We strongly recommend that all materials be reviewed prior to filling out the application. Please recognize that this is a very competitive process, with GOCO receiving requests for approximately four to five times the amount of funding available in each grant cycle. GOCO strongly encourages each applicant to contact staff prior to submitting an application. Contacting GOCO local government staff to discuss your project and the application process will ensure that the application is complete, clear, and concise. Applications must be received in the Great Outdoors Colorado office no later than March 3,2006. Grant applications will be evaluated first on compliance with eligibility requirements and then on responses to selection criteria. Although this section is for information only, please read this section entirely. If you are seeking an exception to any of these requirements, you must contact GOCO staff prior to submitting your application. 1. Project Type: Local Parks and Outdoor Recreation projects must fit into one of the following four categories: Park Land Acquisition Funding will be used to acquire land to be developed into a future park or to expand an existing park. A due diligence process will be required. GOCO will not participate in any acquisitions over fair market value, as determined by a public agency appraiser, OR projects involving condemnation. Applicants interested in applying for GOCO funds to assist in the acquisition of land for future development must complete the Local Parks and Outdoor Recreation Land Acquisition application. This document is different than the document that is before you. Applicants are encouraged to contact staff with questions. New Park Development Funding will be used to create a park and/or environmental education facility where none currently exists. Enhancement of Existing Facilities Funding will be used to enhance or improve current park facilities and/or environmental education facilities. This project type includes the installation or creation of new facilities at an existing park. Fairground Development Funding will be used to enhance outdoor recreation facilities at fairgrounds, i.e. grandstands, outdoor arenas and restroom facilities. Spring 2006 Great Outdoors Colorado Local Park and Outdoor Recreation Grant Application 3 ELIGIBILITY CRITERIA (FOR INFORMATION ONLY) 2. Eligible Applicants: Municipalities, counties, and/or parks and recreation special districts are eligible to apply for Local Government funds. However, the eligible entities mentioned above are encouraged to co-sponsor a project with other public, non-profit, and private agencies. For example, a city may apply on behalf of a local school district. 3. Matching Requirements: All matching resources must be eligible costs (e.g. administrative costs, such as mileage, cannot be paid for by GOCO, or used as match). For some examples of eligible/ineligible costs, refer to the table on the previous page. GOCO will not fund more than 70% of the proposed project's eligible costs. In other words, of all the resources required to complete the proposed project, including the requested GOCO grant, a minimum of 30% must come from sources other than GOCO. Although total matching resources may be either cash or in-kind (see below for more information regarding in-kind match), a minimum of 10% of the total project costs must be a cash match from the applicant and/or partners. In-kind contributions may include donated quantifiable professional services, materials and supplies, equipment, etc. "Volunteer," or non-quantifiable, non-professional time, may not be used as an in-kind contribution, but should be included as part of the Selection Criteria under"Non-Cash Support". Land value: The value of land purchased specifically for the purpose of the project within 3 years of the grant decision date may be used as a cash match. The value of land donated specifically for the purpose of the project within 3 years of the grant decision date may be used as in-kind match. Both instances will assist in satisfying matching requirements only when the land will be developed for GOCO eligible uses (public outdoor recreation or environmental education facilities). The value of the land used to meet the matching requirements may only be used one time. The historic cost, or value at time of purchase or donation, may be used as match. Land donations from one government entity to another cannot be used as match. Design, Architectural and Engineering Costs. Fifty percent (50%) of the total design, engineering, planning or architectural costs may be used as part of the applicant cash or in-kind match. GOCO funds should not be used to pay for these expenses. For example, if you have estimated spending $5,000 on design and engineering for the project $2,500 could be used as match in your budget. Please identify the total design/architectural or engineering as a footnote on the budget (i.e. 50% of$5,000= $2,500). GOCO funds may not be used for these costs, so please list them in the applicant or partner column of the budget. 4. Project Timeline: • Work on the proposed project may not begin until after the grant agreement has been signed by LOCO and the grantee. However, the timeline should begin with the Board decision date (June 20, 2006). Grants are allowed up to two years form the award date for completion. Any proposed changes, including extensions or early completion, must be requested in writing and approved in advance by GOCO. Projects not completed in a timely manner are subject to the Overdue Grants Policy and may be deauthorized. Spring 2006 Great Outdoors Colorado Local Park and Outdoor Recreation Grant Application 4 ELIGIBILITY CRITERIA (FOR INFORMATION ONLY) 5. Property Ownership: All property on which GOCO funded projects take place must be under control of the applicant, without encumbrances, for at least 25 years (deed or long term lease). Should the property happen to be owned by a third party(e.g. a school district), an Intergovernmental Agreement between the applicant and the property owner will be required for this application. 6. Grant Request: The maximum grant request is $200,000. There is no maximum for the total project cost. • • Spring 2006 Great Outdoors Colorado Local Park and Outdoor Recreation Grant Application 5 ELIGIBILITY CRITERIA (REQUIRED ATTACHMENTS) Please read this section entirely and submit the following items. Failure to include all of required documentation may result in ineligibility of your project for this grant cycle. Please label your attachments so they can be easily identified. 1. Budget and In-Kind Forms. Complete and submit the Budget Form (Page 10) using the Sample Budget (Attachment A) as a guide. Please review the matching requirements listed on the previous page before completing this form. Also, please be sure to double check that the calculations presented in the budget are accurate and mirror the figures presented in the narrative. When completing your budget, be as detailed as possible in listing the"use of funds". For example, if you are applying for funds to develop a park, which will include a play system, sod, irrigation, a picnic pavilion and landscaping, do not simply put one line item titled"park development" with a value of$200,000. Breakout the project components and the cost per component, i.e. site preparation=$5,000, sod = $8,000, irrigation system purchase and install=$, toddler play structure=$, 5-12 year-old play structure=$, etc. This level of detail will be helpful in completing the response to the matching funds selection criteria question. This also allows GOCO staff to get their arms around the financial side of the project and provides a background as to how the total project cost was arrived at. If in-kind contributions will be included in your budget, complete and submit the In-Kind Form (Page 11) using the Sample In-Kind Form (Attachment B) as a guide. 2. Timeline Form. Complete the Timeline Form (Page 12) using the Sample Timeline (Attachment C) as a guide. Please review the project timeline requirements listed on the previous page before completing this form. Staff encourages applicants to allow adequate time to complete the proposed project. If the applicant anticipates completing the project within a year of the award date, staff suggests that the 15-16 months is allowed, according to the timeline. 3. Letter from Governing Body of the Applicant Organization. Attach a letter from the governing body with primary jurisdiction (Mayor, City Manager, etc.); use the sample letter(Attachment E) as an example. Demonstrate each of the following: • Support for the proposed project • Maintenance capability A statement verifying that the proposed project will be properly maintained after completion. Also, be sure to provide the estimated annual maintenance cost from the entity responsible for maintaining the project. • Status of property (review the property ownership requirements listed on the previous page) A statement verifying that the property being used for the proposed project will be under control of the applicant for at least 25 years. (If the grant is awarded, a copy of the property deed or long-term lease will be required). If the property is owned by a third party(e.g. a school district), an UNSIGNED DRAFT intergovernmental agreement or memorandum of understanding between the applicant and the property owner must be referenced in the letter and attached to this application. Use the Sample Intergovernmental Agreement (Attachment D) as a guide. Describe your staff expertise and financial resources that will be used to complete this project, and similar information about partners who will assist you in completing the project. Do you have outstanding grants from Great Outdoors Colorado (from any grant program) or the State Parks Trails program that have passed their original or extended completion date? Brief Description of Project (In 250 words or less, describe the components of the proposed project): The town and school district serves an abnormally large area geographically. The only facilities that are in existence are located on school grounds. Currently, the sports complex, which consists of a baseball/football field, is utilized by the entire community as well as the sports teams at the high school. Within the community, we have several youth and adult sports teams and organizations that have utilized the facilities in the past. The current facility is in such poor shape that our community has moved their sporting activities and operations to a vacant dirt lot in a location adjacent to our current facility. Our grant request consists of funding to rectify the following situations. First, we are going to replace to top foot of topsoil throughout the field. We also plan to install a sufficient irrigation system. The next issue to be addressed is to sod the entire facility. Then to improve the current project, it is the desire of the community to add lights to the field, construct a walking/running track, acquire and install additional bleachers and finally add a scoreboard to the perimeter of the field. It is the hope of community that our new and improved facility will not only serve the need of the community, but will allow further development to our struggling community sporting and recreational activities. Because of our centrally located rural area,many feel that our facility would be utilized by surrounding rural communities as well. 12. Will Conservation Trust Fund dollars be used in this project? Eligible projects will be rated on a 100-point system based on the following criteria. The maximum points possible for each section have been shown in parentheses. Please keep your answers to the following selection criteria at or under seven pages, keeping your answers clear and concise. The seven page limit does not include attachments, site maps, lease agreement/deed, etc. Each project will be reviewed by outside reviewers and one GOCO staff, and projects will be ranked according to reviewer averages and staff score. On your own paper, please respond to each section, being sure to identify which question you are answering. Failure to respond to sections may reduce the project's score due to the difficulty of evaluating the project without this information. Please reference all attachments. I. Characteristics: (15 points) Provide a brief description of your community, highlighting its uniqueness. Describe, in detail, the scope, nature and significance of what will be accomplished by this project. Discuss how the project significantly improves parks and outdoor recreation facilities in your community and region. Provide a narrative description and a site plan* identifying and detailing the existing site amenities and proposed project elements. Describe each project component separately and discuss the need for each component. II. Need: (15 points) Clearly address any unmet need and explain how this project will specifically meet those needs. Provide information on similar facilities, or the lack there of, in the surrounding area. What, if any, other parks are available with the community? Discuss the ways in which the community currently is or is not compensating for the lack of the proposed project. Do children visit neighboring communities to swing, slide, etc.? Supply information regarding the number of current and proposed users, user groups and/or programs that similar facilities support. Discuss how the project will impact existing programs and/or create new program opportunities, i.e. new t- ball and girls softball programs, serving 100 children, will be established as a result of the softball field. Estimate the number of users of the proposed project. III. Urgency: (10 points) Discuss why the project needs to happen now. What opportunities will be lost if it is not completed? Will applicant and/or partner funding earmarked for this project be lost or spent elsewhere should this project not receive GOCO's assistance this cycle? How has demand for this project been a catalyst for its completion? Are participants being turned away as a result of limited space? Will project momentum cease to exist? IV. Multiple Uses and Accessibility (10 points) Multiple Uses: Describe how the proposed project will fill more than one need in the community, and explain the multiple uses of the project. Think outside of the box. For example, in building a soccer field you will also create an open area to fly kites, throw a frisbee, play kickball, etc. Also discuss how the project may benefit the various user groups in town, i.e. the local school, senior citizens, daycare providers, etc. What new opportunities and/or activities will be possible because of this project? Also, include narrative and a community map* illustrating the location of the site in relation to other community facilities (i.e. schools, city hall, recreation centers, subdivisions, etc.). By what means (car, bike path or trial, bus route, on foot, etc.) can users access the park? Will the completion of your project enhance accessibility to other outdoor recreation opportunities and/or facilities in the area? Is the proposed project near the recreation center, local school or community center? Attach a local or regional map* highlighting access routes. V. Public Process and Plan (5 points) Demonstrate how the proposed project fits in an adopted parks and recreation master plan for the community or region. Attach a copy of relevant portions of the plan with the application. If the area does not have an approved parks and recreation master plan, discuss and document how the need for this project was determined. Include information regarding community planning and decision-making procedures associated with the project. Discussion should focus on the methodology used (public meetings, surveys, petitions, etc.) to determine need for the proposed project. Is the proposed project part of a GOCO funded planning grant? VI. Matching Funds (20 points) Provide detailed narrative of your Budget Form. List each funding source, the amount contributed and details describing how the funds will be spent (e.g. GOCO dollars in the amount of$15,000 will assist in purchasing and installing the playground equipment, etc.). Carefully review the budget and matching requirements as stated in the Eligibility Criteria on page 3. An inaccurate budget will significantly reduce the points awarded for this section. VI. Partnerships (20 points) Discuss the partnerships established for this project. Who are your partners? How critical are these partners to the success of the project? How were the relationships fostered and how, financially or otherwise, have these partners contributed to the planning and implementation of this project? If partnerships are not possible please explain. GOCO encourages the use of the Colorado Youth Corps to work on eligible components of the project. Documentation from the Youth Corp, outlining their involvement, must be submitted with the completed application. VIII. Non-Cash Support (5 points) List and describe the contributions that will be provided to the project other than those necessary to meet GOCO's minimum required match. Include non-cash partnerships (such as volunteer work) with state agencies, municipalities, environmental organizations, schools, businesses, individuals and/or non-governmental groups, that are not already included as part of your in- kind match. Support letters from each of the individuals/entities mentioned are recommended, however, please limit the total number of support letters and/or commitment letters to 15. Recitals 1. [County] has applied for and received a grant from Great Outdoors Colorado, (GOCO) for[type of project and locationl; 2. The District is an ineligible recipient of the grant and the parties intend by this agreement for the County to be the conduit through which the District will receive the benefit of the grant; 3. The Grant Agreement is attached to this agreement as exhibit"A"; 4. The District intends to bind itself to the County for all of the County's obligations stated in the Grant Agreement; 5. The District intends to convey to the County a limited interest in the real property described in Exhibit "B" which limited property interest shall be for the purposes of satisfying the terms and conditions of the Grant Agreement; Therefore, in consideration of the mutual promises stated below and other valuable consideration, the parties agree: II. Agreement 6. The County shall use its best efforts to fulfill all the conditions precedent to obtain the grant stated in the Grant Agreement. The District will cooperate with the County and provide all documents necessary for the County to fulfill the conditions precedent. The District further assumes all other County liabilities, and binds itself to the County for all the County's obligations to GOCO, contained in the Grant Agreement. 7. The County does not assume any obligation to the District to construct, operate, or maintain the improvements contemplated by the grant. 8. Unless a claim by GOCO arises out of the negligence or other wrongful act of the County, the District shall be responsible to the County for any claim under the Grant Agreement, in the same manner and extent as the County may be responsible to GOCO. 9. The District shall operate and maintain the improvements contemplated by the Grant Agreement, in accordance with established District policy for playground maintenance. Should any claim for personal injuries, property damage or wrongful death be asserted as a result of the construction, operation, maintenance, or use of the improvements contemplated by the Grant Agreement, the parties shall be responsible for such claim in the manner provided by the Colorado Governmental Immunity Act and the Colorado law concerning pro-rata liability. By executing this agreement the parties do not waive any immunity or limit liability contained in the Colorado Governmental Immunity Act; do not create a multi-year fiscal obligation; and do not create any other financial obligation not supported by a current appropriation. 11. This agreement does not create any rights in any individual not a party to this agreement. 12. This document, and exhibits, shall constitute the entire agreement of the parties. 13. The District hereby grants to the County a limited license in, and right of entry to, the property described in Ex. "B" for the purposes stated in the Grant Agreement, Ex. "A", and for no other purpose. Such license and right of entry shall be exercised only in the event the District has failed to comply with the requirements of the Grant Agreement and shall include all rights reasonably necessary, as determined by the County, for the County to enter upon the property and perform its obligations to GOCO under the Grant Agreement. This right includes the ability of the County to use its employees, agents or outside contractors. This license and right of entry further includes the right to enter upon the property with any equipment or vehicles. 14. This agreement, including the limited right of entry and license, shall terminate simultaneously with the termination of all County obligations under the Grant Agreement. The Town of Protivin is applying for funds to complete the first neighborhood park in the town in 20 years. The Parks and Recreation Commission of the Town of Protivin, appointed by the elected members of the Protivin City Council, proudly supports the development of Prairie View Park. As residents and representatives for the citizens of this community, we feel it is important to continue to improve the quality of life in Protivin and provide diverse recreational opportunities. As a Commission we participated in and support the 2004 Protivin Parks and Recreation Master Plan, which calls for the development of neighborhood parks for the town's youth. The park site is owned by the Town, (attachment B). The Town in its annual budget will appropriate funds for operations and maintenance of the park. The Town of Protivin has been very successful in completing capital projects. We completed the downtown curb and gutter project as well as the outdoor swimming pool in 2002 and 2003. We hope that you will look favorably upon our request.
