Speckman, Executive Director, Human Resources Subject Purchase of Service Agreement for Speech-Language therapy for Head Start children in southern Weld County Enclosed for Board approval is a purchase of service agreement between Frank J. Perez, MA, CCC, and the Weld County Division of Human Resources ' Head Start Program. Head Start is contracting with Mr. Perez for the purpose of providing speech and language therapy for the Head Start children in the Frederick Head Start Center. Mr. Perez will provide the required therapy for the price of $15.00 per hour, not to exceed three hours per week. The agreement runs from November 14, ,1984, through June 15, 1985. If you have any questions, please do not hesitate to contact me. 340913 `I/7(f PURCHASE OF SERVICES AGREEMENT THIS AGREEMENT, made and entered into this 19th day of November, 1984, by and between the Weld County Division of Human Resources' Head Start Program, hereinafter referred to as "Head Start," and Mr. Frank J. Perez, MA, CCC, hereinafter referred to as "Provider". WITNESSETH: WHEREAS, Head Start is required to provide diagnostic and therapuetic services addressing speech and hearing disorders of Head Start children; and WHEREAS, Head Start must provide those services to Head Start children who live in southern Weld County; and WHEREAS, the Provider has the qualifications and certifications re- quired to deliver said services; and NOW THEREFORE, in accordance with the mutual benefits, responsibilities and considerations set forth herein, the parties hereto convenant and agree as follows: 1. The Provider will provide speech and language therapy to children with a specific speech or language need that might impair communication or learning. This therapy will be, when possible, in the dominant language of the child. The following tasks will be completed: A. Observe those children in the classroom before initiating therapy. B. Set up therapy schedule. C. Develop for each child in therapy a written, individualized plan stating goals and objectives for speech-language therapy. D. Consult with the teacher of the child receiving therapy and establish an Individualized Education Plan (I.E.P. ) _ and provide a copy of the I.E.P. to the teacher and the Special Needs Technician. E. Conduct evaluations of speech communication. F. Conduct one-to-one therapy for those children with the most severe needs. G. Conduct group therapy sessions for those children not seen individually. 2. This agreement will be effective from November 14, 1984, through June 15, 1984. 3. Head Start will reimburse the Provider for services only after receipt of documentation from the Provider specifying the number and kinds of services delivered. The billing should be made on a monthly basis and reimbursement will be made within two weeks of the billing. 4. Head Start will not reimburse the Provider for mileage. 5. The Provider will bill Head Start at the rate of $15.00 per hour for all services provided (preparation time and actual time) , not to exceed three hours per week. 6. This Agreement may be amended at any time with the consent in writing of both parties and upon receipt of thirty (30) days written notice. This Agreement may also be terminated upon thirty (30) days written notice by either party or upon completion of services or loss of funding to the Head Start Program.
