RE: APPROVE AMENDMENT TO THE ADDENDUM TO TWO AGREEMENTS TO
PURCHASE RESIDENTIAL TREATMENT CENTER/RESIDENTIAL CHILD CARE
FACILITY SERVICES AND AUTHORIZE CHAIR TO SIGN
WHEREAS,the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with an Amendment to the Addendum of Two
Agreements to Purchase Residential Treatment Center/Residential Child Care Facility Services
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County,on behalf of the Department of Social Services,and the following
providers, commencing January 1, 2006, and ending June 30, 2006, with further terms and
conditions being as stated in said amendment, and
1. Youth Emancipation and Services, Inc.
2. El Pueblo Boys' and Girls' Ranch
WHEREAS,after review,the Board deems it advisable to approve said amendment,a copy
of which is attached hereto and incorporated herein by reference.
NOW,THEREFORE,BE IT RESOLVED by the Board of County Commissioners of Weld
County,Colorado,ex-officio Board of Social Services,that the Amendment to the Addendum of Two
Agreements to Purchase Residential Treatment Center/Residential Child Care Facility Services
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County,on behalf of the Department of Social Services, and listed below
be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendment.

Residential Child Care Facilities(RCCF)Service Various Ven ors
Enclosed for Board approval are Amendments to Addendums to Purchase RTC/RCCF Services
between the Weld County Department of Social Services(Department)and various vendors. The
Addendums were reviewed at the Board's Work Session held on February 13,2006.
'Amendments are with those Vendors who previously agreed to freeze their rates and now have
agreed to extend this rate freeze until June 30,2006.
The vendors include:
1. . El Pueblo Boy's and Girl's Ranch
2.i Youth Emancipation&Services

AMENDMENT OF THE
WELD COUNTY ADDENDUM
TO PURCHASE
RESIDENTIAL TREATMENT CENTER SERVICES
AND
RESIDENTIAL CHILD CARE SERVICES AGREEMENT
THIS AMENDMENT made this /q day of an 2006 between the Board of Weld County
Commissioners,sifting as the Board of Social Services,on behalf of the Weld County Department of Social
Services, hereinafter called"County" and El Pueblo Boys'and Girls'Ranch,One El Pueblo Ranch Way, Pueblo,CO
81006,hereinafter called"Contractor".
WHEREAS,the Colorado State Department of Human Services,hereinafter called"State Department"is
authorized to provide social services to individuals and families of individuals through its agents,County
Departments of Social Services,and
WHEREAS,County is authorized to purchase certain services for eligible children under State Department
rules,and
WHEREAS,certain Federal changes regarding Medicaid and related rates have occurred and the State of
Colorado has been granted an extension by the Centers for Medicare and Medicaid Services,until July 1, 2006 to be
in compliance with Federal rules;and
WHEREAS,County and the Contractor wishgs to amend its original agreement by extending the duration of
the Original Agreement entered into on 04 /V41 ,2005;and
WHEREAS,Contractor is licensed as a Residential Child Care Facility or is a certified Residential Treatment
Center.
NOW THEREFORE, it is hereby agreed that in consideration of the mutual Undertakings County and
Contractor agree as follows:
1. The term of the Agreement shall be extended from January 1,2006,to June 30,2006.
2. This Amendment,together with the original Agreement,constitutes the entire understanding among the parties
with respect to the subject matter hereof, and may not be changed or modified except as stated in the original
Agreement.

Services,hereinafter called "County"and Youth Emancipation&Services,Inc.,3400 16th St, Suite 3R,Greeley CO
80634,hereinafter called"Contractor".
