AUGUST 8, 2008
WHEREAS,the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, The Fort 21, LTD, dba The Fort 21, presented to the Board of County
Commissioners of Weld County,Colorado,an application for the renewal of a Tavern Liquor Liquor
License, for the sale of malt, vinous and spirituous liquors, and
WHEREAS,pursuant to Exhibit 5-H of the Weld County Code, said applicant has paid the
sum of$* to the County of Weld for the renewal of the existing license, and
WHEREAS,said applicant has exhibited a State Liquor License for the sale of malt,vinous
and spirituous liquors for consumption by the drink on the premises only,outside the corporate limits
of any town or city in the County of Weld at the location described as follows:

NOW,THEREFORE,BE IT RESOLVED that the Board of County Commissioners of Weld
County,Colorado, having examined said application and the other qualifications of the applicant,
does hereby grant License Number 2007-11 to said applicant to sell malt, vinous and spirituous
liquors for consumption by the drink on the premises or in sealed containers not for consumption
at place where sold,only at retail at said location;and the Board does hereby authorize and direct
the issuance of said license by the Chair of the Board of County Commissioners,attested to by the
Clerk to the Board of Weld County,Colorado,which license shall be in effect until August 8,2008,
providing that said place where the licensee is authorized to sell malt,vinous,and spirituous liquors
for consumption by the drink on the premises only,shall be conducted in strict conformity to all of
the laws of the State of Colorado and the rules and regulations relating thereto,heretofore passed
by the Board of County Commissioners of Weld County,Colorado,and any violations thereof shall
be cause for revocation of the license.
BE IT FURTHER RESOLVED that the Licensee shall ensure that all bartenders and wait
staff,present and future,shall attend the Training for Intervention Procedures(T.I.P.S.)program or
similar training to learn when to stop serving alcoholic beverages to intoxicated patrons. The
Licensee shall provide to the Sheriffs Office and the Clerk to the Board's Office, on an ongoing
basis,within thirty(30)days after hire or employment,copies of certificates showing the successful
completion of said training by the present bartenders and wait staff.

Subject: Fort 21, Ltd Liquor License Renewal
Sirs,
On July 23, 2007,I completed a liquor inspection at Fort 21, Ltd, 985 E. 18th St. The
only problem that was noted was the screw-off top for a bottle of Kahlua liquor was missing
from the bottle. As it was not known how long the cap had been gone,the owner/manager,
DeeAnn Cosner voluntarily poured the contents down the sink in my presence. This issue was
addressed with Ms. Cosner who will ensure the tops are on the bottles when not in use.
During my inspection, I did not find anything else that is of concern. I did not find any
indications of violations of the smoking ban inside the establishment.
In the last 12 months,there have been 6 calls-for-service at the Fort 21, one of those
being a business check during normal business hours. None of the calls were criminal in nature,
and none were related to underage consumption or over-service of liquor.
Based on the above information, I would recommend Fort 21,Ltd. for renewal of their
Tavern License.

for taverns or hotel/restaurant licenses after that:
