WHEREAS,the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with an Application for the Dental,Vision, and
Hearing Program Grant from the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Department of Human Services, Area
Agency on Aging,to the Colorado Health Foundation,with terms and conditions being as stated in
said application, and
WHEREAS,a hearing before the Board was held on the 25th day of June, 2007, at which
time the Board deemed it advisable to continue said matter to June 27, 2007, at 9:00 a.m., and

Executive Summary
One of the more common conditions faced by many people as they age is the
decline and loss of hearing. Saxon and Etten (1994) report that 60% of people over the
age of 65 have some hearing impairment. Estimates of continued impairment escalate to
as much as 90% when those reach 80 (Saxon and Etten, 1994, p 89). In Weld County,
2303 or nine percent of the population over 60 lives in poverty (Colorado Department of
Local Affairs, 2006). Additionally, the Colorado Department of Local Affairs (2006)
projects this population to grow 60.1% between 2000 and 2012. With 9% of the 60+ age
group in Weld County living in poverty, they often lack adequate resources for treatment
or to acquire a hearing device. Because Medicare does not cover hearing treatments,
those experiencing hearing loss will be subject to increased medical risks and potential
costs associated with falls due to balance difficulties. Scholarship points out that
subsequent social withdrawal often leads to depression and other forms of psychological
conditions (Aldwin and Gilmer, 2004). Fire alarms, smoke detectors, and many safety
monitoring devices in and out of the home make elders with hearing loss vulnerable to
life threatening events.
During this fiscal year, July 2006- June 2007, 67 seniors received services from
Weld County AAA-Dental/Vision/Hearing Program. The Weld County Dental, Vision,
and Hearing Program Coordinator, now has 68 seniors, 7.3% of which seeking hearing
services, wait-listed because of inadequate resources.

The purpose of this grant is to assist minority populations in Northern Colorado
with hearing loss. Fortunately, hearing loss and the health conditions associated with loss
of hearing is manageable. Like the Colorado Health Foundation's mission, our primary
concern is to help maintain an individual's independence and dignity in their home and
community.
The hearing program wait-list reduction project has two objectives: (1) the
primary goal of the hearing project grant is to reduce the number of hearing impaired
seniors in Weld County, Colorado. Without treatment, hearing impaired seniors face
significant injury risk and psychological isolation. (2)A secondary goal of this project
focuses maintaining independence for hearing impaired seniors by enabling them to
remain home and reduce the need for institutionalization or high care giver needs.
Weld County AAA hearing, vision, and dental program has served this population
by providing lower income seniors with support and services for many years. For 17
years since initial implementation the program has increased the number of citizens it
serves. During the last ten years from 1997- 2007, the program has served 172 seniors. In
the current fiscal year, 67 have been supported and 68 continue on the wait-list. The
demand continues to accelerate. The Weld County Hearing, Vision, and Dental
program's efforts is another step in preserving seniors continued independence and
quality of life.
Three year program cost is expected to be $171,554.64. This proposal is for
$32,000. Remaining funds for the program comes from grants by State of Colorado AAA
funds, Northern Colorado Medical Center Foundation, and the Community Health
Investment Program sponsored by the Platte Valley Medical Center.

Need Statement
One of the more common conditions faced by many people as they age is the
decline and loss of hearing. Saxon and Etten (1994) report that 60% of people over the
age of 65 have some hearing impairment. Estimates of continued impairment escalate to
as much as 90% when those reach 80 (Saxon and Etten, 1994, p 89). Besides the loss of
communication, research has indicated that, "hearing loss is thought to be the most
devastating sensory handicap of all, frequently resulting in withdrawal from
interactions with family and friends" (Saxon and Etten, 1994, p 85). Aldwin and
Gilmer (2004) maintain in their research Health, illness, and optimal aging:
Biological and Psychosocial Perspectives, which ones balance can be more
problematic. Often, hearing impairment will result in falls which leads to additional
orthopedic conditions and bone fractures. These scholars argue that decrements in
hearing can have a profound effect on an older person's quality of life (Aldwin and
Gilmer, 2004).
For a segment of older people hearing loss management can be easily restored by
a hearing examination and the addition of some hearing aid device. Elders with adequate
resources usually manage hearing loss comfortable. However, because Medicare does not
cover the costs associated with hearing aids, there is a population at risk who lack the
resources to acquire this effective hearing tool. This population,predominantly women
and minorities, often go without hearing examinations and are subject to the wide
range of morbidities and psychological conditions associated with hearing loss.
My project seeks a grant to assist a minority population in Northern Colorado
with hearing loss. In Weld County, 2303 or nine percent of the population over 60 lives

By awarding this grant to fund treatments and hearing aids when required, the
Weld County Area Agency on Aging can meet more of the hearing loss demands from
their served elder population in Weld County. As stated earlier in this request, 67 seniors
received services, dental, vision and/or hearing thorough the WCAAA-D/V/H Program.
Presently 68 seniors, 7.3 % of which seek hearing services, are a waiting July 2007's date
to apply for the program. The DN/H Program, in 99 % of cases, pays for the audiogram.
Dependent upon whom the recipient selects as a provider, new/refurbished and medical
need, a hearing aid unit cost begins at refurbished, $200 - $350; then new $500 - $ 1,200.
Our program allows for some flexibility, pursuant to available funding, to apply
additional funding towards purchase of hearing aid. Program cap is $350.00 per hearing
aid unit. Hearing loss and the health conditions associated with loss of hearing is
manageable. Some may argue that the cost of hearing treatments are more cost effective
than treating one's medical condition resulting from a hearing loss injury.

The Weld County Area Agency on Aging (AAA) has an active portfolio of
services for older adults. Their published mission from their web site stated, "The Weld
County Area Agency on Aging is a support system for Weld County residents 60 years of
age and older and their caregivers. At the macro level, The Colorado Health Foundation's
Philanthropy program's mission and goals align with those of Weld County's AAA. The
Foundation's strategy of improving access to quality health care is a shared interest of
the Area Agency on Aging. Both organizations look to coordinate the delivery of
physical health, improve organizations ability to deliver care effectively and
efficiently, and to strengthen the health care workforce, particularly among
underrepresented populations. Our primary concern is to help maintain an individual's
independence and dignity in their home and community. The Area Agency on Aging
supports services for seniors either through contracts with community agencies or by
providing the service directly. These services include adult day care, case management,
home delivered meals, legal assistance, nursing home and assisted living ombudsman
services, peer counseling, rural senior centers, senior nutrition programs, Hispanic elderly
outreach, and a minor dental, vision and hearing program. The Area Agency on Aging
also is a Help-Source (clearing house) for seniors and may be contacted for a wide range
of information and referral questions" (Weld County, 2006).

Because AAA provides case management, coordination of a small dental, vision
and hearing program, and a number of other elders' services, they have active
involvement with the population this grant intends to benefit. In addition, AAA's
reputation within the community for service is excellent. Minority and Hispanic citizens
make up 14% of the 60+ Weld population which lends itself to a denser minority pool of
hearing impaired than other Colorado locations (Colorado Dept of Public Health, 2005).

Goals and Objectives
The primary goal of the hearing project grant is to reduce the number of hearing impaired
elders in Weld County, Colorado. Because of AAA's limited funding for their hearing
program, as many as 50% of the need each year is wait listed until the following year.
Without treatment, hearing impaired elders face significant injury risk and psychological
isolation. A secondary goal of this project focuses maintaining independence for hearing
impaired elders by enabling them to remain home and reduce the need for
institutionalization or high care giver needs. The project proposed has two goals directed
to address hearing impaired elders in Weld County.
Outcome Objectives
1. Reduce by 25% the number of elders in Weld County that are waitlisted while
seeking hearing treatments.
2. Reduce the frequency of injury and potential added morbidities attributed to
hearing impairment by 50%.
Process Objectives
1. Weld County AAA to grant services for elders on their waitlist based on
vulnerability. Reduce hearing waitlist by half in 2007-2008 fiscal years.
2. Weld County will have case managers contact 100% elders receiving hearing
treatments to ascertain their level of independence and injury since treatment.

Methods
To accomplish the objectives stated in the previous section, the hearing program will use
the following methods. This choice to follow this course of action is because of the
success of Weld Counties AAA existing hearing program processes already established.
A time line is in the appendixes: it provides the planned starting and ending dates for
each activity.
Objective One
Reduce by 25% the number of elders in Weld County that are wait-listed while seeking
hearing treatments.
Methods
• Expand the scope and work hours of the dental, vision and hearing program
coordinator who will have the added responsibilities to coordinate and support the
increased request and processing of wait listed hearing impaired elders.
• Hearing coordinator will contact all wait listed elders who seek hearing treatment.
All wait listed elders will be revaluated to reconfirm financial and hearing
treatment needs.
• Program coordinator will amend outreach strategies to acquaint targeted elders
with the new program within the first 3 months
• Program coordinator will arrange hearing treatment for the most vulnerable to
least vulnerable from a current hearing practitioners list used by the AAA.
• Each elder hearing need will be prioritized based upon medical costs for
treatment. ( average rate estimate of$350 per treatment)

• Services will be performed by hearing practitioners within the first six months of
the program for all confirmed elders identified, as funding permits.
• Program coordinator will evaluate progress of treated elder at 6 and 9 month and
document findings.
Objective Two
Reduce the frequency of injury and potential added morbidities attributed to hearing
impairment by 50%.
• As hearing coordinator contacts all wait listed elders seeking hearing treatment
(item two in objective one), elders will be evaluated to determine existing health
morbidities that are attributed to hearing impairment.
• As the treated population pool is established, hearing coordinator will establish a
baseline for all additional physical morbidities.
• After hearing treatment, and at 60 and 90 day intervals, hearing coordinator will
contact elders and care giver to ascertain level of health.
• Rates of improvement/decline will be compared to initial baseline information
established in step one above.
• Information will be documented in charts and summarized.
• From the baseline data, the post-treatment data summary will be compared against
the 50% standard for the intervention program.
• Levels of independence or improvement by treated elder from the initial baseline
results will included within evaluation processes.

Evaluation
Within 60 days, the hearing program coordinator will contact the waitlisted elders
in Weld County as stated in the proposal. The two goals and the two objectives of the
hearing program will establish a baseline for data measures. Data collection will begin
with each treated individual as treatment occurs. Quantitative data points on treatment
date, treatment costs, treatment completion date, and hearing related morbidities will be
established. At six month interval post treatment, qualitative data will be collected and
measured against the pretreatment points. Program coordinator will collect and document
all baseline and post treatment data. The waitlist reduction and hearing morbidities
reduction effectiveness measures will be provided to each funder at the six month and
annual timeframes. Additionally, funder reports evaluating the program objectives
against program proposed standards will be identified. The hearing project will be
considered a success when noticeable reductions of hearing impaired elders are removed
from the Weld County's hearing program waitlist. Based upon program successes and
program effectiveness, the Weld County hearing program will seek additional grant
support to continue the program in subsequent years.

Budget Narrative
The projected program budget for the next three years is $ 171, 554.64 includes
The Health Foundation grant of$ 32, 000. Below are the committed and pending
revenue sources:

By the Colorado Health Foundation (CHF) awarding this grant, this project will reduce
the number of those waitlisted that are hearing impaired sixty plus seniors. The attached
Budget will provide a breakdown of Proposal Revene, Proposal Expenses, and other costs
related to the successful functioning of this successful nine year program.
Please note that the program is run with a number of in-kind services: personnel,
administrative/operating, hardware, software, travel/mileage/conferences/meetings and
training as well as consultants.

Family Caregiver Support
The Family Caregiver Support Program offers support,
resources and information to assist family caregivers
in finding relief and assistance through a variety of
respite and support programs.
Eligibility: Care recipient must be 60 years or older.
Home Delivered Meals
Specially prepared, nutritious noon time meals are delivered to
the home five days per week. Frozen meals available for
weekends.
Legal Assistance
The Legal Assistance Program provides legal representation in
civil matters such as Medicare, Medicaid, Social Security,
housing, consumer complaints and estate and nursing home
planning. Eligibility: Must be 60 years or older.

Long Term Care Ombudsman
The Ombudsman protects the human and civil rights of seniors
residing in nursing homes and personal care boarding homes.
Eligibility: Must be 60 years or older.
Peer Counseling
Individual and family counseling services help clients cope with
the emotional stresses related to aging and are provided in the
clients home by trained counselors who are also seniors.
Rural Senior Centers
Nineteen (19) rural senior centers throughout Weld County
provide a range of services including meals, trips, information
on services, fun and educational opportunities and socialization.
Each center is staffed by a senior coordinator. These programs
are for mature adults with no particular age limitation.
Senior Nutrition
The Senior Nutrition Program provides nutritious and delicious
noon-time meals for seniors 60 and older served at twenty-two
sites throughout Weld County. Social and educational
opportunities are also available before and after meals. Call for
information on site locations in Weld County. Eligibility: Must be
60 years or older or the spouse of someone 60 years or older.
Hispanic Elderly Outreach
Spanish-speaking seniors receive help in their own language to
find community resources that they need.
