We are the attorneys for State Farm Mutual Automobile Insurance Company. This letter will
serve as notice of our clients subrogation rights in the amount of$4,851.16.
The information we have in our file is that a Mr.Jeffrey A. York, the driver of a vehicle owned
by Weld County, backed his vehicle into State Farm's insured causing damage to her vehicle and
resulting in medical claims being paid on her behalf as a result of the collision. Our insured was driving
behind Mr. York through a construction zone. Mr. York stopped and started backing up his vehicle.
State Farm's insured started honking her horn and attempted to put her vehicle in reverse but was struck
by Mr. York.
