RE: APPROVE TASK ORDER CHANGE ORDER LETTER #1 FOR THE FAMILY
PLANNING SERVICES PROGRAM AND AUTHORIZE CHAIR TO SIGN
WHEREAS,the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with a Task Order Change Order Letter#1 for
the Family Planning Services Program between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Weld County
Department of Public Health and Environment,and the Colorado Department of Public Health and
Environment, commencing July 1, 2006, and ending June 30, 2007, with further terms and
conditions being as stated in said letter, and
WHEREAS,after review, the Board deems it advisable to approve said letter, a copy of
which is attached hereto and incorporated herein by reference.
NOW,THEREFORE,BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Task Order Change Order Letter #1 for Family Planning Services
Program between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the Colorado Department of Public Health and Environment be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said task order.
The above and foregoing Resolution was,on motion duly made and seconded,adopted by
the following vote on the 4th day of October, A.D., 2006, nunc pro tunc July 1, 2006.

SUBJECT: Task Order Change Order Letter#1
Enclosed for Board review and approval is Task Order Change Order Letter#1 between the
Colorado Department of Public Health and Environment (CDPHE) and Weld County for
additional funding for the Family Planning Program.
This change order letter will provide additional funding in the amount of$6,368 to be used to
purchase methods of contraception including one or more of the following: Evra,Nuvaring,
Depo Pro‘era. and/or oral contraceptives.
With the additional funding, the total amount of the contract for the time period June 30, 2006
through June 30, 2007 will be $171,624.

State Fiscal Year 2006-2007, Family Planning Program
This Task Order Change Order Letter is issued pursuant to paragraph 4_C.of the Task Order identified as contract
routing number 07 FLA 00079 and contract encumbrance number PO FLA FFP07000079. This Task Order
Change Order Letter is between the COLORADO DEPARTMENT OF PUBLIC HEALTH AND
ENVIRONMENT and Board of County Commissioners of Weld County for the use and benefit of the Weld
County Department of Public Health and Environment. The Task Order,as amended,is referred to as the
"Original Task Order". This Task Order Change Order Letter is for the current term of July 01,2006,through
June 30,2007. The maximum amount payable by the State for the work to be performed by the Contractor during
this current term is increased by SIX THOUSAND THREE HUNDRED AND SIXTY EIGHT Dollars,
J$6,368.00)for an amended total financial obligation of the State of ONE HUNDRED SEVENTY-ONE
THOUSAND SIX HUNDRED AND TWENTY-FOUR DOLLARS ($171,624.00).
It is agreed the original Task Order Letter is and shall be modified,altered,and changed in the following respects
only:
a. Attachment A—Statement of Work for Family Planning Services is hereby revised by adding Paragraph 14.
14. The contractor shall use the additional funding to purchase efficacious methods of contraception,
including one or more of the following Evra,Nuvaring,Depo Provera,and/or oral contraceptives. The
funds are Title X federal funding and must be fully expended by December 31,2006. The contractor
shall keep a separate file of all invoices paid with these funds,as back up,in the event of an audit of
the utilization of this supplemental funding. Payments for these funds will be made in accordance with
paragraph 3.C.of the Task Order Letter and added to the total automatic payment.
h. Attachment A—Statement of Work for Family Planning Services is hereby revised by adding the following to
the Budget on page 4 of 4:
$6,368.00 for Method Purchases
$117,710.00 Total Title X Funding
$171,624.0Q Total Current Award
The first sentence in paragraph C. of the Original Task Order is modified accordingly. All other terms and
conditions of the Original Task Order are reaffirmed. This change to the Task Order shall be effective upon
approval by the State controller,or designee,or on November 1,2006,whichever is later.
Please sign,date,and return all 3 originals of this Task Order Change Order Letter by October 23,2006,to the

CRS 24-30-202 requires that the State Controller approve all state contracts.This contract is not valid until
the State Controller,or such assistant as he may delegate,has signed it.The contractor is not authorized to
begin performance until the contract is signed and dated below.If performance begins prior to the date
below,the State of Colorado may not be obligated to pay for the goods and/or services vided.
