RE: APPROVE AMENDMENT TO THE ADDENDUM TO TWENTY-THREE AGREEMENTS
TO PURCHASE RESIDENTIAL TREATMENT CENTER/RESIDENTIAL CHILD CARE
FACILITY SERVICES AND AUTHORIZE CHAIR TO SIGN
WHEREAS,the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to the Addendum to
Twenty-three Agreements to Purchase Residential Treatment Center/Residential Child Care Facility
Services between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County,on behalf of the Department of Social Services,and the following
providers, commencing January 1, 2006, and ending June 30, 2006, with further terms and
conditions being as stated in said amendment, and
1. Alternative Youth Adventures 13. Mountain Star Center
2. Alternative Homes for Youth 14. Mountain States Children's Home, Inc.
3. Childrens Ark, Inc. 15. Reflections for Youth, Inc.
4. Courthouse, Inc. 16. Roundup Fellowship I

6. Friends of Children 18. Roundup Fellowship V
7. Griffith Centers for Children, Inc. 19. Serenity II Children's Home
8. Jefferson Hills 20. Shiloh Home
9. Kathleen Painter Littler/North Range 21. The University of Colorado Health
Behavioral Sciences Center, dba Synergy
10. Kids Ark in Sterling 22. Third Way Center
11. Midway Youth Services, Inc. 23. Youthtrack

WHEREAS,after review,the Board deems it advisable to approve said amendment,a copy
of which is attached hereto and incorporated herein by reference.
NOW,THEREFORE,BE IT RESOLVED by the Board of County Commissioners of Weld
County,Colorado,ex-officio Board of Social Services,that the Amendment to the Addendum to the
Twenty-three Agreements to Purchase Residential Treatment Center/Residential Child Care Facility
Services between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Social Services, and the above
listed providers be, and hereby are, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said addendums.

(RTC)/Residential Child Care Facilities(RCCF) Services with Various
Vendors
Enclosed for Board approval are Amendments to Addendums to Purchase RTC/RCCF Services
between the Weld County Department of Social Services (Department)with various vendors.
The Amendments were reviewed at the Board's Work Session held on February 13, 2006.
Amendments are with those providers who have previously frozen their rates and have now
agreed to continue with those rates until June 30, 2006.

Services, hereinafter called"County"and Alternative Youth Adventures, 1236 N.Townsend, Montrose, CO 81401,
hereinafter called"Contractor".
WHEREAS,the Colorado State Department of Human Services,hereinafter called"State Department" is
authorized to provide social services to individuals and families of individuals through its agents,County
Departments of Social Services, and
WHEREAS,County is authorized to purchase certain services for eligible children under State Department
rules,and
WHEREAS,certain Federal changes regarding Medicaid and related rates have occurred and the State of
Colorado has been granted an extension by the Centers for Medicare and Medicaid Services,until July 1, 2006 to be
in compliance with Federal rules;and
WHEREAS,County and the Contractor wishe$to amend its original agreement by extending the duration of
the Original Agreement entered into on 13 Du 3'd,2005;and
WHEREAS, Contractor is licensed as a Residential Child Care Facility or is a certified Residential Treatment
Center.
NOW THEREFORE,it is hereby agreed that in consideration of the mutual Undertakings County and
Contractor agree as follows:
1. The term of the Agreement shall be extended from January 1,2006,to June 30, 2006.
2. This Amendment,together with the original Agreement,constitutes the entire understanding among the parties
with respect to the subject matter hereof,and may not be changed or modified except as stated in the original
Agreement.

WHEREAS,the Colorado State Department of Human Services,hereinafter called"State Department" is
authorized to provide social services to individuals and families of individuals through its agents,County
Departments of Social Services,and
WHEREAS,County is authorized to purchase certain services for eligible children under State Department
rules,and
WHEREAS,certain Federal changes regarding Medicaid and related rates have occurred and the State of
Colorado has been granted an extension by the Centers for Medicare and Medicaid Services,until July 1,2006 to be
in compliance with Federal rules; and
WHEREAS,County and the Contractor wishes to amend its original agreement by extending the duration of
the Original Agreement entered into on I}n ti 3 rA,2005;and
WHEREAS,Contractor is licensed as a Residential Child Care Facility or is a certified Residential Treatment
Center.
NOW THEREFORE, it is hereby agreed that in consideration of the mutual Undertakings County and
Contractor agree as follows:
1. The term of the Agreement shall be extended from January I, 2006,to June 30,2006.
2. This Amendment,together with the original Agreement, constitutes the entire understanding among the parties
with respect to the subject matter hereof,and may not be changed or modified except as stated in the original
Agreement.

Commissioners, sitting as the Board of Social Services,on behalf of the Weld County Department of Social A
Services,hereinafter called"County"and Serenity II Children's Home, 7169 S.Liverpool St.,Centennial,CO
80016,hereinafter called"Contractor". Ca;
WHEREAS,the Colorado State Department of Human Services,hereinafter called"State Department"is
authorized to provide social services to individuals and families of individuals through its agents,County
Departments of Social Services, and
WHEREAS,County is authorized to purchase certain services for eligible children under State Department
rules,and
WHEREAS,certain Federal changes regarding Medicaid and related rates have occurred and the State of
Colorado has been granted an extension by the Centers for Medicare and Medicaid Services,until July 1,2006 to be
in compliance with Federal rules;and
WHEREAS,County and the Contractor wishes to amend its original agreement by extending the duration of
the Original Agreement entered into on "v cjc 9,7 ,2005;and
WHEREAS,Contractor is licensed as a Residential Child Care Facility or is a certified Residential Treatment
Center.
NOW THEREFORE,it is hereby agreed that in consideration of the mutual Undertakings County and
Contractor agree as follows:
1. The term of the Agreement shall be extended from January 1,2006,to June 30, 2006.
2. This Amendment,together with the original Agreement,constitutes the entire understanding among the parties
with respect to the subject matter hereof,and may not be changed or modified except as stated in the original
Agreement.
IN WITNESS WHEREOF,the parties hereto have duly executed the Agreement as of the day,month,and
year first above written.
