Home My WebLink About20071565.tiff RESOLUTION RE: APPROVE TASK ORDER CHANGE ORDER LETTER#1 FOR NURSE HOME VISITOR PROGRAM AND AUTHORIZE CHAIR TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS,the Board has been presented with Task Order Change Order Letter#1 for the Nurse Home Visitor Program between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Department of Public Health and Environment, and the Colorado Department of Public Health and Environment, commencing July 1, 2006, and ending June 30, 2007, with further terms and conditions being as stated in said change order, and WHEREAS, after review, the Board deems it advisable to approve said change order, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Task Order Change Order Letter #1 for the Nurse Home Visitor Program between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Department of Public Health and Environment, and the Colorado Department of Public Health and Environment be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to sign said change order. Wallace, MD, MPHII P Director, Department of Public H 1 L and Environments COLORADO DATE: May 29,2007 SUBJECT: Task Order Change Order Letter for the Nurse Home Visitor Program Enclosed for Board review and approval is a task order change order letter between the Weld County Board of Commissioners on behalf of the Department of Public Health and Environment (WCDPHE) and the State of Colorado through the Colorado Department of Public Health and Environment (CDPHE) for the Nurse Home Visitor Program. Through this change order letter the CDPHE is reducing funding for the 2006-2007 Nurse Home Visitor Program by $43,945. The reductions are a result of Weld County staff vacancy savings in the amount of$31,540 during the current term of the program, July 1, 2006 through June 30, 2007 and of$12,405 higher than anticipated Medicaid payments to WCDPHE during the 2005- 2006 contract year. With the above reduction of$43,945, the total financial obligation of the State for the time period July 1, 2006 through June 30, 2007 will be $413,454. I recommend your approval of this change order letter.
