RE: APPROVE TASK ORDER CHANGE ORDER LETTER #3 FOR TUBERCULOSIS
PROGRAM AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Task Order Change Order Letter #3
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the Colorado Department of Public Health and Environment, commencing
July 1, 2006, and ending June 30, 2007, with further terms and conditions being as stated in said
Task Order Change Order Letter, and
WHEREAS,after review,the Board deems it advisable to approve said Task Order Change
Order Letter, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that Task Order Change Order Letter#3 between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Weld County Department of Public Health and Environment, and the Colorado Department of
Public Health and Environment be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said Task Order Change Order Letter .
The above and foregoing Resolution was,on motion duly made and seconded, adopted by
the following vote on the 16th day of April, A.D., 2007, nunc pro tunc July 1, 2006.

SUBJECT: Change Order Letter#3 for the TB
Program
Enclosed for Board review and approval is Task Order Change Order Letter#3 between Weld
County and the Colorado Department of Public Health and Environment for the Tuberculosis
Program.
This change order letter will provide additional funding of$4,500 for the TB program during the
current term of the task order, July 1, 2006 through June 30, 2007. With this funding the total
amount of the task order for this term will be $56,013 plus $12.50 per onsite visit and $25.00 per
field visit for directly observed therapy.
Weld County Department of Public Health and Environment staff members provide active TB
treatment which includes directly observed therapy; ensuring contacts to all newly identified
infectious TB cases are identified and receive appropriate evaluation; collecting specimens for
testing on all persons suspected of having TB; providing for and interpreting chest x-rays; and
conducting TB skin test screening programs. Health Department staff also follow-up with active
patients to ensure they receive appropriate information and education and they adhere to the
completion of their TB therapy.
I recommend your approval of this change order letter.

This Task Order Change Order Letter is issued pursuant to paragraph F. of the Master Contract identified as
contract routing number 05 FAA 00054 and paragraph C.4. of the Task Order identified as contract routing number
05 FHA 00017 and contract encumbrance number PO FHA EPI0500017. This Task Order Change Order Letter is
between the COLORADO DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT and BOARD OF
COUNTY COMMISSIONERS OF WELD COUNTY(a political subdivision of the state of Colorado.)The
Task Order has been amended by Task Order Option to Renew Letter L contract routing number 07 FHA 00019,
Task Order Change Order Letter 2,contract routing number 06 FHA 00230,Task Order Option to Renew Letter L
contract routing number 06 FHA 00002,and Task Order Change Order Letter 1, contract routing number 05 FHA
00264. The Task Order, as amended,if applicable, is referred to as the"Original Task Order". This Task Order
Change Order Letter is for the current term of July 1,2006,through June 30,2007. The maximum amount
payable by the State for the work to be performed by the Contractor during this current term is increased by FOUR
THOUSAND FIVE HUNDRED DOLLARS,($4,500.001 for an amended total financial obligation of the State of
ONE HUNDRED SEVENTY THREE THOUSAND FIVE HUNDRED FORTY FOUR DOLLARS,
($173,544.00). The revised specifications to the Budget are incorporated herein by this reference, made a part
hereof,and attached hereto as"Attachment N". The first sentence in paragraph C_1.of the Original Task Order is
modified accordingly. All other terms and conditions of the Original Task Order are reaffirmed. This change to the
Task Order shall be effective upon approval by the State controller, or designee, or on April 16,2007,whichever is
later.
Please sign,date,and return all 3 originals of this Task Order Change Order Letter by April 11,2007,to the
attention of Pam Pergande,Colorado Department of Public Health and Environment,4300 Cherry Creek
Drive South,Mail Code DCEED A-3,Denver,Colorado 80246. One original of this Task Order Change Order
Letter will be returned to you when fully approved.
