HomeMy WebLinkAbout20070812 RESOLUTION RE: APPROVE GRANT APPLICATION FOR COMMUNITY HEALTH INVESTMENT PROGRAM - PLATTE VALLEY MEDICAL CENTER WHEREAS,the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS,the Board has been presented with a Grant Application for the Community Health Investment Program from the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County,on behalf of the Department of Human Services,Area Agency on Aging,to the Platte Valley Medical Center,with terms and conditions being as stated in said application, and WHEREAS,after review,the Board deems it advisable to approve said grant application,a copy of which is attached hereto and incorporated herein by reference. This application, with at least one additional proposal page, and a copy of the following must be submitted to the CHIP Committee: 1) Names and addresses of applicant officers and members of its Board of Directors or governing body. 2) Articles of Incorporation and By-laws or other documents which define the applicant's purposes, membership, management and operation. 3) Statute, regulation or the applicant's Internal Revenue Service letter which shows the applicant is exempt from Federal income taxes, is not a private foundation and that donors may deduct contributions to the applicant as provided in Section 170 of the Internal Revenue Code. 4) A financial audit or other financial information for the most recent three fiscal years. 5) A detailed budget for the proposal. All applications must be submitted to the CHIP Committee no later than the month ending previous to scheduled CHIP Committee meetings (The Committee meets the second Wednesday every other month as follows: January, March, May, July, September and November). t o?C,,,-Cf/,, COMMUNITY HEALTH INVESTMENT PROGRAM GRANT REQUEST FOR PROGRAM YEAR BEGINNING AUGUST 1, 2007 AND ENDING JULY 30, 2008. PROPOSAL: The Weld County Area Agency on Aging has received assistance from the CHIP Committee since 2002. These funds have been used to enhance the services that are provided through the Area Agency on Aging's Dental, Vision and Hearing Program (DVH) in the South Weld County area. Over the last five years, the program has successfully reached at risk seniors who otherwise would have been unable to access these types of services due to their financial limitations. According to the August 22, 2006 article in The Greeley Tribune, the Greeley metropolitan area (encompasses all of Weld County) is the second fastest growing area in the United States. To place this growth into perspective from 2000-2005 the Greeley metropolitan area grew 26.5 percent while Las Vegas only grew 24.3 percent. These growth patterns are creating increase needs for DVH services currently and will create greater demands in the near future as the population continues to age. The Dental/Vision/Hearing Program is currently being funded, in part, by dollars the Area Agency on Aging receives from the State of Colorado known as State Funding for Senior Services (SFSS). The Area Agency on Aging also continues to seek additional funding partners such as the North Colorado Medical Center Foundation in order to try to meet the ever growing demands on the program. If funds are received from the NCMC Foundation in 2007-2008, these dollars will be used to help address the ever growing number of individuals on the Wait List. The Area Agency on Aging distributes the allocated dollars equitable across the county. Unfortunately the funding amount falls short of the multiple needs that exist in the community. The CHIP funds have allowed the Area Agency on Aging to serve additional number of senior who live in South Weld County. The Area Agency on Aging is requesting the continuation of funding at the rate of $10,000 for the Dental/Vision/ Hearing Program. The dollars, if received, will be spent providing dental/vision/hearing services for seniors living in South Weld County who are unable to access these services due to financial limitations. These dollars will not supplant the current level of state dollars that have been allocated to the south county region. DOCUMENTED NEED: In September of 2004 a state-wide Strengths and Needs Assessment of Older Adults was completed. The Area Agencies on Aging across the state were able to obtain detailed information regarding their specific region. Based on the information obtain from the study over 2,500 Weld County seniors indicated that they needed but could not afford eyeglasses, hearing aids or dentures. It is anticipated that the need will continue to grow 1 as the population increases and it is not anticipated that programs such as Medicaid will cover these types of service needs in the foreseeable future. Key provider agency staff members, located in the South Weld County, continue to express ongoing need for dental, vision and hearing services. In September of 2006, Hispanic Senior Outreach and Caring Heart (Friendly Visitor Program), which are programs of Catholic Charities Northern, coordinated respectively by Yolanda Garza and Connie Herman developed a fact sheet identifying the following needs: • Prescriptions, hearing aids, dentures and eye glasses/exams, medical equipment and other necessities they can not afford. Seniors in South Weld County have been instrumental in notifying other seniors about the services available. The Area Agency on Aging continues to receive requests for information on the dental, hearing and vision services from adult children whose parents reside in South Weld County. Seniors who have been successfully assisted with one of the three services in the past, find they need additional services contact us. This is a clear indication of how important these services are to seniors. Public service ads, targeting South Weld County, have appeared in the Senior Marketplace News providing information to seniors regarding the dental, vision and hearing program. The program coordinator continues to use the assistance of the Fort Lupton Senior Center and Salud Family Health Clinic in promoting the program. EXPECTED RESULTS OF THE PROGRAM: Historically, the goals for Program Years 2002-2003, 2003-2004, 2004-2005 and 2005- 2006 were met. The goal for the 2006-2007 funding cycle was to serve thirty (30) seniors. By the middle of January 2007, twenty (20) South Weld County senior residents have been found eligible and funds committed for assistance. Applications have been sent to interested parties, and I anticipate we will meet our program goal of thirty (30) seniors served. SPECIFIC OBJECTIVES, TIMETABLES AND PROCESS UTILIZED TO RESEARCH PROJECT GOALS: For Program Year 2007-2008, the goal is to enable seniors to obtain the services needed to maintain their health and a quality of life. The target goal is to assist thirty (30) seniors in South Weld County in 2007-2008. Coordination with community partners will insure that the dental, vision and hearing program will continue to be effectively marketed in the community. In addition, expansion of needed South Weld County area service providers will continue. 2 e HOW WILL RESULTS BE MEASURED? The participants and services provided through these programs are tracked using SAMS 2000, the Area Agency on Aging's data system. This system accurately tracks all the activity of the Dental/Vision/Hearing Program (number of service units, type of service provided, all client demographics and case notes). The program coordinator also seeks feedback from clients as to their level of satisfaction for services received. Below are three examples of feedback that the program has received: "Being able to maintain healthy teeth, in the long run, impacts my overall health, thus enabling me to avoid hospitalization" Male recipient. "Thank you, thank you, thank you! I really needed glasses,for sure!"Female recipient "It has been a few months now and it is so good having teeth. I chew my food better, so my stomach problems are gone. It is the best gift (my partial plate) I have ever received. Hope He blesses all that are involved in this project"Female recipient(See attached flyer that was distributed in the Colorado House of Representatives and Senate in January 2007) Due to the funding received from the CHIP Committee, the program has been able to reach additional numbers of seniors residing in South Weld County. The Program Coordinator has contacted providers in the area who have a genuine interest in assisting seniors in need of the program services. With the funds provided by the CHIP Committee, the State Funding for Senior Services, and other funders, the Dental, Vision and Hearing Program in South Weld County has been successful. The Program has enabled seniors to obtain the services needed to maintain their health and a quality of life. The Area Agency on Aging is asking the CHIP Committee members, to once again partner with us to meet the needs of South Weld County seniors, Program Year 2007- 2008 at a funding level of$10,000. Funds received from the CHIP Committee will be used as supplemental funds.
