RE: APPROVE AMENDMENT TO GENERAL ASSISTANCE AGREEMENT FOR SERVICES
AND AUTHORIZE CHAIR TO SIGN - WELD COUNTY ASSOCIATION OF
OPTOMETRISTS AND OPHTHALMOLOGISTS
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with an Amendment to the General Assistance
Agreement for Services between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Social Services, and the
Weld County Association of Optometrists and Ophthalmologists,commencing upon full execution,
with further terms and conditions being as stated in said agreement, and
WHEREAS,after review,the Board deems it advisable to approve said amendment, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, ex-officio Board of Social Services, that the Amendment to the General
Assistance Agreement for Services between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County,on behalf of the Department of Social
Services, and the Weld County Association of Optometrists and Ophthalmologists be, and hereby
is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 26th day of December, A.D., 2007.

County Association of Optometrists and Ophthalmologists and Weld County
Department of Social Services
Enclosed for Board approval is an Amendment to an Agreement for Services between Weld
County Association of Optometrists and Ophthalmologists (Association) and Weld County
Department of Social Services (Department). This Agreement was reviewed at the Board's Work
Session of December 17, 2007.
The reason for the Amendment is to increase the reimbursement level form $22,000 to $38,000.
All other provisions of the original Agreement remain the same including the term of the
Agreement of Calendar Year 2007.

AMENDMENT
GENERAL ASSISTANCE AGREEMENT FOR SERVICES
BETWEEN THE WELD COUNTY DEPARTMENT
OF SOCIAL SERVICES AND WELD COUNTY
ASSOCIATION OF OPTOMETRISTS AND OPHTHALMOLOGISTS
This AMENDMENT to General Assistance Agreement for Services made and entered into the 26 day of
December 2007, by and between the Board of County Commissioners, on behalf of the Weld County Department of
Social Services, hereinafter referred to as "Social Services," and "Weld County Association of Optometrists and
Ophthalmologists,"hereinafter referred to as the "Contractor."
WITNESSETH
WHEREAS,Social Services and Weld County Association of Optometrists and Ophthalmologists entered into a
General Assistance Agreement for Services(Contract Number PY07-GA-20)on March 10,2007,to provide services to
individuals determined by Social Services to be eligible for general assistance and referred for vision services.
WHEREAS,the monies provided under the agreement is not sufficient for the term of the Agreement(Contract
Number PY07-GA-20)to serve the vision services needs of General Assistance clients.
WHEREAS,Social Services desires to adjust its payment schedule to respond to the increased needs of General
Assistance clients.
NOW THEREFORE, in consideration of the premises,the parties hereto covenant and agree as follows:
1. Exhibit B,Payment Schedule,Item 1,Funding and Method of Payment,shall be amended as follows:
Social Services agrees to reimburse to Weld County Association of Optometrists and
Ophthalmologists in consideration for the work and services performed,a total amount not to exceed
Thirty-eight Thousand Dollars($38,000.00).
2. This Amendment,together with the original General Assistance Agreement for Services constitutes
the entire understanding among the parties with respect to the subject matter hereof,and may not be
changed or modified except as stated in the General Assistance Agreement.
IN WITNESS WHEREOF, the parties hereto have duly executed the Agreement as of the day, month, and year first
above written.
