Metro Ms. Feldt was involved in a motor vehicle accident on October 24, 2005 at
303-623-5010 approximately 12:35 p.m. Ms. Feldt was driving eastbound on 37th Street at 65th
Avenue, Greeley, Colorado when she was hit by a Weld County vehicle driven by
Toll Free Dale Mikkelson; address 1305 12th Avenue, Greeley, Colorado 80631 and/or 915
866-356-5515 10th Street, Greeley,Colorado 80631. Mr. Mikkelson did not stop for the stop
sign and broadsided Ms. Feldt. Mr. Mikkelson was cited for disregarding traffic

employment with Weld County at the time of the accident.

At the time of the accident, Ms. Feldt,who was pregnant at the time of this accident,was taken
via ambulance to North Colorado Medical Center for treatment and monitoring. Ms. Feldt was
treated for back pain as a result of the motor vehicle accident. Ms. Feldt underwent testing to
reveal the health of her unborn child as well as physical therapy treatments. Ms. Feldt has been
limited in way of medical treatment throughout the duration of her pregnancy.
The actions or omissions of Dale Mikkelson constitute negligent operation of a motor vehicle,
in that he failed to stop at a stop sign and failed to keep a look out for Ms. Feldt to avoid a
collision.
The nature and extent of the injuries to Ms. Feldt are unknown at this time. Ms. Feldt has
suffered injury to her back and neck and may in the future endure pain, suffering and
permanent impairment as a result of the above-mentioned accident. The total of medical
expenses associated with the care and treatment of Ms. Feldt resulting from the above
described incident exceeds $1,500.00. The total of monetary damages asserted is: $7,500.00.
