RE: APPROVE AMENDMENT TO PURCHASE OF SERVICES AGREEMENT AND
AUTHORIZE CHAIR TO SIGN -JACQUELINE JOHNSON
WHEREAS,the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with an Amendment to the Purchase of Services
Agreement between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Social Services, and Jacqueline
Johnson,commencing September30,2006,and ending December 31,2006,with further terms and
conditions being as stated in said amendment, and
WHEREAS,after review,the Board deems it advisable to approve said amendment,a copy
of which is attached hereto and incorporated herein by reference.
NOW,THEREFORE,BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, ex-officio Board of Social Services, that the Amendment to the Purchase of
Services Agreement between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Department of Social Services, and
Jacqueline Johnson be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendment.
The above and foregoing Resolution was,on motion duly made and seconded,adopted by
the following vote on the 1st day of November, A.D., 2006, nunc pro tunc September 30, 2006.

FR: Judy A. Griego, Director, Social Services \k c,,,,, (.13 RE: Amendment of Purchase of Services Agree ent—Jacqueline Johnson
Enclosed for Board approval is an Amendment of the Purchase of Services Agreement between
the Weld County Department of Social Services (Department)and Jacqueline Johnson. This
Amendment was reviewed at the Board's Work Session held on October 4, 2006.
The major reasons for the Amendment are to:
1. Extend the end date of the Agreement from September 30,2006, to December 31,
2006. This extension will allow a transition and support for the new Executive Director
of the Interagency Oversight Committee of Weld County's Collaborative Management
of Multi-agency Services Provided to Children and Families, established under
Colorado Statute 14-1.9-101.
2. Increase the compensation to Ms.Johnson,Facilitator, by$6,000 or a total of
$46,000.
All other provisions of the Agreement remain the same including the maximum reimbursement
level of$40,000.

AMENDMENT
PURCHASE OF SERVICES AGREEMENT

THIS AMENDMENT is made and entered into this �5 day of A :n be 2006,by and between the Board
of County Commissioners of the County of Weld, State of Colorado, sitting on behalf of the Weld County
Department of Social Services,hereinafter referred to as"Social Services",and Jacqueline Johnson,hereinafter
referred to as"Facilitator".
WITNESSETH:
WHEREAS,Social Services and the Facilitator wish to amend her Purchase of Services Agreement
originally entered into on July 1,2005,and the amendment entered into on July 10,2006; and
WHEREAS,the Interagency Oversight Committee of Weld County's Collaborative Management of
Multi-agency Services Provided to Children and Families,established under Colorado Statute 14-1.9-101,
requests additional services of the Facilitator;and
WHEREAS,the Facilitator agrees to continue its facilitation services, according to the original Purchase of
Services Agreement.
NOW,THEREFORE,in consideration of the mutual promises and covenants contained herein,the parties
hereto agree to amend its original Agreement as follows:
1. TERM: The term of this Agreement shall be extended from its ending date of September 30,
2006,to December 31,2006.
2. COMPENSATION: The funding under this Agreement shall be increased by$6,000 or to a total
level of compensation of$46,000.
3. This Amendment, together with the original Purchase of Services Agreement constitutes the entire
understanding among the parties with respect to the subject matter hereof,and may not be changed
or modified except as stated in the Purchase of Services Agreement.
