RE: APPROVE CONTRACT FOR CANCER SCREENING SERVICES FOR THE WOMEN'S
WELLNESS CONNECTION AND AUTHORIZE CHAIR TO SIGN - WOMEN'S
RESOURCE CENTER
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with a Contract for Cancer Screening Services
for the Women's Wellness Connection between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Weld County
Department of Public Health and Environment, and the Women's Resource Center, commencing
September 1, 2007, and ending June 30, 2008, with further terms and conditions being as stated
in said contract, and
WHEREAS, after review, the Board deems it advisable to approve said contract, a copy of
which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Contract for Cancer Screening Services for the Women's Wellness
Connection between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the Women's Resource Center, be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said contract.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 26th day of September, A.D., 2007.

SUBJECT: Women's Wellness Connection -
Contract for services
Enclosed for Board review and approval is a contract between the Weld County Department of
Public Health and Environment and the Women's Resource Center in Ft. Collins for cancer
screening services for the Women's Wellness Connection(formerly the Colorado Women's
Cancer Control Initiative) in Larimer County. The time period for the contract is September 1,
2007 through June 30, 2008 for the total amount of$30,000.00. Payment for these services will
be provided through the Women's Wellness Connection grant (CWCCI) from the Colorado
Department of Public Health and Environment previously approved by the Board.
The Women's Wellness Connection (WWC)provides funding for breast and cervical cancer
education and screening for the underserved, uninsured and underinsured residents in 12 counties
in the northeast region of the state, including Larimer and Weld. The Women's Resource Center
in Ft. Collins has participated in this initiative for several years. With this funding they will be
able to dedicate staff to identifying women eligible for WWC services in Larimer County, giving
the WWC Coordinator, based at Weld County Department of Public Health and Environment,
more opportunity to serve the residents of Weld County and the northeast region.

The Weld County Department of Public Health and Environment and the Women's Wellness
Connection (WWC) will sub contract with the Women's Resource Center for a Community
Coordinator to complete the Scope of Work outlined in this agreement for the fiscal Year 2007 —
2008 for Larimer County.

Project Period: September 1, 2007 through June 28, 2008.
Project Goal
To establish a partnership with Women's Resource Center to identify women eligible for WWC
based at the Weld County Department of Public Health and Environment. This will entail
providing and evaluating community-based cancer-screening education and navigation services
to Priority Population women residing in Larimer County who are age 40 but not older than age
65. All referrals and screening services will be provided by current WWC providers.
Reimbursement for Services: Weld County Department of Public Health and Environment/WWC
program will pay$3,000.00 each month for a total project budget of$30,000 during the remaining 10
months of the contract period.
