RE: APPROVE AMENDMENT #1 TO THE INTERGOVERNMENTAL CONTRACT FOR
TEEN SEATBELT EDUCATION IN WELD COUNTYAND AUTHORIZE CHAIRTO SIGN
WHEREAS,the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with Amendment#1 to the Intergovernmental
Contract for Teen Seatbelt Education in Weld County between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Weld County Sheriffs Office, and the Colorado Department of Transportation (CDOT),
commencing upon full execution,and ending December 31,2006,with further terms and conditions
being as stated in said amendment, and
WHEREAS,after review,the Board deems it advisable to approve said amendment, a copy
of which is attached hereto and incorporated herein by reference.
NOW,THEREFORE,BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that Amendment #1 to the Intergovernmental Contract for Teen Seatbelt
Education in Weld County between the County of Weld,State of Colorado,by and through the Board
of County Commissioners of Weld County,on behalf of the Weld County Sheriffs Office, and the
Colorado Department of Transportation (CDOT) be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendment.
The above and foregoing Resolution was,on motion duly made and seconded,adopted by
the following vote on the 6th day of September, A.D., 2006.

RE: Contract Amendment and Objective Changes as of July 31, 2006,
Project #06-06-61-07
This memo serves as notice of the following changes made to the Teen Seatbelt Safety
Program with Weld County Sheriffs Office:
1. New objectives to be amended to the contract include:
a. Research existing Seat Belt Challenge programs and seat belt activities and
present to each high school staff.
b. Develop a memo of understanding with each of the participating high schools in
Weld County to execute the teen safety program activities.
c. Work with school staff to develop a student traffic safety council to develop and
organize seatbelt safety and other traffic safety activities. Students administer
activities.
d. Identify teen seat belt and other motor vehicle educational and training materials
needed for executing the program activities.
e. Coordinate seat belt observations (rewards to seat belt users, and letters to non-
seat belt users) with the traffic safety activities at the high schools.
2. Objectives being deleted for existing contract include:
a. Produce and distribute 1,000 Spanish speaking video (10-15 minutes) about the
Colorado State Law concerning seat belt use and other traffic issues. This video
will be given to all mono-lingual parents in elementary schools across Weld
County.
b. Conduct at least one traffic safety presentation in each high school in Weld
County. (P.A.R.T.Y.) Prevent Alcohol and Risk-Related Trauma and/or Every
Fifteen Minutes.

3. Project #06-06-61-07 contract will be dose date will be extended from September 30,
2006 to December 31, 2006. Remaining funds of the contract will be rolled-forward to
Federal Fiscal Year 2007 to be spent by December 31, 2006.

CONTRACT AMENDMENT # 1
THIS AMENDMENT, made this 6th day of September 2006 , by and
between the State of Colorado for the use and benefit of the Department of Transportation, Office
of Transportation Safety, 4201 East Arkansas Avenue,Denver, Colorado, 80222, hereinafter
referred to as the State,WELD COUNTY SHERIFF'S OFFICE, 1950 O Street,Greeley, CO
80631,hereinafter referred to as the contractor,
FACTUAL RECITALS
WHEREAS, authority exists in the law and funds have been budgeted, appropriated and
otherwise made available and a sufficient unencumbered balance thereof remains available for
payment in Fund No. 400,Appropriation Code 303, Organization Unit 9836, GBL Number 617C,
Function 8001, FEIN Number 846000813; Total contract encumbrance amount for this
amendment: TWENTY THREE THOUSAND DOLLARS ($23,000.00).
WHEREAS, Required approval, clearance, and coordination has been accomplished
from and with appropriate agencies; and
WHEREAS, The parties entered into a contract dated January 25,2006, for conducting
activities in high schools to inform students of consequences of non-seat belt usage and reward
students who do use seat belts,the purpose for this amendment is described below.
The contract amendment is to:
1. Research existing Seat Belt Challenge programs and seat belt activities and
present to each high school staff.
2. Develop a memo of understanding with each of the participating high schools in
Weld County to execute the teen safety program activities.
3. Work with school staff to develop a student traffic safety council to develop and
organize seatbelt safety and other traffic safety activities. Students administer

NOW THEREFORE, it is hereby agreed that
1. Consideration for this amendment to the original contract, Contract Routing
Number 06 HTS 00005 , dated January 25,2006 consists of the payments which shall be made
pursuant to this amendment and the promises and agreements herein set forth.

2. It is expressly agreed by the parties that this amendment is supplemental to the
original Contract Routing Number 06 FITS 00005, dated January 25, 2006 referred to as the
"original contract," which is, by this reference, incorporated, and made a part hereof, and attached
as exhibit A, and all terms, conditions, and provisions thereof, unless specifically modified
herein, are to apply to this amendment as though they were expressly rewritten, incorporated,and
included herein.
3. It is agreed the original contract is and shall be modified, altered, and changed in
the following respects only:
a. Additional seat belt safety challenge contract objectives to be included (please
see Attachment A-Contract Objectives and Tasks, Evaluation and Budget—
Scope of Work"the Work")
b. Contract termination date to be changed to December 31,2006
4. The effective date of this amendment is upon approval of the State Controller.
5. Except for the "Special Provisions," in the event of any conflict, inconsistency,
variance, or contradiction between the provisions of this amendment and any of the provisions of
the original contract, the provisions of this amendment shall in all respects supersede, govern, and
control. The "Special Provisions" shall always be controlling over other provisions in the
contract or amendments. The representations in the Special Provisions concerning the absence of
bribery or corrupt influences and personal interest of State employees are presently reaffirmed.
6. FINANCIAL OBLIGATIONS OF THE STATE PAYABLE AFTER THE
CURRENT FISCAL YEAR ARE CONTINGENT UPON FUNDS FOR THAT PURPOSE
BEING APPROPRIATED, BUDGETED, AND OTHERWISE MADE AVAILABLE.

IN WITNESS WHEREOF, the parties hereto have executed this amendment on the day first
above written.

ALL CONTRACTS MUST BE APPROVED BY THE STATE CONTROLLER
CRS 24-30-202 requires that the State Controller approve all state contracts. This contract is not
valid until the State Controller, or such assistant as he may delegate, has signed it. The contractor
is not authorized to begin performance until the contract is signed and dated below. If
performance begins prior to the date below, the State of Colorado may not be obligated to pay for
goods and/or services provided.
