(was Andesite Rock Company) USRs - Company Name Change on two USRs -VacateNoid USR and NCU numbers of sites which are no longer operating Dear Kim: We are submitting the following information to update our county records regarding our company name and for our Use-by-Special-Review permits. Company Name Change L. G. Everist, Inc. is no longer using the name Andesite Rock Company to do business. Andesite was always a fully-owned subsidiary of L. G. Everist, Inc., so this name change is simply that - a name change. The ownership and management of the parent company, L. G. Everist, remain the same. We are now conducting all business as L. G. Everist, Inc. A letter is attached, signed by Dennis Fields, a Vice President of L. G. Everist, Inc., confirming the company name change. We have two operations in Weld County which are currently operating under the Andesite Rock Company name, and will now be operating as L. G. Everist, Inc. sites. They are: AmUSR-840/USR-840 - known as the Carr Gravel Resource We are in the process of changing the operator name from Andesite Rock Company to L. G. Everist, Inc., with the Colorado Division of Reclamation, Mining, and Safety (DRMS, formerly called the Division of Mining and Geology, DMG). The state permit number for the site is M- 1988-048. When we get the state's approval letter, we will send a copy for your records. AmUSR-874/USR-874 -formerly known as the Camenisch Resource We have already changed the operator name and site name with the DRMS/DMG. The state permit number for the site is M-1997-093. We changed the operator name from Andesite Rock Company to L. G. Everist, Inc., and changed the site name from Phillip Camenisch Resource to Rinn Valley Resource. (was Andesite Rock Company) USRs - Company Name Change on two USRs - VacateNoid USR and NCU numbers of sites which are no longer operating VacateNoid USR numbers of sites which are no longer operating USR-1206, which had been USR-615, AmUSR-615, and 2ndAmUSR-615 can all be vacated/voided from the records. Also, a Non-Conforming-Use permit , NCU-9, was associated with the property. This permit can be vacated/voided also. The property, located in Section 3, Township 2N, Range 68W, was known as the Del Camino Pit, operated by Andesite Rock Company. The site was reclaimed and released by the state in August 2003. The state's release letter is attached for your reference. The property was then annexed into Firestone, and bought by Colorado State Parks. The property is now part of St. Vrain State Park. If we have missed any permit numbers associated with the any of sites listed above, or any other permits with Andesite Rock Company listed as the operator, please contact us and we will submit additional information in order to update your records.
