RE: APPROVAL OF EQUIPMENT LEASE BETWEEN COULTER LEASING CORPORA-
TION AND WELD COUNTY GENERAL HOSPITAL AND AUTHORIZATION FOR
CHAIRMAN TO SIGN THE SAME.
WHEREAS, the Board of County Commissioners of Weld County,
Colorado, pursuant to Colorado statute and the Weld County Home
Rule Charter, is vested with the authority of administering the
affairs of Weld County, Colorado, and
WHEREAS, an equipment lease between Weld County General
Hospital and Coulter Leasing Corporation concerning the leasing
of a Coulter S Plus has been presented to the Board of County
Commissioners of Weld County, Colorado, and
WHEREAS, the term of said lease is for 5 years and the
monthly lease cost is $2 , 771. 75 , and
WHEREAS, after studying said lease, the Board deems it
advisable to approve the same.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Com-
missioners of Weld County, Colorado that the equipment lease
between Coulter Leasing Corporation and Weld County General
Hospital be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman of
the Board be , and hereby is, authorized to sign said lease.
The above and foregoing Resolution was, on motion duly made
and seconded, adopted by the following vote on the 26th day of
March, A.D. , 1980.

SUBJECT: Coulter S Plus Lease Contract
The Coulter S Plus was requested by the Laboratory in the 1980 budget process to
replace our existing Coulter S Automated Blood Counter leased in 1973. The new
Coulter will do all the same tests more efficiently (using less reagents) and
also Platelet Counts. The lease is for 5 years. The capital cost of the equip-
ment is $77,650.00. The monthly lease cost is $2,771.75 which includes $194.25
for the instrument, $391.00 for service costs, and $439.50 for reagents.

PROPOSAL:
Obtain signature on lease contract with Coulter Electronics, Inc. for a new
Coulter Model S Plus which will replace our existing Coulter S Automated
Blood Counter which was leased in 1973. The new lease is for S years.
PURPOSE & EXPLANATION:
The upgrade was approved in the 1980 budget process and in addition to doing all
present tests more efficiently as the reagent costs are less (approximately $200
a month) , it also will do platelet counts. Our present lease terminates on April
1, 1980. The new equipment is here and the Coulter is requesting that the lease
be signed by March 31, 1980.
Capital cost of equipment is $77,650.00.

. For the purpose of maintaining equipment in good operating condition,COULTER ELECTRONICS,INC.'agrees to
•perform maintenance service at rates and frequency indicated below: y zr
1. The Customer will be•contacted to schedule the first Preventive Maintenance Inspection after receipt of the signed
contract.
2. For Subsequent Preventive Maintenance schedule,refer to Section I. A ❑ BO CO DO
3. Schedule Preventive Maintenance Inspections will include;Refer to Section II. A ❑ B ❑ C❑ D ❑ E❑ F ❑

4. Emergency service calls necessary between inspections,due to instrument failures,will be performed at no extra
charge.
5. All calls necessary between inspections, not attributed to instrument failure (to include clause No. 12),will be
charged at 2/3 the applicable hourly service charge.
6. Loan instruments will be provided at all times whenever customer's unit has to be repaired at the Service Center,
free of charge:
7. Where distance prohibits the presence of a Service Engineer, collect phone calls will be accepted by the Service
Department and emergency service advice will be given. If it is deemed necessary to ship the instrument to the
Repair Center,Coulter Electronics will pay for all freight charges on the loaner equipment only.,'- ,,. . ,
8. If loaner instrument is not returned to service center within (10)days after repaired unit is returned to owner,a
charge will be made for each day loaner is not returned unless special arrangements are made with the service de-

10. Time must be provided by the customer to perform Preventive Maintenance Inspections during normal working +,

This Agreement becomes effective only under the following Terms and Conditions
11. This equipment must be inspected by an Authorized Coulter Service Representative to insure that it meets
Coulter Electronics Service Contract Acceptability Standards. Instruments that do not meet with Service Con
tract Acceptability Standards will be reconditioned on order by the customer at commercial rates, less 20%,and
then accepted under this agreement.
12. This agreement shall not apply to any instrument damaged through accident,misuse,theft,neglect,fire,Acts of '-.
God,or deterioration caused by chemicals that are not used for normal unit operation b •-' -'.„ •

13. On any anniversary date of this agreement, COULTER ELECTRONICS, INC,may eliminate from the agreement
