HomeMy WebLinkAbout20070813 RESOLUTION RE: APPROVE GRANT PROPOSAL FOR THE DENTAL,VISION,AND HEARING PROGRAM - NORTH COLORADO MEDICAL CENTER FOUNDATION WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS,the Board has been presented with a Grant Proposal for the Dental,Vision,and Hearing Program from the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services,Area Agency on Aging,to the North Colorado Medical Center Foundation,with terms and conditions being as stated in said grant proposal, and WHEREAS, after review, the Board deems it advisable to approve said grant proposal, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Grant Proposal for the Dental, Vision, and Hearing Program from the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, Area Agency on Aging, to the North Colorado Medical Center Foundation be, and hereby is, approved. The Weld County Area Agency on Aging is submitting this proposal to the North Colorado Medical Center Foundation, it is ourdesire to obtain a $ 10,000 grant to further expand services rendered via the DentalNision/Hearing Program. This program enables needy Weld County residents who are sixty (60) and older to apply for dental, vision and/or hearing financial assistance. In fiscal year 2006-2007, the NCMC Foundation awarded the Weld County Area Agency on Aging $ 7,000.00. It is my privilege to report we have committed fundingto assistance nineteen (19) recipients. Dental recipients count at ten (10), vision five (5), heaing three (3) closing with one individual granted funding for two services (dental/vision). With assistance from the North Colorado Medical Foundation, we will continue to reach older adults who can benefit from this growing program. Most Sincerely, Kathleen Alvarado, Coordinator Weld County Area Agency on Aging- DentalNision/Hearing Program (970) 353-3800 Ext: 3314 Al North Colorado Medical Center Foundation Grant Request: March 2007 General Information The Weld County Area Agency on Aging is a support system for Weld County residents who are sixty (60) years of age and older as well as their caregivers. The Area Agency on Aging is a county agency in the Human Services Department,governed by the Weld County Board of Commissioners and receives support from a sixteen(16) member community advisory board. The primary goal of the agency is to help older individuals maintain independence and dignity in their home and community. Support services for older adults are provided directly by the Area Agency on Aging or through contracts with community agencies. The Area Agency on Aging is a help source for older adults and their children seeking information and assistance about vital senior services. Some of the services provided directly by the Area Agency on Aging include Legal Assistance, Senior Nutrition, Dental/Vision/ Hearing Program, Chore Program, Family Caregiver, Long Term Care Ombudsman and Case Management. Description and Purpose of Program/Project In 1998, the Weld County Area Agency on Aging implemented a Dental, Vision and Hearing Program. Funding for this program and other like programs was received from the State of Colorado (Older Coloradan Fund)to the local Area Agencies on Aging. The Weld County Area Agency on Aging allocated funds to assist older adults with dental/vision/hearing services that they were unable to receive due to their limited income. The Area Agency on Aging saw the value for a dental, vision and hearing program because of the tremendous need and benefits it would provide the older adults in our community. The purpose of the program is to help low income Weld County residents age sixty (60) and older who are in need of monetary assistance for dental,vision and hearing services and/or assistive devices such as glasses, dentures or hearing aids in order to remain healthy and independent in the community. We believe that this program is in line with NCMC Foundation's mission of enhancing the health of the community and its citizens. The recipients and services provided through this program are tracked using SAMS 2000, our agency software system. Supportive Research In September of 2004 a state-wide Strengths and Needs Assessment of Older Adults was completed. The Area Agencies on Aging across the state were able to obtain detailed information regarding their specific region. Based on the information obtain from the study over 2,500 Weld County seniors stated that they needed but could not afford eyeglasses, hearing aids or dentures. We anticipate that the need will continue to grow 1 as the population increases since we do not anticipate programs such as Medicaid covering these types of service needs in the future. Key Weld County provider agencies such as Salud Dental Health Clinic, Sunrise Dental Clinic, Catholic Charities Northern, and private providers, continue to express ongoing need for dental, vision and hearing services and provide a steady stream of referrals to the program. In addition, dental/vision/hearing program coordinator and other WAAA staff are made aware of the ongoing need when attending community meetings such as the North Adams/South Weld County Resource Meeting and attending events such as the Annual Weld County Health Fair. Seniors also call us directly regarding their needs after they become aware of the program. To increase the community's awareness of the program public service ads, targeting north and south Weld County, have appeared in such newspapers as the Senior Marketplace News. Population Served The funding for this grant will continue to be used to provide dental/vision/hearing services to low income Weld County residents age sixty (60) and older who are unable to receive assistance from any other payment source. In fiscal year 2006-2007, the NCMC Foundation awarded the Weld County Area Agency on Aging $7,000. We were able to commit funds to provide dental/vision/hearing services for nineteen (19)recipients. The actual types of services the clients have received are as follows: ten (10)clients received dental services; one (1) client received both dental and vision services; five (5)vision services rendered,then closing with three (3)hearing services provided. The average dollars allocated per client for dental services $536.00, $341.00 vision and hearing $ 400.00; however not all clients may utilize the full allocation, and funds maybe recaptured to assist additional recipients. Amount Requested Due to the funding received from North Colorado Medical Foundation 2006-2007, we have been able to enhance our Dental, Hearing and Vision program in Weld County. As previously stated in last year's proposal "The older adult population of Weld County is expected to grow by 13,442 or 60%between 2000 and 2012 (NRC Inc, 2004). In an effort to meet the growing needs of seniors, the Weld County Area Agency on Aging is requesting $10,000 from the North Colorado Medical Center Foundation for fiscal year 2007-2008. We firmly believe that there is a need for these services; however,we do not have the funds to actively pursue this growing need alone. 2 Committed Funding Sources The DentalNision/Hearing Program receives the majority of its funding from the Older Coloradan Fund (state only dollars). In State Fiscal Year 2006, $ 25,000 was allocated for the dental/vision/hearing program. In 2006, the Dental/Vision/Hearing Program also received a Community Health Investment Program (CHIP) grant for $10,000 from Platte Valley Medical Center in Brighton, Colorado. The funds from this grant were restricted by the funding entity for seniors in need who live in the southern regions of Weld County. The NCMC Foundation grant dollars have been extremely helpful in meeting the needs of northern area Weld County seniors, but especially for those clients that have had an emergency situation(i.e. bleeding gums or pain/infection) and we were able to address the needs immediately. Unfortunately we still have a Wait List of thirty (30) seniors who are in need of services but have not been able to access services due to our funding limitations. Please note, an unofficial listing of fourteen (14) additional seniors is maintained. Should an individual drop from the official listing then another senior is invited to seek financial assistance for services from us. The Area Agency on Aging staff, senior organizations and individual seniors continue to advocate on a state and national level for improved funding levels. The Older Coloradan Coalition's flyer which spot-lighted a Patricia Harper, a Weld County recipient of the Dental/Vision/ Hearing program is an example of a major advocacy effort that recently occurred to increase state funding for senior services. We will continue to view the Older Coloradan Fund as our primary source of funding for the DentalNision/Hearing Program. However, with the funds from the North Colorado Medical Center Foundation, we have been able to reach out to many additional older adults. We hope that you will continue to view the program as needed and important to the community and fully fund this 2007 funding request of$10,000. On a final note, when Ms. Harper was asked what the program meant to her, she stated "It has been a few months now, and it is so good having teeth. As is the case with all elected county officials, commissioners must be at least 21 years of a have resided in the county for at least one year prior to their election. As a board, the commissioners are required to establish a budget, levy taxes, appropriate funds for business, establish fees and licenses, purchase or otherwise acquire, own, sell, lease, trade, and of deal in real and personal property in the name of the county, receive gifts and grants for the count' approve and execute contracts and lawsuits in the name of the county. The board also is required to sit as a board of appeals to hear complaints on actions taken by count boards, commissions and departments, as a board of equalization to hear appeals on tax assessme a county board of welfare, and to make appointments to fill vacancies in all county offices except tl board of commissioners and the county council. Each commissioner has primary responsibility for one of the county's five departments and serves i assistant in another. The five departments are communication services, health services, planning si engineering services, and finance, administration, central purchasing, and personnel. Their respons rotate each year. The chairman of the Board of County Commissioners is chosen by the commissioners each year an( always serves as chairman of the finance and administration department during his tenure as chair the board. Commissioners are only allowed to serve two complete elected terms under charter provisions. The board meets at 9:00 a.m. each Monday and Wednesday in the first floor meeting room at the Centennial Center Complex, 915 10th Street, Greeley.
