RE: APPROVE GRANT APPLICATION FOR AREA AGENCY ON AGING
DENTALNISION/HEARING PROGRAM AND AUTHORIZE CHAIR TO SIGN - POUDRE
VALLEY HOSPITAL FOUNDATION
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Grant Application for the
DentalNision/Hearing Program from the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Department of Human Services,
Area Agency on Aging, to the Poudre Valley Hospital Foundation, commencing July 1, 2007, and
ending June 30, 2008, with further terms and conditions being as stated in said application, and
WHEREAS, after review,the Board deems it advisable to approve said application, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Grant Application for the DentalNision/Hearing Program from the
County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Department of Human Services, Area Agency on Aging, to the Poudre
Valley Hospital Foundation be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said application.
The above and foregoing Resolution was, on motion duly made and seconded,adopted by
the following vote on the 9th day of July, A.D., 2007.

Enclosed for board approval is the Area Agency m Aging's funding request of$5,000 from the
Poudre Valley Hospital Foundation The funds will be used to enhance the Area Agency on
Aging's Dental, Hearing and Vision services to seniors 60 years of aT and older who reside in
Weld County.

Dear Ms. Lytle-Barnaby:
The Weld County Area Agency on Aging (AAA) is submitting this proposal to The
Poudre Valley Hospital Foundation for a$5,000 grant to fund and implement an
expansion of the Weld County's AAA Hearing, Dental, and Vision program. Expansion
of the current program will address the needs of seniors on the program's wait-list.
Certainly, treating these seniors will enable them to maintain their individual
independence, health and dignity within their homes and community.
This expansion, unanimously supported by the Weld County Advisory Board of
Directors, will reduce the current wait-listed seniors, numbering sixty seven (67). We
hope that you are interested in partnering with us to help enable older adults (60+) to
remain healthy and independent in the community.
All of us who serve older adults in Weld County express our gratitude for The Poudre
Valley Foundation's consideration and time.

I. HOW THE FUNDS WILL BE USED:
For Program Year 2008 (July 1, 2007-June 30, 2008), if awarded the $5,000,the
target goal would be to serve eight(8)to ten(10) individuals. According to the current program
guidelines the program is able to financially assist clients through a voucher process at the
following levels: dental-$600,00; vision-$350.00; hearing- $350.00. There are times that a client
will need more than one service from the dental/vision/hearing program and if funds are available
will be able to access assistance in the other need categories. If the full cost of the service
exceeds the financial assistance provided by the program the client is responsible for the
remaining balance and must work directly with the provider to establish a payment plan.The
program coordinator will assist the applicant in completing a Friends of Man application, if
appropriate and/or needed.
2. OTHER RESOURCES OF FUNDING/COLLABORATIVE EFFORTS:
The role of Area Agency on Aging is to foster collaboration with community entities to ensure
the success of new and established senior programs and to avoid unnecessary duplication of
services in the community.
To ensure the success of the dental/vision/hearing program we have collaborated with the
following agencies and community providers:the Weld County Department of Social Services,
Friends of Man, Catholic Charities Northern,the Sunrise Health Clinic, various civic
organizations, long term care social workers, local dentists, audiologists and ophthalmologists.
These agencies/providers are closely linked with seniors who may be in need of assistance with
accessing dental/vision/hearing services.
The dental/vision/hearing program is currently being funded by : Platte Valley Medical Center
Foundation,Northern Colorado Medical Center Foundation, and the Weld County Area Agency
on Aging.

4. HOW DOES YOUR ORGANIZATION IMPACT OUR COMMUNITY AND WITH
WHOM DO YOU PARTNER?
The Weld County Area Agency on Aging is mandated to provide services to seniors
(60+) who reside in Weld County. We are one of 16 Area Agencies on Aging in Colorado. We
provide senior services such as: the dental/vision/hearing program, chore program, senior legal
services, long term care Ombudsman services, homemaker, personal care, counseling, senior
meal program, etc. directly or through contracts with other service providers in the community.
Even though our primary focus is on seniors who reside in Weld County we also work with
clients who live in neighboring communities that have expanded and have grown over county
lines. We envision as the Loveland-Greeley area continues to grow we will see an increase in
number of seniors who will reach across county lines in search of needed services. The new
Medical Center of the Rockies in Loveland is a perfect example of an entity that will also be
serving clients in a multi-county region. We are eager to develop a partnership with Poudre
Valley Hospital Foundation so we can serve the needs of our mutual clients effectively.
Our current dental/vision/hearing program funding partners are: Northern Colorado Medical
Center Foundation and the Platte Valley Hospital Foundation. We are also in the process of
approaching other potential funding partners such as the Colorado Health Foundation.
5. DO YOU HAVE A 501 ( C) (3) OR FISCAL AGENT WITH 501 (C) (3) ?
No,we are part of Weld County Government.
6. WHAT IS YOUR OUTCOME MEASUREMENT FOR THIS PROGRAM ?
Initially a client's eligibility is determined through a formal intake process. During the intake
process the program coordinator determines the client's eligibility for the program and the
client's general dental/vision and hearing needs. Once the client has been determined eligible for
assistance he/she then assesses the appropriate service provider. List of service providers are
made available to clients if the client is in need of assistance locating a provider. The program's
primary outcome is met when a client receives the needed treatment and/or obtains the needed
dental appliances, glasses and/or hearing aides. In order to insure that the services are fully
rendered, the provider submits a summary of services provided to the program coordinator.
Client follow up is completed by the program coordinator by phone or in person, as appropriate.
In addition,all specific client information and services rendered are recorded and tracked in
SAMS 2000, a statewide data management system. A variety of program reports can be
generated from SAMS 2000 based on the needs of the funding entity as well as the needs of the
Weld County Area Agency on Aging..
7. HOW WILL THE UNDERSERVED BY SUPPORTED ?
The mission of the dental/vision/hearing program is to financially assist low income and at risk
seniors access critical dental/vision/hearing services. Due to agency's community outreach and
networking efforts we have been able to identify clients in greatest need. Unfortunately due to
the programs limited funds we are unable to assist every client who is in need of services. At this
time, services have been provided to sixty-seven(67) low income seniors with sixty-eight wait-
listed for the July 2007-June 2008 funding cycle.

In response to the funding limitations, the Weld County Area Agency on Aging staff, other
senior organizations, advocates and individual seniors are advocating on a state and national
level for improved Sate and Federal funding levels. The enclosed article entitled "A Face to
Senior Services" and the Older Coloradan Coalition's flyer which spot-lights a Wallace
Schreiber, a Weld County recipient of the DentalNision/Hearing Program is an example of a
major advocacy effort to increase state funding for seniors services. The flyer was placed on the
desk of each member of the House of Representatives and Mr. Schreiber attended the first session
of the House and was introduced to the members.

The Weld County Area Agency on Aging and the Weld County Area Agency on Aging Advisory
Board believe that the DentalNision/Hearing Program provides a needed service to many seniors
in Weld County. We will fund the program as long as we continue to receive dollars from the
State Funds for Senior Services and our funding partners. In order to supplement these funds we
will also continue to seek additional assistance from local and state level foundations.

Wallace Schreiber — Greeley, Colorado
Mr.Wallace Schreiber was born in Limon, Colorado seventy-seven years
ago. The eldest of four children, he grew-up on his father's ranch in
Arriba, Colorado. Mr. Schreiber grew accustomed to hard work on the kttli
ranch. He married; they had two children. With the passage of time, Mr. Ali

Schreiber became a proud grandfather.
In November 1996,Mr. Schreiber suffered an injury on a construction
job site. Hospitalization, then rehabilitation in a long-term care facility, ,
enabled Mr. Schreiber to use a motorized wheelchair.
With time, Mr. Schreiber made the move to a private residence. He f,
became a volunteer board member of Connections for Independent
Living of Greeley, an organization that works to promote independence
and equality for people with disabilities.
When Mr. Schreiber, with limited financial resources, needed critical
dental assistance, he was able to access the Dental,Vision and Hearing
Program of the Weld County Area Agency on Aging. The program,
coordinated by Kathleen Alvarado, is designed to maintain the independence and a healthy quality of life for
those who participate.
"Being able to maintain healthy teeth, in the long run, impacts my overall health, thus enabling me to avoid
unnecessary hospitalization,"commented Mr. Schreiber,when he was asked what the program meant to him.
