RE: APPROVE SERVICE ENHANCEMENTAGREEMENTFOR WEBSITE DEVELOPMENT
OF HEALTH EDUCATION GRANTS AND AUTHORIZE CHAIR TO SIGN - ACS
GOVERNMENT SYSTEMS, INC.
WHEREAS,the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with a Service Enhancement Agreement for
Website Development of Health Education Grants between the County of Weld, State of Colorado,
by and through the Board of County Commissioners of Weld County, and ACS Government
Systems, Inc.,commencing December 1,2006, and ending September 30,2007,with further terms
and conditions being as stated in said agreement, and
WHEREAS,after review,the Board deems it advisable to approve said agreement, a copy
of which is attached hereto and incorporated herein by reference.
NOW,THEREFORE,BE IT RESOLVED by the Board of County Commissioners of Weld
County,Colorado,that the Service Enhancement Agreement for Website Development of Health
Education Grants between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, and ACS Government Systems, Inc., be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said agreement.
The above and foregoing Resolution was,on motion duly made and seconded, adopted by
the following vote on the 6th day of December, A.D., 2006, nunc pro tunc December 1, 2006.

Related Agreement: Master OnSite Services Agreement between Weld County and ACS Government
Systems, Inc. (ACS)dated November 1993
All terms defined in the Agreement and its amendments shall have the same meaning ascribed to them
therein when used in this Service Enhancement Request. Pursuant to Section 2.2 of the Agreement,
Client hereby requests that ACS perform the Supplemental Services described below. The parties
acknowledge that any Supplemental Services to be performed pursuant to this Service Enhancement
Request shall be performed under the terms and conditions of the Agreement. In addition, unless
otherwise set forth below, any payments to be made by Client to ACS under this Service Enhancement
Request shall be made pursuant to the terms and conditions of the Agreement. The amounts to be paid by
Client to ACS under this Service Enhancement Request shall be in addition to any other amounts which
Client is obligated to pay to ACS under the Agreement.

ACS will provide one additional half-time position with the skills necessary to develop and/or
modify the Weld County Health Department web pages.
This Service Enhancement Request will be effective December 1, 2006.
Time Frame
Projected Start Date: December 1, 2006
Projected End Date: September 30, 2007
Payment
The total payment for the above services will be $25,000. This is to be paid in nine equal monthly
payments of$2,777.78.
