RE: APPROVE CORRECTIVE ACTION PLAN IN RESPONSE TO THE PRISM HEAD
START REVIEW REPORT AND AUTHORIZE CHAIR TO SIGN
WHEREAS,the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with a Corrective Action Plan in Response to
The PRISM Head Start Review Report from the County of Weld, State of Colorado,by and through
the Board of County Commissioners of Weld County, on behalf of the Department of Human
Services, Family Educational Network of Weld County, to the Department of Health and Human
Services,Administration for Children and Families,Office of Head Start,with terms and conditions
being as stated in said plan, and
WHEREAS,after review,the Board deems it advisable to approve said plan,a copy of which
is attached hereto and incorporated herein by reference.
NOW,THEREFORE,BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado,that the Corrective Action Plan in Response to The PRISM Head Start Review
Report from the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services, Family
Educational Network of Weld County, to the Department of Health and Human Services,
Administration for Children and Families, Office of Head Start, be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said plan.
The above and foregoing Resolution was,on motion duly made and seconded,adopted by
the following vote on the 27th day of December, A.D., 2006.

Re: PRISM Monitoring/Retraining Directive
I appreciate your consistent efforts in training and monitoring to ensure that our Center staff is
well versed on and compliant with applicable standards, laws and regulations. However,
yesterday, our PRISM Reviewers observed activity at our East Memorial Center that violated
standards and policies.
Teachers in the young preschool classroom took children to the elementary playground
instead of the age-appropriate preschool playground. In addition, one teacher left the other
alone with the children on that playground for a period of at least 10 minutes. Upon their
return to the classroom, one teacher went into the kitchen area and left the other to supervise
the children alone.
I realize that this kind of violation has not been a finding in our programs' internal monitoring,
in state licensing monitoring (announced or unannounced), or in past federal reviews. While I
recognize that this was an isolated incident, as with any monitoring finding, it must be
addressed and remedied so that it does not become a systemic problem. We must look at
this as a teachable moment for East Memorial staff and an opportunity to confirm and
reinforce the knowledge base of other staff.
Please plan to conduct a retraining of program staff at our Migrant Team Meeting on August
3, 2006. The retraining should include, at a minimum,the following: 1)the implementation of
regulations and standards regarding play equipment and materials, 2)the implementation of
regulations and standards regarding ratios and supervision of children. Contact our State
Licensing people to assist you.
The safety of our children is paramount. Let us take advantage of this opportunity to close
gaps and strengthen our Program.

Re: Child Supervision
At East Memorial Head Start, on Wednesday,July 12,2006, Federal Reviewers observed children not
being properly supervised on the playground and in the classroom (East Classroom). This is in
violation of Colorado State Licensing Rules and Regulations, FENWC Policies and Procedures and
Head Start Standards.
In order to be in compliance with Colorado Licensing Rules and Regulations,FENWC policies and
Head Start Performance Standards,staff members are expected to maintain the required teacher-child
ratio at all times. Under no circumstances are classrooms to be out of ratio at any time.
By Thursday,July 13,2006, at 3:00 p.m.,East Memorial is to submit a Plan of Action to the Program
Director outlining how the Center's East Classroom will maintain compliance for the balance of the
program year.
The safety of our children is paramount in this organization. Failure to properly supervise children is
cause for disciplinary action.

Plan of Corrective Action
Effective July 12, 2006
Two Head Start staff will remain with the children at all times, indoors and outdoors. Teacher to
child ratios will be maintained at all times both indoors and outdoors.
b During outdoor time, if a child should need to use the bathroom or require a change of pull-
up, the teachers will bring in the whole class in order to not leave a single staff person with
the children. If the Center Director is available, she may return to the playground with one
teacher and the other children. The teacher remaining with the child who needs to go potty
will use the classroom where the other staff are present in order to not be alone with the
child.
b During indoor class time, when the teacher assistant is preparing snack or lunch, she will use
the work- cart inside the classroom so that she can maintain supervision of the children and
maintain ratios.
