Amendment
This is an Amendment(hereinafter"Amendment")to the License and Support
Agreement effective July 10,2004 (hereinafter"Agreement")between Decade Software
Company,LLC with principal place of business at 4201 West Shaw Avenue, Suite#102,
Fresno, California 93722 ("Decade"), and Weld County Health Department("Client")
with principal place of business at 1555 North 17th Avenue, Greeley,CO 80631.
It is mutually agreed by the parties that the above identified original Agreement be
amended by all terms of this Amendment. All other previous existing terms and
conditions and Appendices of the Agreement shall remain the same and shall continue in
full force and effect.
4. Effective Date and Term
The Agreement shall be extended beginning 07/10/06 for renewal on a month to month
basis until terminated. Either party may terminate this Amendment by giving the other
party at least 30 days prior written notice. Upon termination, Client is responsible for all
duties outlined in Section 10 of the Agreement.
5. Compensation
The price for the term beginning 07/10/06 and ending when terminated is $1,275.00 per
month. Decade will invoice Client at the beginning of each month for services to be
provided that month. Invoices have a payment term of net 30 days.
