RE: APPROVE TASK ORDER OPTION TO RENEW LETTER#2 FOR WEST NILE VIRUS
DISEASE SURVEILLANCE PROGRAM AND AUTHORIZE CHAIR TO SIGN
WHEREAS,the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with Task Order Option to Renew Letter#2 for
West Nile Virus Disease Surveillance Program between the County of Weld, State of Colorado, by
and through the Board of County Commissioners of Weld County, on behalf of the Weld County
Department of Public Health and Environment,and the Colorado Department of Public Health and
Environment,commencing January 1,2006,and ending December31,2006,with further terms and
conditions being as stated in said task order, and
WHEREAS,after review, the Board deems it advisable to approve said letter, a copy of
which is attached hereto and incorporated herein by reference.
NOW,THEREFORE,BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that Task Order Option to Renew Letter #2 for West Nile Virus Disease
Surveillance Program between the County of Weld, State of Colorado,by and through the Board of
County Commissioners of Weld County,on behalf of the Weld County Department of Public Health
and Environment,and the Colorado Department of Public Health and Environment be, and hereby
is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said letter.
The above and foregoing Resolution was,on motion duly made and seconded,adopted by
the following vote on the 25th day of January, A.D., 2006, nunc pro tunc January 1, 2006.

SUBJECT: Option to Renew Letter for Disease
Surveillance Program (West Nile Virus
Surveillance and Testing)
Enclosed for Board review and approval is Task Order Option to Renew Letter Number Two
from the Colorado Department of Public Health and Environment for surveillance and laboratory
testing for West Nile Virus.
This option to renew letter will provide finding of$44,681 for the time period January 1, 2006
through December 31, 2006. Of this amount, $19,598 is for staff salaries and fringe benefits,
$4,000 for laboratory supplies, $11,450 for field surveillance and equipment, $2,000 for
educational supplies, $385 for travel, and $7,248 for indirect costs. The funds are passed
through the Colorado Department of Public Health and Environment from the Centers for
Disease Control.
I recommend your approval of this option to renew letter.

Renewal Letter#2, for contract#PO FHA EPI0500098, (Renewal Letter Routing#06 FHA 00208),
with the Colorado Department of Public Health and Environment.
All three copies must be signed with two appropriate signatures, one the designated authority binding
the contractor to the contract, and the other attesting to the authority of the first. If you have a seal,
please affix it to the signature page of each copy.
Please return all 3 copies to my attention. Upon receiving the agreements, I will have them approved by
the proper authorities. I will return to you a verified copy as soon as possible. Please be reminded this

State Fiscal Year 2006-2007 Disease Surveillance Program
This Task Order Option to Renew Letter is issued pursuant to paragraph F_4.of the Master Contract identified by
contract routing number 05 FAA 00054 and paragraph C.5. of the Task Order identified by contract routing number
05 FHA 00098 and contract encumbrance number PO FHA EPI0500098. This Task Order Option to Renew Letter
is between the COLORADO DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT and BOARD
OF COUNTY COMMISSIONERS OF WELD COUNTY. The Task Order has been amended by Task Order
Change Order Letter 1, contract routing number 05 FHA 00166,Task Order Option to Renew Letter 1, contract
routing number 06 FHA 00063, and Task Order Change Order Letter 2,contract routing number 06 FHA 00114 if
any. The Task Order,as amended, if applicable, is referred to as the "Original Task Order". This Task Order
Option to Renew Letter is for the renewal term of January 1,2006, through December 31,2006. The maximum
amount payable by the State for the work to be performed by the Contractor during this renewal term is FORTY
FOUR THOUSAND SIX HUNDRED EIGHTY ONE DOLLARS,f$44,681.00) for an amended total financial
obligation of the State of ONE HUNDRED THRITY THREE THOUSAND EIGHT HUNDRED SEVENTEEN
DOLLARS, This is an increase of($44,681.00)of the amount payable from the previous term. The Budget for this
renewal term is incorporated herein by this reference, made a part hereof, and attached hereto as"Attachment G".
The first sentence in paragraph C.1.of the Original Task Order is modified accordingly. All other terms and
conditions of the Original Task Order are reaffirmed. This Task Order Option to Renew Letter is effective upon
approval by the State Controller,or designee, or on January 1,2006, whichever is later.

Description of Activities:The contract supports the WNV surveillance including
collection,identification and submission for testing of dead birds and vector
mosquitoes,public education on prevention and personal mosquito control and
human case investigation.
Method of accountability: submission of copies of receipts and documentation for
personnel, purchase of equipment and educational activities.Provide animal surveillance
data,conduct investigation of human cases and report to CDPHE. Identify all mosquitoes
and dead bird sample and submit to appropriate laboratory for testing within 1 week of
collection.
