Enclosed for Board approval is an Agreement with the Weld County Community
Center Foundation to provide transportation services to their clients.
This is a continuous Agreement. The Community Center Foundation will
provide reimbursement to Human Resources at approximately $.82 per mile,
or the maximum reimbursement the Community Center Foundation receives
for these transportation services from the Department of Institutions.
The reimbLresment will be used to cover the costs associated with gas
and oil , repair, maintenance and salaries.

AGREEMENT
Amendment
Agreement between the Weld County Division of Human
Resources and the Weld County Community Center Foundation .
WHEREAS , it is the desire and intent of the Division
of Human Resources , hereinafter referred to as "Human
Resources, " and Weld County Community Center Foundation ,
hereinafter referred to as "WCCCF, " to continue the transpor-
tation services to WCCCF clients .
NOYY THEREFORE , the parties hereto agrees to the fol-
lowing:
1. Human Resources agrees to provide transportation
services to WCCCF clients .
2 . Human Resources shall provide insurance in accor-
dance with laws of the State of Colorado .
3. a. WCCCF agrees to reimburse Human Resources at
$. 82 per mile or the maximum reimbursement WCCCF receives
for these transportation services. The reimbursement will
be used to cover the costs of administration overhead , gas
and oil , repair, maintainance , and salaries.
b. Human Resources agrees to bill WCCCF quarterly
on the basis of actual transportation miles provided.
c. Human Resources and WCCCF agrees to review
actual transportation costs and reimbursement rates before
the end of each fiscal year and make adjustments at this time .
d. WCCCF agrees to provide Human Resources , at their
request, with verification of reimbursement received for trans-
portation provided by Human Resources .
4 . This Agreement shall be a continuous agreement by the
WCCCF Fiscal Year (July 1 through June) .
5 . It . s agreed between both parties that either party
hereto has the right to terminate this Agreement upon thirty
(30) days written notice given to the other party .
6. This Agreement may be amended with the consent of
both parties .
