SUBJECT: 2007 Extension Distinguished Service Awards:
Alton Scofield,F. A.Anderson,Team,and Diversity Awards
We are pleased that the Colorado Cooperative Council will again sponsor the Alton Scofield Award and CSU
Extension will sponsor F.A. Anderson,Team and Diversity Awards recognizing outstanding service in Extension
education. The awards, consisting of a cash stipend and recognition plaque,will be presented at our Extension
Annual Forum on October 4,2007.
Your local observation of the activities of CSU Extension personnel is valued in learning of worthy candidates.
Over the years,excellent nominations have been submitted by county commissioner boards. We solicit your
nomination of Extension staff for the 2007 Alton Scofield,Cooperative Extension Team,and Diversity Awards.
Nominations should be sent to CSU Extension,Office of the Director,Campus Delivery 4040,Colorado State
University, Fort Collins,CO 80523-4040. Guidelines to be followed in the documentation of your nominations are
enclosed or are available at the CSUCE web site: http://www.ext.colostate.eduistafTres/nomination.html. We have
established a nomination deadline of Friday, August 3, 2007.

2007 DISTINGUISHED SERVICE AWARDS
FOR
F. A. ANDERSON AWARD
ALTON SCOFIELD AWARD
IN RECOGNITION OF SUPERIOR SERVICE
ELIGIBILITY FOR THE F. A. ANDERSON AWARD
Any member of the state staff who is by appointment at least 50% on a
CSU Extension assignment. Previous winners of this award are not
eligible for a second award for five years.
ELIGIBILITY FOR THE ALTON SCOFIELD AWARD
County, area, or regional CSU Extension personnel. Previous winners of
this award are not eligible for a second award for five years.
NOMINATIONS
Nominations are invited from:
organizations which work with CSU Extension staff:
Colorado County Agents Association, Colorado Extension
Association of Family and Consumer Sciences, Colorado
Association of Extension 4-H Agents, Epsilon Sigma Phi
and Colorado State University Extension Specialists
Association;
- academic departments, Extension administrators, and
regional directors;
- county commissioners; and
- Extension colleagues.
NOMINATION GUIDELINES
Those individuals or organizations wishing to submit nominations for the above
Distinguished Service Awards for 2007 should follow the following format:
The entire nomination (excluding letters of support and cover page) is
limited to five double-spaced, typewritten pages and must be organized
following this specific format:
Cover Sheet including:
A. Name of nominee; county/area, department, or unit; present
position and/or title of individual;
B. Name of Nominator (individual(s)/organization).
II. A narrative summary organized in the following sections:
Major Accomplishments -
A. The most outstanding 2-3 accomplishments of work in CSU
Extension. (20 points)
B. Impact of Responsibilities on Clientele - who benefited, how did
lives or communities change, implications for future programs. (25
points)
C. Evidence of Collaboration
community involvement, contribution to team efforts, campus
involvement, etc. (20 points)
D. Commitment to Diversity fostering an environment that respects
and values all people. (15 points)
E. Service to Extension - committees served on, leadership in
professional organizations and committees, service at local, state,
and national levels, etc. (10 points)
F. Awards and Recognition - by peers, clientele, others (5 points)
(Note: An additional 5 points will be awarded based on how closely the
nomination follows nomination guidelines)
III. A. A maximum of four letters of support, testimonials or other
evidence of impact - limited to a single page each.

IN RECOGNITION OF SUPERIOR SERVICE TO THE DIVERSE NEEDS OF
CLIENTEL THROUGH CSU EXTENSION EDUCATION
ELIGIBILITY FOR THE DIVERSITY AWARD
One or more CSU Extension staff members along with other county,
area, district, state, or interdisciplinary participants. Previous winners of
this award are not eligible for a second award for five years.
NOMINATIONS
Nominations are invited from:
organizations that work with CSU Extension staff: Colorado
County Agents Association, Colorado Extension Association of
Family and Consumer Sciences, Colorado Association of Extension
4-H Agents, Epsilon Sigma Phi and Colorado State University
Extension Specialists Association;
academic departments, Extension administrators, and regional
directors;
county commissioners; and
Extension colleagues.

Diversity Award for 2007 should follow the following format:

specific format:
I. A. Cover Sheet including: Name of nominee(s); county/area,
department, or unit; present position and/or title of individual or
team members; name of nominator (individual, organization, self).
II. A narrative summary organized as follows:
A. Situation Statement - including the situation that prompted the
program /effort, how diversity was defined, (20 points).
B. Description of the program/effort - including obstacles overcome,
innovation, length, sustainability. (25 points)
C. Diversity efforts - Way(s) the effort has fostered an environment
that respects and values diversity, cultural sensitivity and
pluralism. (20 points)
D. Significant achievements and impacts. (30 points)

support from other members of the staff and any other data that might
be helpful in assessing the nominee's (individual or team) qualifications.

2007 CSU EXTENSION TEAM AWARD
IN RECOGNITION OF SUPERIOR SERVICE
ELIGIBILITY FOR TEAM AWARD
This award recognizes teams which come together for a specific period of
time to work on a special initiative/goal. One or more team members
must be a CSU Extension employee. Teams who win this award are not
eligible for a second award for five years. However, individuals who are a
part of a winning team, are eligible to compete as a part of another team.
NOMINATIONS
Nominations are invited from:
- organizations which work with CSU Extension staff:
Colorado County Agents Association, Colorado Extension
Association of Family and Consumer Sciences, Colorado
Association of Extension 4-H Agents, Epsilon Sigma Phi,
and Colorado State University Extension Specialists
Association;
- Academic departments, Extension administrators, and
regional directors;
- County commissioners; and
- Extension colleagues.

Team Award for 2007 should follow the following format:

limited to five double-spaced, typewritten pages and must be organized as

Please submit nominations following this specific format:
I. Cover Sheet including:
A. Name of team and names of individuals involved; county/area,
department, or agencies/organizations represented; present
positions and/or titles of individuals. Please clearly identify those
Extension individuals who are responsible for the team.
B. Name of nominator (individual, organization, self)
II. A narrative summary in the following specific format which clearly
explains the role that the team's program has played in preparing
people or communities to deal with critical issues which address
social, economic, and/or environmental concerns.
Include:
* Situation Statement: Identification of need/issue. (10
points)
* Resources involved: team members, volunteers,
agencies/cooperators and funds, supplies or equipment
utilized. (15 points)
* Strategies: innovative or creative problem solving,
educational or delivery methods, etc. (15 points)
* Commitment to fostering diversity and an
environment that respects and values all people. (10
points)
* Evidence of collaboration to improve Extension's efforts
(i.e. community involvement, contribution to team efforts,
campus involvement, etc.). (10 points)
* Impacts: who benefited, what the impacts were, how lives
or communities changed, implications for future programs,
etc. (30 points)
* Awards and recognition of the team's effort by peers,
clientele, others (if any). (5 points)
