Attached are original copies of Change Order No. 1 for Work Order # 4 — WCR 74 and SH 257
Intersection Improvements — Construction Inspection, and Change Order No. 1 Work Order# 5 —
WCR 74 and SH 257 Intersection Improvements Materials Testing Administration.These have been
approved and signed by the Chair of the BOCC.
Please record these with the above referenced Work Orders Commission Action#2006-1105 (WO
#4) and 2006-1106 (WO #5)

Project: SH 257 and Weld County Road 74 Intersection Improvement. The project is located at the intersection of
SH 257 and WCR 74 north of Windsor and is funded by CDOT, HES Grant, Weld County, Town of Severance and
the Town of Windsor. The engineering consulting firm Felsburg, Holt and Ullevig (FHU) was selected as the
consultant for the project to provide engineering construction management services.
Owner: Weld County,Colorado
Consultant: Felsburg,Holt and Ullevig, 6300 South Syracuse Way, Suite 600, Centennial,CO 80111
The following change is hereby made to the Contract Documents:
o Additional services to the original Scope of Work were required. Additional services included:
Construction time extension required additional time to complete
scope of work
$2,233.56
Total Increase Cost $2,233.56
CHANGE TO CONTRACT PRICE:
Original Contract Price: $ 49,515.00
Current Contract Price adjusted by previous Change Order: $ 49,515.00
The Contract Price due to this Change Order will be increased by: $ 2,233.56
The new Contract Price, including this Change Order, will be$ 51,748.56
CHANGE TO CONTRACT TIME:
The Contract time will be increased by 6 calendar days.
