The original permit numbers for these two wells were 35063-F and 35062-F. These permit numbers have been amended to be 60471-F and 60470-F together comprising Public Water System ID #16525. Well #242268 is no longer in use. The process to abandon this well will be completed with confirmation to be sent to your office. The Cottonwood Falls PUD is regulated by the Colorado Primary Drinking Water Regulations (5 CCR 1003-1). As such, once Cottonwood Falls PUD regularly serves a population of at least 25 people over 6 months of the year it is required to contact the Colorado Department of Public Health and Environment, Water Quality Control Division to activate the Cottonwood Falls Public Water System. At that time the Public Water System must be under the responsible control of a Class "D" Water Treatment Plant Operator and a Class "1" Water Distribution System Operator. Wm. Fronczak's November 24, 2003 letter from the SEO's office to Kim Ogle indicates GKD had submitted well permit amendment applications for 35063-F and 35062-F to serve all 13 lots. Kevin Rein's January 11, 2007 letter from the SEO's office to Chris Gathman indictes it could not be determined if the concerns raised in the SEO's November 24, 2003 letter had been addressed. Gary Dejohn's February 21, 2007 letter to the SEO's office indicates wells 35063-F and 35062-F have been amended to 60471-F and 60470-F, and that these two wells will serve the 13-lot subdivision. The letter also indicates Well 242268 is no longer in use and that this well will be abandoned. Gary Dejohn's February 21, 2007 letter to WQCD indicates the two wells, 60471-F and 60470-F, will serve the 13-lot subdivision under PWSID# 162525, and that the number of people currently using water from the system is 13. My September 8, 2003 letter to John Kadavy regarding approval of Residential Well No. 3 (Permit No. 242268). This letter indicates the PWS will serve 14 homes within and adjancent to the subdivision. —Here are questions that should be resolved: 1) Were the well permit amendment applications referenced in Wm. Fronczak's letter approved? 2) If Well 242268 is to be abandoned, where will the 1 home adjacent to the 13-lot subdivision get its water? This lot was one of the 14 homes to be served. 3) The PWS is currenity inactive in our database. Does the owner understand that it must activate the PWS once the population served reaches 25 persons or the number of connections reaches 15, and that at such time the PWS must be under the responsible control of a Class "D" Water Treatment Plant Operator and a Class "1" Water Distribution System Opertor? The applicant shall submit to the Department of Planning Services written proof that the owner of the house located along WCR 69, on property outside of the Cottonwood Falls PUD, is provided the opportunity to connect to the water system of said PUD. Said opportunity shall be honored by the Cottonwood Falls PUD Home Owners Association and shall extend to the owner's successors in interest. The owner and/or his successor in interest shall be responsible for the entire cost of said connection and any associated maintenance therewith. This requirement shall be shown as a note on the plat.
