RE: GRANT PERMIT FOR TEMPORARYASSEMBLYTO FRANK'S RIDE FOR CHILDREN,

WHEREAS,the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,Frank's Ride for Children,Go Greg Risedorf,2544 Jarrett Drive,Mead,Colorado
80542, has presented the Board of County Commissioners with an application for a Permit for
Temporary Assembly on land within the unincorporated portion of the County of Weld, State of
Colorado, to be held from 6:00 a.m. until 10:00 p.m., on May 20, 2007, on property described as
follows:
Dandy Acres Painted Horse Farm, 1763 Weld County
Road 20.5, Longmont,Colorado,80504;being further
described as part of Section 18, Township 2 North,
Range 68 West of the 6th P.M., Weld County,
Colorado
WHEREAS, said applicant has paid to the Weld County the sum of ONE-HUNDRED
DOLLARS ($100.00) for said Weld County Permit for Temporary Assembly, and
WHEREAS,having examined said application, the Board deems it appropriate to grant a
Weld County Permit for Temporary Assembly to Frank's Ride for Children, c/o Greg Risedorf, and
WHEREAS,said permit shall be issued subject to compliance with the terms and conditions
as set forth in the application materials and upon the express condition and agreement that the
temporary assemblage will be conducted in strict conformity to all laws of the State of Colorado,and
WHEREAS,any violation to said terms and conditions shall be cause for revocation of said
permit and forfeiture of bond.
NOW,THEREFORE,BE IT RESOLVED by the Board of County Commissioners of Weld
County,Colorado,that said Weld County Permit for Temporary Assembly on May 20,2007,be,and
hereby is, granted to Frank's Ride for Children, c/o Greg Risedorf, to conduct a temporary
assemblage at the hereinabove described location,in accordance with the terms and conditions set
forth in the Permit and as set forth in the memorandum of Dan Joseph, Environmental Health
Specialist, Weld County Department of Public Health and Environment, dated April 10, 2007.

of the Weld County Code, to protect against injury to persons or property occurring as a
result of such an assembly.
23. Submit$100.00 fee for each eventwhere three-hundred fifty(350)or more people assemble
or can reasonably by anticipated to assemble at any one(1)location, unless the location is
properly zoned for such assembly, in which case one permit shall be required for the total
number of events scheduled for that location for the reminder of the calendar year, not to
exceed ten (10) events per year. As used herein, the term event means an assembly or
anticipated assembly which is scheduled to last for three (3) consecutive days or less.

LETTER OF CONSENT
I Jesse Thomas, do hereby authorize the use of my property for the sole
purpose of hosting the event known as Frank's Ride for Children. This event
benefits Make-A-Wish foundation of Colorado and is scheduled for Sunday
May 20th 2007.
I do understand that there will be approximately 1,000 — 1,500 people
attending, as well as, staff members from Frank's Ride and Make-A-Wish
Foundation of Colorado. It is also my understanding that there will be a pig
roast (served with all the trimmings), alcoholic/non alcoholic beverages, as
well as, live entertainment on the event premises.
My property is completely fenced with one main gate serving as the main
entrance to the event premises.

Pursuant to the Weld County Code, a public hearing will be held in the Chambers of the
Board of County Commissioners of Weld County, Colorado, Weld County Centennial Center,
915 10th Street, First Floor, Greeley, Colorado 80631, at the time specified.
If a court reporter is desired, please advise the Clerk to the Board, in writing, at least five
days prior of the hearing. The cost of engaging a court reporter shall be borne by the requesting

Events Liquor License for LC0022
In response to your request, Environmental Health Services has reviewed the Special
Events Permit and Temporary Assembly Permit, LC0022, for Frank's Ride for Children
2007, located at 1763 Weld County Road 20.5, Longmont, Colorado.
The following are the recommendations made by this Department:
1) Temporary food service guidelines be followed (attached).
2) Temporary hand washing stations be located outside the portable toilets. In
addition, provide temporary hand washing stations that are accessible only by food
preparation staff.
3) In the application it is stated that there will be 7 portable toilets available for the
estimated 1,000-1,500 participants. Based on this number of participants we are
recommending that the event have at minimum 12 portable toilets.
4) That the applicant submit a Temporary Event Vendor Application for food,with
the intent that this department can provide guidance and assistance to ensure that
the food is being served in a safe and sanitary manner (application provided).

towels that are dispensed, have a cooler with spigot that provides constant flow of water,powered hand
washing sink,etc... Note: Hand sanitizers may not be used as a replacement for hand washing).
20. How will you prevent Bare Hand Contact with ready to eat foods (You cannot touch any
product that will not be cooked with bare hands)?
21. What type of Sanitizer are you going to use Bleach/Quaternary Ammonia(products like 409
and Lysol are not approved for use)?
22. Do you have test strips for the sanitizer you are using (bleach should be white, Quaternary
Ammonia strips are orange in color)?
