RE: APPROVE APPLICATION FOR COMMUNITY HEALTH INVESTMENT PROGRAM -
PLATTE VALLEY MEDICAL CENTER
WHEREAS,the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Application for Community Health
Investment Program from the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Department of Human Services, Area
Agency on Aging,to the Platte Valley Medical Center,with terms and conditions being as stated in
said application, and
WHEREAS,after review,the Board deems it advisable to approve said application,a copy
of which is attached hereto and incorporated herein by reference.
NOW,THEREFORE,BE IT RESOLVED by the Board of County Commissioners of Weld
County,Colorado,that the Application for Community Health Investment Program from the County
of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County,
on behalf of the Department of Human Services,Area Agency on Aging,to the Platte Valley Medical
Center be, and hereby is, approved.

Weld County Area Agency on Aging, DentalNision/Hearing Program is
requesting funding from the Community Health Investment Program, the
Platte Valley Medical Center Foundation for the amount $10,000 These
dollars, if awarded, will be used to supplement the existing
DentalNision/Hearing Program. The Area Agency on Aging has been
successful since 2002 in securing these additional funds. The
DentalNision /Hearing program is currently being funded by the Older
Coloradan Fund (State funding), North Colorado Medical Center Foundation
and the Community Health Investment Program (CHIP)

COMMUNITY HEALTH INVESTMENT PROGRAM GRANT REQUEST
FOR PROGRAM YEAR BEGINNING AUGUST 1, 2006 AND ENDING
JULY 30, 2007.
PROPOSAL:
The Weld County Area Agency on Aging has received assistance from the CHIP
Committee since 2002 to enhance the existing dental, vision and hearing program (DVH)
in South Weld County. When we submitted the first grant in 2002 we believed that there
was an ever increasing need for these very important services in South Weld County.
During the last four years we have identified seniors who were clearly in need of one or
more of the services offered through the DVH Program.. We anticipate that as our county
continues to grow the number of seniors needing assistance will only continue to
increase.
The Dental/Vision/Hearing Program is currently being funded, in part, by dollars the
Area Agency on Aging receives from the State of Colorado known as State Funding for
Senior Services (SFSS). The Area Agency on Aging distributes the allocated dollars
equitable across the county. Unfortunately the funding amount falls short of the multiple
needs that exist in our community. The funding we have received through the CHIP
Committee, have allowed us to serve a larger number of seniors who live in South Weld
County.
We are requesting the continuation of supplement funding for the Dental/Vision Hearing
Program. The dollars, if received, will again be spend providing services for seniors
living in South Weld County who are unable to access these services due to financial
limitations. We will continue to allocate State Funding to South Weld County region in
order to increase the number of clients served.
DOCUMENTED NEED:
In September of 2004 a state-wide Strengths and Needs Assessment of Older Adults was
completed. The Area Agencies on Aging across the state were able to obtain detailed
information regarding their specific region. Based on the information obtain from the
study over 2,500 Weld County seniors indicated that they needed but could not afford
eyeglasses, hearing aides or dentures. We anticipate that the need will continue to gross
as the population increases since we do not anticipate programs such as Medicaid
covering these types of service needs.
Key provider agency staff members, located in the South Weld County, continue to
express ongoing need for dental, vision and hearing services. In addition, by attending
North Adams/South Weld County Resource Meeting, Employment Services Disabilities
Agency Resource Conference, and the 7th Annual Weld County Health Fair we continue
to hear about the value of this program and the ongoing need.

Seniors in South Weld County have been instrumental in notifying other seniors about
the services available. We continue to receive requests for information on the dental,
hearing and vision services from adult children whose parents reside in South Weld
County. Seniors who have been successfully assisted with one of the three services in the
past, find they need additional services contact us. This is a clear indication of how
important these services are to seniors.
Public service ads, targeting South Weld County, have appeared in the Senior
Marketplace News providing information to seniors regarding the dental, hearing and
vision program. We also continue to use the assistance of the Fort Lupton Senior Center
and Salud Family Health Clinic in promoting the program.
EXPECTED RESULTS OF THE PROGRAM:
Historically, the goals for Program Years 2002-2003, 2003-2004, 2004-2005 were met,
and 2005-2006 we are on our way to meeting our goal of thirty (30) seniors. By the end
of January 2006, seventeen (17) South Weld County residents have been found eligible
and funds committed for assistance. We anticipate that within the next five months, we
will be able to meet our set program goal. Because we are able to work with service
providers that are willing to, in most cases, offer a discount to seniors, allowing more
participants to be eligible for assistance. An established list of service providers is
available to participant, but the participant may chose to utilize any service provider.
SPECIFIC OBJECTIVES. TIMETABLES AND PROCESS UTILIZED TO
RESEARCH PROJECT GOALS:
For Program Year 2006-2007, our goal is to enable seniors to obtain the services needed
to maintain their health and a quality of life. Our desire is to assist thirty (30) seniors in
South Weld County in 2006-2007. Continued coordination with aforementioned entities
we believe will prove to be effective in marketing the dental, vision and hearing aid
program. We will continue to encourage local doctors (dental, vision and hearing) to
make referrals for seniors who may need assistance. Continued expansion of South Weld
County area service providers is desired.
HOW WILL RESULTS BE MEASURED?
The participants and services provided through these programs are tracked using SAMS
2000. our agency software system. This software allows us to continue to submit
progress reports as required by CHIP Committee. We will continue to seek feedback
from clients as to their level of satisfaction for services received.
Due to the funding received from the CHIP Committee, we have been able to enhance
our dental, hearing and vision program in South Weld County. We have contacted
providers in the area who have a genuine interest in assisting seniors in need of the
services that we are offering. With the funds provided by the CHIP Committee as
supplement funds and dollars received from the State Funding for Senior Services, the
dental, vision and hearing program in South Weld County has been successful. We arc
enabling seniors to obtain the services needed to maintain their health and a quality of
life.
We are asking the CHIP Committee members, to once again partner with us in continuing
to meet the needs of South Weld County seniors, Program Year 2006-2007. Any funds
received from the CHIP Committee will be used as supplemental funds. The Older
Coloradan Funds will continue as our primary source of funding for the Dental, Vision
and Hearing Program. The North Colorado Medical Center Foundation funds are directed
towards addressing the ever growing number of individuals on the North Weld County
Wait List.

Mr.Wallace Schreiber was born in Limon, Colorado seventy-seven years
ago. The eldest of four children, he grew-up on his father's ranch in
Arriba, Colorado. Mr. Schreiber grew accustomed to hard work on the
ranch. He married; they had two children. With the passage of time, Mr. i

Schreiber became a proud grandfather.

In November 1996, Mr. Schreiber suffered an injury on a construction
job site. Hospitalization, then rehabilitation in a long-term care facility, VT-tiltR`,Ir
enabled Mr Schreiber to use a motorized wheelchair.

When Mr. Schreiber, with limited financial resources.needed critical tr, h } 1.1

Program of the Weld County Area Agency on Aging. The program,
coordinated by Kathleen Alvarado, is designed to maintain the independence and a healthy quality of life for
those who participate.
"Being able to maintain healthy teeth, in the long run, impacts my overall health, thus enabling me to avoid
unnecessary hospitalization," commented Mr. Schreiber, when he was asked what the program meant to him.

The five member Board of County Commissioners is the statutory head of Weld County's governme
of the commissioners are elected at large and the other three are elected from the district in which
reside. As is the case with all elected county officials, commissioners must be at least 21 years of a
have resided in the county for at least one year prior to their election. Their elected position must t
primary employment during the term they serve.
As a board, the commissioners are required to establish a budget, levy taxes, appropriate funds for

business, establish fees and licenses, purchase or otherwise acquire, own, sell, lease, trade, and of
deal in real and personal property in the name of the county, receive gifts and grants for the count'
approve and execute contracts and lawsuits in the name of the county.
;The board also is required to sit as a board of appeals to hear complaints on actions taken by coun'
boards, commissions and departments, as a board of equalization to hear appeals on tax assessme
; a county board of welfare, and to make appointments to fill vacancies in all county offices except ti
board of commissioners and the county council.
Each commissioner has primary responsibility for one of the county's five departments and serves
assistant in another. The five departments are communication services, health services, planning s,
engineering services, and finance, administration, central purchasing, and personnel. Their respons
rotate each year.
The chairman of the Board of County Commissioners is chosen by the commissioners each year an(
always serves as chairman of the finance and administration department during his tenure as chair
the board.
Commissioners are only allowed to serve two complete elected terms under charter provisions.
The board meets at 9:00 a.m. each Monday and Wednesday in the first floor meeting room at the
Centennial Center Complex, 915 10th Street, Greeley.
