RE: APPROVE PROMISES FOR CHILDREN EARLY CHILDHOOD INITIATIVE GRANT
PROPOSAL FOR THE PRENATAL OUTREACH PROJECT AND AUTHORIZE CHAIR
TO SIGN - UNITED WAY OF WELD COUNTY
WHEREAS,the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with a Promises for Children Early Childhood
Initiative Grant Proposal for the Prenatal Outreach Project between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Weld County Department of Public Health and Environment, and United Way of Weld County,
commencing January 1, 2007, and ending June 30,2008,with further terms and conditions being
as stated in said grant proposal, and
WHEREAS,after review, the Board deems it advisable to approve said grant proposal, a
copy of which is attached hereto and incorporated herein by reference.
NOW,THEREFORE,BE IT RESOLVED by the Board of County Commissioners of Weld
County,Colorado,that the Promises for Children Early Childhood Initiative Grant Proposal for the
Prenatal Outreach Project between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Weld County Department of
Public Health and Environment,and the Colorado Department of Public Health and Environment be,
and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said grant proposal.
The above and foregoing Resolution was,on motion duly made and seconded,adopted by
the following vote on the 25th day of October, A.D., 2006.

Subject: Grant proposal to United Way
Promises For Children for the
Prenatal Outreach Project
Enclosed for Board review and approval is an application from Weld County Department of Public
Health and Environment to United Way of Weld County Promises for Children. The funding
requested is for a 1.0 FTE Prenatal Outreach Coordinator Specialist.
Assistance for pregnant women entering into prenatal care during the first trimester of pregnancy
is one of the goals that Promises for Children has identified as a need in Weld County to improve
the health of children. This position will help pregnant women with the Medicaid application
process and ensure timely first appointments for prenatal care.
For providing these services, Weld County is requesting the amount of$95,773.00 for the time
period of January 1 , 2007 through June 30, 2008, which is an 18 month contract.

Narrative Information
SECTION I: Program Narrative
1. Organizational History-
The Weld County Department of Public Health & Environment was established in 1938 by the
Weld County Commissioners. The County Commissioners provide governance and are
responsible for the public health policies and public health program's county wide.
2. Mission Statement- The mission of the Weld County Department of Public Health and
Environment is to preserve, promote and protect a healthy life and safe environment for the
residents of Weld County.
3. Description of the Program Services
• Year of program inception: The Prenatal Outreach Coordinator position was
established in October 2004.
• Description on program services: From October 2004 to August 2006, the Prenatal
Outreach Coordinator assisted pregnant women through the Medicaid/CHP+
application, educated them about not smoking, taking drugs (illegal and over the
counter), not drinking and answering any questions they may have. The Prenatal
Outreach Coordinator assisted in scheduling their first prenatal health care
appointment and then follows up with them during the course of their pregnancy to
ensure they are continuing to go to their appointments and access community
support available
• How this program meets the overall initiative outcome(s): From October 2004 to
August 2006, the Prenatal Outreach Coordinator contacted 782 pregnant women in
Weld County. Of these women, 90% have established prenatal care and completed
a first visit, and 92% have completed a Medicaid/CHP+ Application.
The Prenatal Outreach Coordinator completed 412 referrals to community agencies
for needed services that pregnant women disclosed as needing. These referrals
generated services such as: WIC, Bright Beginnings Newborn Home Visitations,
food; clothing; rental assistance and other health and human services through our
local 211 system.
• Target population: The target population for this project is pregnant women
needing access to prenatal care and prenatal health care coverage. Referrals
will come from the Weld County Dept. of Social Services; WCDPHE; Sunrise
Community Health Center; private physicians and other agencies serving
uninsured pregnant women.
• Parameters for client eligibility: Parameters for client eligibility for this project
addresses the issues of barriers of pregnant women accessing prenatal care,
such as financial barriers, limited providers that offer reduced fees for prenatal
care and the lack of knowledge of the importance of receiving early prenatal
care. With the changes of Medicaid Presumptive Eligibility, it is vital that
women understand and are assisted in applying for health care programs that
they are eligible for.
• Fees paid by clients, if applicable: There is no charge for pregnant women to
obtain assistance from the Prenatal Outreach Coordinator.
• Program partnerships and collaborations organizations The Weld County Department
of Public Health & Environment has collaborated with numerous agencies and
providers in the community. The community agencies include: United Way/

Promises For Children, Sunrise Community Health Center, First Steps of Weld
County, a prenatal case management program; North Colorado Family Medicine, ob-
gyn and family physician service providers; Weld County Department of Social
Services for Medicaid eligibility; Ft. Lupton Salud Family Health Center and other
health providers throughout Weld County are participating in this project. Two retired
pediatricians, Dr. Donald Cook and Dr. Tony Fink, who have served as President of
the National Pediatric Association, are integrally involved in this project.
4. The staffing and management of the program
Linda Henry RN, BSN, MA
Linda Henry RN, BSN, MA is the Director of the Public Health Services Division for the Weld County
Department of Public Health and Environment (WCDPHE). She has been an employee of the
WCDPHE for over 13 years. Linda has oversight of all Public Health programs such Maternal/Child
Health Services. These services target women, children, and infants. Prenatal care, Nurse Family
Partnership, Prenatal Plus, Immunizations, Health Care Program for Children with Special Needs,
EPSDT Medicaid, and Family Planning Programs.
Mary Lloyd-Cumley RN, NP
Mary Lloyd-Cumley is the Manager of Clinical Services at the Weld County Department of Public Health
and Environment. She is the supervisor of the Family Planning and Prenatal Plus Coordinator. Mary is
also the supervisor of the Nurse Practitioners, Medical Records and the Medical Mobile Unit
Coordinator. Her professional career has spanned over 30 years, and she has devoted most of that
time to helping people in a variety of health care settings.
Vanessa Wilkins RN, BSN
Vanessa Wilkins is the supervisor for the Family Planning Clinic RN's and is the Family Planning and
Prenatal Plus Coordinator at the Weld County Department of Public Health and Environment. She is
the direct supervisor of the position of the Prenatal Community Outreach Coordinator. This
arrangement is designed so that staff members working with pregnant women are on the same team.
Vanessa is able to orchestrate networking and share with staff from the Prenatal Plus, Nurse Family
Partnership, EPSDT, Family Planning and the Prenatal Outreach Coordinator.
Nelly Sanchez-Capcha
Nelly Sanchez-Capcha is currently employed by the Weld County Department of Public Health and
Environment, as the Prenatal Community Outreach Coordinator. She has held this position since
October 2004 when the initial grant funding for this endeavor was put in place through collaboration
with United Way.

Agencies will need to complete a cumulative program budget narrative and form as well as a budget
form for Year 1 (01/01/07- 06/30/07) and Year 2 (07/01/07- 06/30/08) including income and expenses.
NARRATIVE
Budget Detail
Personnel— Funding is for a 1 FTE Prenatal Outreach Coordinator who will support the efforts of
assisting clients with filing the Medicaid/CHP+ Application, assist them in getting into early prenatal
healthcare; tracking and providing follow up to ensure that they went to their prenatal appointment and
have their next appointment scheduled. In addition, the coordinator will provide necessary referrals to
pregnant women and their families. These referrals will be tracked for completion. This staff person
will be housed at Weld County Department of Public Health and Environment, but will also serve areas
of the Southern Weld County. Weld County Department of Public Health & Environment's Family
Visiting Supervisor will provide oversight of the Prenatal Outreach Coordinator.
1 FTE Prenatal Outreach Coordinator: Year 1: $15,756 salary Year 2: $ 32,300 salary
Fringe Benefits - The benefits are figured for Weld County benefit package for the Prenatal Outreach
Coordinator. Year 1 $5,247 and Year 2: $10,756
Supplies - include both general office and supplies for educational packets. Year 1: $1500
Year 2 $3000
Printing/Copies—Promotional and educational material handouts. Year 1 $3000 Year 2: $6000
Telephone - Year 1: $60 x 6 mos. = $320. Year 2 $720 = $ 60per mo. x 12 mos. Phone use for the
Prenatal Outreach Coordinator at the Health Department.
Postage - correspondence between Prenatal Outreach Coordinator and clients served as well as mailing
of surveys and marketing/educational materials. Year 1: 1,100: Year 2: $ 2,200
Travel—To distribute awareness materials, meet with clients, attend meetings pertaining to prenatal
care and work with community partners.
Year 1: $750 Year 2: $1,500.
Meetings— Year 1: $330, Year 2: 660 The finding is for meetings between partnering agencies,
community meetings to promote awareness of prenatal issues.
Consultant -- No consultant will be needed for this project.
Equipment—No equipment will be purchased for this project.
Other—Staff Development for the Prenatal Outreach Coordinator & Indirect cost, which covers office
space at the Weld County Dept. of Public Health and Environment and utilities, liability insurance,
accounting services, etc. at both the Health Dept. and United Way of Weld County— Year 1: $1,260
Year 2: $2584 .

Independent Auditors' Report
We have audited the accompanying financial statements of the governmental activities,
the business-type activities, the discretely presented component units, each major fund,
and the aggregate remaining fund information of Weld County, Colorado, as of and for
the year ended December 31, 2005, which collectively comprise Weld County's basic
financial statements as listed in the table of contents. These financial statements are the
responsibility of Weld County's management. Our responsibility is to express opinions on
these financial statements based on our audit. The financial statements of Weld County,
Colorado, as of December 31, 2004, were audited by other auditors whose report dated
May 20, 2005, expressed unqualified opinions on those statements. We did not audit the
financial statements of the Weld County Housing Authority. Those financial statements
were audited by other auditors whose report has been furnished to us, and our opinions
on the basic financial statements, insofar as they relate to the amounts included for the
Weld County Housing Authority, are based on the report of the other auditors.
We conducted our audit in accordance with auditing standards generally accepted in the
United States of America and the standards applicable to financial audits contained in
Government Auditing Standards, issued by the Comptroller General of the United States.
Those standards require that we plan and perform the audit to obtain reasonable
assurance about whether the financial statements are free of material misstatement. An
audit includes examining, on a test basis, evidence supporting the amounts and
disclosures in the financial statements. An audit also includes assessing the accounting
principles used and significant estimates made by management, as well as evaluating the
overall financial statement presentation. We believe that our audit provides a reasonable
basis for our opinions.
In our opinion, based on our audit and the report of other auditors, the financial
statements referred to above present fairly, in all material respects, the respective
financial position of the governmental activities, the business-type activities, the discretely
presented component units, each major fund, and the aggregate remaining fund
information of Weld County, Colorado, as of December 31, 2005, and the respective
changes in financial position and cash flows, where applicable, thereof for the year then

In accordance with Government Auditing Standards, we have also issued our report dated
May 5, 2006 on our consideration of Weld County's internal control over financial
reporting and on our tests of its compliance with certain provisions of laws, regulations,
contracts, and grant agreements and other matters. The purpose of that report is to
describe the scope of our testing of internal control over financial reporting and
compliance and the results of that testing, and not to provide an opinion on the internal
control over financial reporting or on compliance. That report is an internal part of an
audit performed in accordance with Government Auditing Standards and should be

inquiries of management regarding the methods of measurement and presentation of the

Our audit was conducted for the purpose of forming opinions on the financial statements
that collectively comprise Weld County, Colorado's, basic financial statements. The
introductory section, combining and individual nonmajor fund financial statements,

Article II Section 3-2-10.
A. The county strives to attain complete compliance with all provisions of equal
employment opportunity laws. Recruiting, selection and advancement of employees will
be on the basis of their relative ability, knowledge, skills and other relevant factors.
B. It is the intent of the County to:

Prenatal Community Outreach Coordinator Specialist: Weld County
General Statement of Duties: To impact and increase Medicaid enrollments for pregnant women
and assist enrolled women in gaining access to needed medical, social, educational, and allied health
services to improve healthy birth outcomes and client health status.
Supervision Received: Works under the Weld County Department of Public Health and
Environment (WCDPHE) policies and procedures.
Duties and responsibilities:
• Assist and facilitate clients with the Medicaid approval process.
• Refer clients to identified health care providers.
• Follow up with clients to ensure first appointments were completed.
• Act as a liaison to community organizations, area providers, and health and human services
agencies to raise awareness, strengthen relationships, and share information.
• Develop and implement an outreach plan for client recruitment and to inform community of
available services.
• Collect data on completed first appointments and stage of pregnancy.
• Perform other duties that are required by the Health Department in conjunction with the
requirements of the Grant.
• Develop reports that include, but not limited to, the following demographic information
concerning clients: initials of each client; age; ethnicity; stage of pregnancy the client initially
received prenatal care; outcome of Medicaid application, or whether another funding source
was located for each client.
• Provide to United Way reports and other documents as required by the terms and conditions
of the Grant. Ability to meet deadlines.
Required Knowledge, Skills, and Abilities: Ability to exercise initiative and considerable amount of
independent judgment; has excellent verbal and written communication skills; Bi-lingual in Spanish
and English; computer knowledge of software systems to enhance development of program
enrollment process, tracking system; establish and maintain effective working relationships with
community agencies, clinics, and families identified for Medicaid services. Utilization of office
equipment, practices, and procedures. Personal insured vehicle and current Colorado driver's
license.
Education: High School Graduate or G.E.D.
