, has been presented to the Board of County Commissioners for
accounts receivable billing for the Weld County Ambulance Service.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, that they hereby authorize the Chairman of the Board of the
County Commissioners to sign the attached service contract for billing
services with Data Control Systems.
The above and foregoing resolution was, on motion duly made and seconded,
adopted by the following vote on the 30 day of January, A.D. , 1984.

COUNTY OF WELD, STATE OF COLORADO
THIS AGREEMENT, Made and entered into this 30th day of January, 1984, by and
between the COUNTY OF WELD, State of Colorado, hereinafter called the County,
and Data Control Systems, hereinafter called the Contractor.
WITNESSETH: That the Contractor for and in consideration of the covenants,
conditions, agreements, and stipulations of the County hereinafter expressed,
does hereby agree to furnish to the County data processing billing services
for the Weld County Ambulance Service.
The term of this Agreement shall be nunc pro tunc from January 1, 1984 to and
until December 31, 1984, at which time this Agreement shall be automatically
renewed for successive one (1) year periods.
Either party may terminate this agreement at any time by giving the other
party a ten day written notice of such sooner termination and may be
terminated at any time without notice upon a material breach of the terms of
this agreement by either party.
County agrees to pay Contractor for services performed hereunder at the
following rates:
$ . 34 per statement
. 15 per name and address change or master file change
.,09 per transaction
2.00 minimum UPS charge
Postage at actual cost to contractor
Contractor may not assign or transfer this agreement, any interest therein or
claim thereunder without the prior written approval of County.
Payment to Contractor will be made only upon presentation of a proper claim by
Contractor subject to the approval of the Weld County Ambulance Service.
IN WITNESS WHEREOF, the parties hereto have executed this agreement as of the
day and year first above written.
