RE: APPROVAL OF EXTENSION OF BUSINESS LEASE BETWEEN CONCORDIA
CORPORATION AND NORTH COLORADO MEDICAL CENTER AND AUTHORI-
ZATION FOR CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County,
Colorado, pursuant to Colorado statute and the Weld County Home
Rule Charter, is vested with the authority of administering the
affairs of Weld County, Colorado, and
WHEREAS, Concordia Corporation has tendered a Business Lease
to the Board of Trustees of North Colorado Medical Center, which
lease covers certain property located at 1801 17th Street, Greeley,
Colorado, and which lease proposes to house the Family Practice
Residency Training Program for the term commencing May 15, 1982
through May 15, 1983, and
WHEREAS, the Board of County Commissioners of Weld County,
Colorado deems it in the best interests of Weld County to approve
the execution of said lease.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Com-
missioners of Weld County, Colorado that the lease between the
Concordia Corporation and North Colorado Medical Center, a copy
of said lease being attached hereto and incorporated herein by
reference, be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be,
and hereby is, authorized to sign same.
The above and foregoing Resolution was, on motion duly made
and seconded, adopted by the following vote on the 1st day of
September, A.D. , 1982 , nunc pro tunc May 15, 1982.

EXTENSION OF BUSINESS LEASE

THIS AGREEMENT made this day LoL lii., ),
1982 , between CONCORDIA COMPANY (formerly CONCORDIA ,CORPORATION) ,
Lessor and the BOARD OF TRUSTEES,'OF THE NORTH COLORADO MEDICAL
CENTER, both of Greeley, Colorado, Lessee, Witnesseth:
RECITALS
A. The Parties entered into a Business Lease, dated
the 7th day of March, 1979 , covering the premises located at
1801 17th Street, Greeley, Colorado. This lease expired by
its terms on the 15th day of May, 1982 .
B. The Parties desire and have agreed to extend said
lease and all of its terms, except as if hereinafter noted, from
May 15, 1982 to May 15, 1983 .
Now Therefore, in consideration of the mutual promises
of the Parties, it is understood and agreed as follows :
I. The foregoing Recitals are incorporated into this
Lease and made a part hereof.
II. Lessee agrees to pay a monthly charge of $4201.67
dollars, plus 52% of the actual cost of the utilities, taxes and
insurance.
III. As modified by this Extension, the Business lease,
between the Parties, dated the 7th day of March, 1979 , shall
renew automatically, from year to year, unless either Party
provides the other with written notice, by registered mail,
return receipt requested, on or before November 15 of each lease
year, that it does not intend to renew the lease when the existing
one-year term expires.
IV. Any renewal of the lease hereafter will be subject
to a potential rent increase, as unilaterally determined by the
Lessor, and the Lessee will be advised in writing by the Lessor
of the scheduled rent increase, if any, by the 15th day of
February of the lease year.
In the event the Lessee finds the scheduled rent increase
unacceptable and so notifies the Lessor in writing within ten
days of Lessee ' s receipt of the notice of rent increase, Lessee
may, if it desires, remain as a tenant in the demised premises
until August 15 of the year in which the lease expires, the
rental to be paid by the Lessee during the period between May 15
and August 15 shall be the same as it has paid during the lease
year which expired on May 15 .
IN WITNESS WHEREOF, the parties hereto have executed
these presents on the day and year first above written.
